About this file

This is a Certified Drug-Free Workplace Report Coversheet issued by the West Virginia Purchasing Division for a Transitional Living Facility HVAC Replacement project (Contract Number ARFQ WSH2700000005) managed by the West Virginia Department of Health and Human Resources at William R. Sharpe, Jr. Hospital. The coversheet serves as a mandatory compliance document required by West Virginia Code 21-1D-7b, mandating that all contractors submit a certified drug-free workplace report no less than once per year or upon project completion. The document outlines the required report content that vendors must provide, including evidence of education and training services regarding West Virginia Code 21-1D-5 requirements, identification of the United States Department of Health and Human Services-certified laboratory conducting drug testing, the average number of employees engaged in the construction project, and comprehensive drug test results categorized by pre-employment and new hires, reasonable suspicion, post-accident, and random testing, with documented counts of positive and negative results for each category.

Submission requirements for the drug-free workplace report are contingent upon contract value, with contracts exceeding $25,000 requiring submission to the West Virginia Purchasing Division at 2019 Washington Street East, Charleston, WV 25305, while contracts valued at $25,000 or less are submitted directly to the public authority issuing the contract. The coversheet requires vendor contact information including name, telephone number, address, and fax number. This reporting requirement reflects West Virginia's regulatory framework establishing standards for contractor compliance with drug-free workplace policies on publicly funded projects.
