About this file

This is a Location Certification form for Broward County, Florida, used to determine vendor eligibility for local business preferences and tiebreaker considerations in competitive solicitations, specifically related to an Artificial Intelligence-Enabled System for Employee Screening X-Ray Machines at Fort Lauderdale-Hollywood International Airport. Vendors must submit the completed certification form with all required supporting documentation by the solicitation end date or within three business days of a County written request to qualify for local preference points or to be considered for the location tiebreaker. The form requires vendors to certify their business status under one of five options: Local Business only; Local Business and Locally Based Business; Local Business and Locally Based Subsidiary; Joint Venture composed of qualifying local entities; or non-local business. All qualifying options require vendors to maintain a physical business address within Broward County limits for at least one year preceding the bid posting date, listed on a valid Broward County business tax receipt, and operate from that location on a day-to-day basis providing services substantially related to the solicitation.

Vendors selecting Options 2 or 3 must demonstrate that less than or equal to fifty percent of equity interests are owned by entities outside Broward County, that the local location serves as the primary business address for the majority of employees, and that management directs all substantial day-to-day activities from that location. Joint venture applicants (Option 4) must submit Broward County business tax receipts for each participating local entity and specify equity percentages on a separate sheet. The County may request additional documentation including proof of property ownership or use rights, corporate organization charts identifying parent entities, joint venture agreements, and other location-related documentation. Vendors must certify their intent to remain at the stated local business location for the entire contract term, including renewals. The form warns that false or misleading information may result in suspension, debarment, contract termination, and civil or criminal penalties.
