About this file
This is a Workforce Certificate Form issued by the State of Minnesota for use in conjunction with a request for proposal (RFP) from the Metropolitan Airports Commission for Uninterruptible Power Supply (UPS) maintenance services at Minneapolis-St. Paul International Airport (MSP). The form must be completed and submitted with bid responses for any contract expected to exceed $100,000. The solicitation requires vendors to provide personnel, labor, transportation, trucks, tools, and equipment necessary for UPS system maintenance at the MSP campus. A mandatory pre-proposal meeting and facility tour is scheduled for August 31, 2026, from 9:00 a.m. to 11:00 a.m. CST, with the meeting portion running from 9:00 a.m. to 10:00 a.m. followed by a tour.
The Workforce Certificate Form establishes compliance requirements based on vendor employment history. Vendors who have employed more than 40 full-time employees on any single working day in one state during the previous 12 months must submit either a current MDHR Workforce Certificate or confirmation of a pending certificate application. Vendors with fewer than 40 full-time employees are exempt from this requirement but must submit employee information to the Minnesota Department of Human Rights within five business days of final contract execution, including employee names, separation dates if applicable, and employment location. Vendors must contact MDHR at 651-539-1095 (metro), 1-800-657-3704 (toll free), or compliance.MDHR@state.mn.us with questions. The signatory certifies authorization, accuracy of information, compliance with federal affirmative action requirements, and acknowledges that false information may result in suspension or revocation of any resulting agreement.
