1.5 GENERAL PROPOSAL INFORMATION

Offerors should limit submissions to the data essential for evaluation so that a minimum of time and funds are spent in preparing the information. However, in order to be effectively and equitably evaluated, offerors must include, for each phase as appropriate, information that is sufficiently detailed to describe the offeror's qualifications and experience, equipment features of the proposal, and management capabilities to successfully complete the project.

Requirements stated in this Request for Proposal (RFP) are minimums. Proposals that meet or exceed these requirements are encouraged, although over-elaboration and irrelevancies are strongly cautioned against.

All data submitted in response to this RFP must be clearly reflected in the offeror's interpretation of criteria furnished in the RFP or that it has obtained or developed data by its own efforts.

Material indicated in the plans and specifications but not included in the offeror's proposal will be assumed to be included and a part of the proposal. All alternate proposals shall be specifically marked as such and separated in the submittal.

The Government will not be liable for any costs incurred by the Offeror submitting an offer in response to this Solicitation.

The criteria specified in this RFP are binding contract criteria and in cases of any conflict, subsequent to award, between RFP criteria and contractor's submittals, the RFP criteria shall govern unless there is a written agreement between the Contracting Officer and the contractor on the waiving of a specific requirement.

1.6 PROPOSAL REQUIREMENTS

1.6.1. Section 1: TECHNICAL APPROACH AND MANAGEMENT (TAB1)

The offerors shall submit a work plan describing the methods to be used to ensure successful completion of the specified work.

Drilling Approach (Tab 1a). Provide a description of the methods and approach to be used to ensure successful completion of the work in accordance with the plans and specifications. As a minimum, the Drilling Approach must include: 1) anticipated drilling production rates for the equipment and approach proposed, 2) methods to minimize deviation of grout holes, 3) the plan to be taken to prepare the work platform and prevent and/or repair damage to the overburden soil-bentonite cutoff wall as a result of installation of the grout curtain, 4) the procedures to produce a clean hole suitable for grouting, 5) a list of equipment to be used at the site and a list of equipment available for use if necessitated by schedule considerations. For each piece of equipment provide the following information: photograph, performance data, condition of equipment, last time equipment was used on a production basis, and age of equipment. If rental equipment is to be used provide names and phone numbers of equipment suppliers.

Computer Monitoring (Tab 1b). Provide a description of the hardware and software to be used for the real-time computer monitoring with on-screen graphical display of pressure, flow rate, and permeability; and how this system will be integrated with the physical work for quality control, routine grout mix selections, quantity determinations, job control, and documentation of work performed. Include previous experiences with both software and hardware to demonstrate successful use of computer monitoring. If the software or hardware (including sensors) proposed for use includes versions or models not previously used in production grouting, then describe contingency plans to be implemented if this software or hardware performs unsatisfactorily.

Grouting Approach (Tab 1c). Provide a description of the methods and approach to be used to design and test balanced stabilized grout mixes with various additives and the procedures that will be implemented to ensure that the grouting achieves closure to below the required residual effective permeability. Provide a list of grout materials to be used and the suppliers from which these materials are to be secured. Describe previous experiences using these materials and suppliers. Describe how the various components will be formulated into a stable grout with the properties required to complete the work as specified.

Schedule and Cost Control (Tab 1d). Provide a projected schedule for the work and the supplemental resources that are available in the event the work falls behind schedule. Also include a discussion of the anticipated production rate required to meet the proposed schedule and the sequencing of activities anticipated to support the estimated production capability and the anticipated impacts of standby time for blasting. As a minimum, the schedule shall include Mobilization, Work Platform construction, Set up and Calibration, Tunnel Plug, Overburden drilling, Rock Drilling, Placing Grout, and Verification Holes for the Base, Option 1 and Option 2 separately. Assume for this projected schedule that Option 1 will be awarded 16 months and Option 2 awarded 20 months after NTP, respectively.

Management Plan (Tab 1e). Provide a management plan for the project that describes how your resources, including subcontractors and material suppliers, will be coordinated and used to ensure successful completion of the project. Provide an organizational chart of key personnel that will depict how the various components will interact and who will interface with Government representatives. On-site personnel and equipment must be clearly distinguished from resources that are available but not present at the site. Minimum key personnel that shall be listed are:

Project Manager, CQC System Manager, Superintendent, Automated Grout System Operator, and Professional Geologist.

