About this file

This is a Conflict of Interest Questionnaire (Form CIQ) issued by the Texas Ethics Commission for vendors conducting business with local governmental entities in Texas. The form is required under Chapter 176 of the Texas Local Government Code and must be completed by any vendor that has established a business relationship with a local government entity and meets specific disclosure requirements. The questionnaire serves as a disclosure mechanism to identify potential conflicts of interest between vendors and local government officers or their family members prior to contract execution or during contract consideration phases.

The form requires vendors to disclose employment or business relationships with local government officers or their family members that result in taxable income exceeding $2,500 during the preceding 12-month period, gifts with an aggregate value exceeding $100 provided to officers or their families within the same timeframe, and any family relationships with local government officers. Vendors must file the completed questionnaire with the local governmental entity's records administrator no later than the seventh business day after the later of either when the vendor begins contract discussions or negotiations, or when the vendor becomes aware of a triggering conflict of interest relationship. The form emphasizes that vendors commit a misdemeanor offense if they knowingly violate the disclosure requirements under Section 176.006 of the Local Government Code, establishing significant legal consequences for non-compliance or falsification of conflict information.
