About this file
This is a Certified Drug-Free Workplace Report Coversheet issued by the State of West Virginia Purchasing Division for a boiler maintenance contract (Contract Number: ARFQ WSH2700000003) with the West Virginia Department of Health and Human Resources at William R. Sharpe, Jr. Hospital. The form serves as a compliance document required under West Virginia Code 21-1D-7b, mandating that contractors submit a certified drug-free workplace report no less than once per year or upon project completion. The coversheet requires vendors to attach a comprehensive report detailing education and training provided regarding drug-free workplace requirements, identification of the HHS-certified laboratory performing drug tests, the average number of employees involved in the work, and categorized drug test results including pre-employment and new hire tests, reasonable suspicion tests, post-accident tests, and random tests with corresponding positive and negative results.
The report submission procedures differentiate based on contract value, with contracts exceeding $25,000 requiring submission to the West Virginia Purchasing Division at 2019 Washington Street East, Charleston, WV 25305, while contracts of $25,000 or less are submitted directly to the public authority issuing the contract. The coversheet includes sections for vendor contact information and requires vendors to verify completion of each required report element through checkboxes, though the specific values for these checkboxes are not indicated in this document.