Tunnel Plug Approach (Tab 1f). Provide a plan for the construction of the tunnel plug that describes the approach and procedures to be used. Indicate the methodology to completely fill the cavity within the plug area ensuring that voids will not develop during concrete placement.

1.6.2 Section 2: EXPERIENCE (TAB 2)

Demonstrate corporate and individual experience to adequately manage, perform, and evaluate both drilling to depths in excess of 300 feet and grouting with balanced stabilized grouts using real time on-screen computer display of grouting parameters and experience in achieving effective residual permeability of less than 1 lugeon. Demonstrate corporate and individual experience to adequately perform Quality Control for both drilling and grouting operations.

Prime Contractor Experience (Tab 2a). Provide descriptions of projects ongoing or completed within the last 5 years that are similar to this project in size, scope and/or dollar value. Include a description of the work self-performed for each project, the original and final contract amounts, the original and final completion dates, and an explanation for any differences. Describe previous experience, if any, in working with the proposed subcontractors.

Individual Personnel Experience (Tab 2b). Provide resumes for key personnel identified in the management plan organizational chart and describe their experience, which qualifies them for this work. Specifically identify particular experience of the individuals on the projects list in Tab 2a.

Subcontractor Experience (Tab 2c). Identify all major subcontractors and provide job histories indicative of successful performance within the past five years relevant to the work to be performed on this project. Provide resumes for key personnel to be furnished under subcontract as identified on the organization chart.

1.6.3 Section 3: PAST PERFORMANCE (Tab 3)

The offeror shall provide a listing of three relevant projects, with detailed project description of each, along with a Point of Contact Name and Telephone Number. References will be interviewed and requested to provide a rating based on the Offeror’s: a) Quality of Work; b) Timely Performance; c) Effectiveness of Management; d) Compliance with Safety Standards;

and e) Compliance with Labor Standards. In addition, the Government will query and use the most recent ratings in the Construction Contractor Appraisal Support System (CCASS) or its successor.

Past Performance Rating: Any unsatisfactory ratings or more than two marginal ratings will result in an Unsatisfactory Rating.

In lieu of providing a list of relevant projects, Offerors who have sufficient ratings in the Construction Contractor Appraisal Support System (CCASS) may provide copies of their most recent ratings for evaluation by the Source Selection Evaluation Team. However, if the Offeror does not have a sufficient number of ratings in the CCASS, it must ensure that an adequate number of projects/references are provided to allow for a minimum of two references to be interviewed.

If the Government does not obtain past performance information for the projects identified by the Offeror or cannot establish a past performance record for the Offeror through other sources, past performance will be rated neither favorably nor unfavorably. The performance risk will be consider unknown.

1.6.4. Section 4: PRICE, BID GUARANTEE, REPRESENTATIONS AND CERTIFICATIONS (Documents Shall Be Submitted in Originals and Enclosed in a Clearly Marked Envelope) Note: If any Amendments are issued against this Solicitation, the offeror shall include its acknowledgment of those Amendments in this Section.

SF 1442 (Page 1 of the Solicitation): Block Numbers 16., 17., and 18. on the first page of the Solicitation (SF 1442) shall be completed by an authorized person from the company or joint venture.

SECTION 00010 (BID SCHEDULE): The Bid Schedule is contained in Section B of this Solicitation and shall serve as the offeror’s Price Proposal. Price will be evaluated for fairness and reasonableness through the use of price analysis. Price will also be checked for unbalancing of line items. Offerors are cautioned to distribute its costs appropriately and fully complete it’s pricing on the Bid Schedule.

SECTION 00600 (REPRESENTATIONS AND CERTIFICATIONS): The offeror must complete and submit the Representations and Certifications contained in Section 00600 of this Solicitation along with all other documents required by this Solicitation. Offers received without this Section fully completed will be rejected, considered non-responsive and ineligible for award.

Any offeror failing to submit an authentic Bid Guarantee will be considered non-responsive to the solicitation and ineligible for initial evaluations and award. The Bid Guarantee shall be in the amount of 20% percent of the offeror’s proposed price or $3,000,000.00, whichever is less.

1.7 TECHNICAL QUALITY EVALUATION OF PROPOSALS

Offerors are advised that the technical evaluations of proposals are conducted in strict confidence in that technical evaluation personnel review each proposal without knowledge of the price offered. Offerors are required to segregate their cost and pricing information (Section

4) from the technical quality parts (Sections 1 through 3) of the proposal.

Proposals will be evaluated based on the factors described herein. The technical and non-technical aspects of each proposal will be evaluated.

1.1 EVALUATION PROCESS.

The evaluation process essentially consists of four parts: proposal compliance review, technical/quality evaluation, price evaluation and cost/technical trade-off analysis.

1.2 PROPOSAL COMPLIANCE REVIEW.

The Source Selection Chairperson will make an initial review of each of the proposals to look for apparent errors, omissions and areas that require clarification. Thereafter, the findings will be submitted to the Contracting Officer for review. This information will state the specific nature of the question and the page and paragraph of the proposal requiring correction or clarification.

The Contracting Officer will review the findings and, if considered appropriate, task the Chairperson to submit questions to the offerors for a quick response to the Contracting Officer.

The Chairperson and the Contracting Officer will not cite apparent deficiencies (parts of the proposal that fail to meet the Government’s requirements). The proposals will not be evaluated at that time. The purpose of this preliminary review session is to eliminate minor irregularities or apparent clerical mistakes. This preliminary evaluation will assure that each proposal contains a technical and price proposal, and that all required forms and certifications have been fully completed.

1.3 TECHNICAL ACCEPTABILITY EVALUATION.

The evaluation team will evaluate each proposal passing the Proposal Compliance Review.

Proposals will be evaluated against the evaluation criteria (factors) indicated below and in accordance the requirements of the “PROPOSAL REQUIREMENTS.”

FACTOR 1: TECHNICAL APPROACH AND MANAGEMENT (SECTION 1, TAB 1)

FACTOR 2: EXPERIENCE (SECTION 2, TAB 2)

Evaluators will apply the appropriate adjective to each criterion (factor) rated. As each criterion is evaluated an assessment of Risk will be made. Past performance risk will be rated using the Past Performance Risk Assessment ratings listed below the rating adjectives.

RATING ADJECTIVES:

EXCELLENT - The proposal has exceptional merit and reflects an excellent approach which will clearlyresult in the superior attainment of all requirements and objectives. This clearly achievable approach includes numerous advantageous characteristics of substance, and essentially no disadvantages, which can be expected to result in outstanding performance. The risk of unsuccessful performance is very low as the proposal provides solutions which are unquestionably feasible and practical. These solutions are further considered very low risk in that they are exceptionally clear and precise, fully supported, and demonstrate a clear understanding of the requirements.

Risk Level (Excellent): Very Low

GOOD - The proposal demonstrates a sound approach which is expected to meet all requirements and objectives. This sound approach includes advantageous characteristics of substance, and few relatively minor disadvantages, which collectively can be expected to result in satisfactory performance. The risk of unsuccessful performance is low as the proposal contains solutions which are considered feasible and practical. These solutions are further considered to reflect low risk in that they are clear and precise, supported, and demonstrate an understanding of the requirements.

Risk Level (Good): Low

ACCEPTABLE - The proposal demonstrates an approach which is capable of meeting all requirements and objectives. The approach includes both advantageous and disadvantageous characteristics of substance, where the advantages are not outweighed by the disadvantages.

Collectively, the advantages and disadvantages are likely to result in acceptable performance.

The risk of unsuccessful performance is moderate, as the proposal solutions are generally feasible and practical. These solutions are further considered to reflect moderate risk in that they are somewhat clear and precise, partially supported, and demonstrate a general understanding of the requirements.

Risk Level (Acceptable): Moderate

MARGINAL - The proposal demonstrates an approach which may not be capable of meeting all requirements and objectives. The approach has disadvantages of substance and advantages, which if they exist, are outweighed by the disadvantages. Collectively, the advantages and disadvantages are not likely to result in satisfactory performance. The risk of unsuccessful performance is high as the proposal contains solutions which may not be feasible and practical.

These solutions are further considered to reflect high risk in that they lack clarity and precision, are generally unsupported, and do not demonstrate a complete understanding of the requirements.

Risk Level (Marginal): High

SUSCEPTIBLE TO BEING MADE ACCEPTABLE -An approach which, as initially proposed, cannot be rated Marginal because of a minor error(s), omission(s) or deficiency(ies) which is capable of being corrected without a major rewrite or revision of the proposal. NOTE: A Susceptible rating cannot be a final rating. The final rating will either increase to a rating of Marginal or better or decrease to Unacceptable. Risk Level: High to Very High

UNSATISFACTORY - The proposal demonstrates an approach which, based on a very high risk, will very likely not be capable of meeting all requirements and objectives. This approach has numerous disadvantages of substance, and advantages which, if they exist, are far outweighed by disadvantages. Collectively, the advantages and disadvantages will not result in satisfactory performance. The risk of unsuccessful performance is very high as the proposal contains solutions which are not feasible and practical. The solutions are further considered to reflect very high risk in that they lack any clarity or precision, are unsupported, and do not demonstrate an understanding of the requirement.

Risk Level (Unsatisfactory): Very High

PAST PERFORMANCE RISK ASSESSMENT:

LOW RISK – Based on offeror’s past performance record, essentially no doubt exists that the offeror will successfully perform the required effort.

MODERATE RISK – Based on the offeror’s past performance record, some doubt exists that the offeror willsuccessfully perform the required effort.

HIGH RISK – Based on the offeror’s past performance record, extreme doubt exists that the offeror will successfully perform the required effort.

UNKNOWN RISK – No relevant performance record is identifiable upon which to base a meaningful performance risk prediction. A search was unable to identify any relevant past performance information for the offeror or key team members/subcontractors or their key personnel. This is neither a negative or positive assessment.

1.4 PRICE EVALUATION.

The proposals are evaluated for all technical (non-price) criteria and then all who successfully meet the evaluation criteria are evaluated by price. Price will be evaluated for fairness and reasonableness using price analysis. Price will also be checked for unbalancing of line items.

Pricing will not be scored, but will be a factor in establishing the competitive range prior to discussions (if held), and in making the best value determination for award.

1.5 PRICE/TECHNICAL TRADE-OFF ANALYSIS.

After all above evaluations are complete, the evaluation team will then consider all factors to determine the proposal offering the most advantage to the Government. The team will compare the relative advantages and disadvantages of the technical proposals and compare prices.

For the purpose of this evaluation, the relative order of importance is as follows:

- All evaluation factors, other than cost or price, when combined are approximately equal to the cost or price.

- The Technical Evaluation Criteria, as shown below, is listed in the order of importance, with the first being the most important and the last being the least important

FACTOR 1: TECHNICAL APPROACH AND MANAGEMENT (SECTION 1, TAB 1)

Subfactor 1a: Construction Approach Subfactor 1b. Schedule and Cost Control Subfactor 1c. Management Plan Subfactor 1d. Equipment

FACTOR 2: EXPERIENCE (SECTION 2, TAB 2)

Subfactor 2a. Prime Contractor Experience Subfactor 2b. Individual Personnel Experience Subfactor 2c. Subcontractor Experience

Proposals will be evaluated on their own merit, independently and as objectively as possible.

Due to the interrelationships among evaluation criteria; however, final determination of contract award will be based on the best composite offer, all factors considered.

2. DESCRIPTION OF PROPOSAL EVALUATION CRITERIA

The Technical/Quality evaluation criteria (Sections 1 through 2) will be rated on a qualitative basis. To receive consideration for award, ratings of no less than Acceptable must be achieved for individual Technical Subfactor and no less than Moderate Risk must be achieved for the Performance Risk Factor. These criteria are listed in their relative order of importance, with the first being the most important and the last being the least important. Price (Section 4) is considered separately as discussed herein.

2.1 SECTION 1: TECHNICAL APPROACH AND MANAGEMENT (Tab 1)

Drilling Approach (Tab 1a). Provide a reasonably detailed discussion of the methods and approach for drilling. The following sub-factors will be considered in evaluating the drilling approach and each sub-factor is considered to be of equal importance:

a. An approach that shows a reasonably efficient drilling production rate, consistent with the overall project schedule and equipment listed.

b. Methods presented that are shown to be reasonably effective in controlling hole deviation.

c. A plan that shows careful consideration in protecting the existing overburden cutoff wall and corrective measures that can be employed to repair damage to the cutoff wall.

d. Procedures that produce a hole that is clean and readily able to be effectively grouted.

e. Equipment that is shown to be capable of reasonably achieving the drilling capability.

Computer Monitoring (Tab 1b). Consideration will be given to whether the proposed system presents full integration of hardware and software for monitoring of the key parameters of grouting operations and has been proven in projects will be rated higher.

Grouting Approach (Tab 1c). Provide a description of the proposed grouting approach which demonstrates a clear understanding of developing balanced stabilized grout mixes for this project, and a detailed listing of the specific materials to be used .

Schedule and Cost Control (Tab 1d). Provide a sufficiently detailed construction schedule, containing the specific tasks described, that are consistent with the production capability for drilling and grouting activities, which meets or exceeds the contract performance period.

Explicitly identify cost control measures that are presented that minimize delays and costs due to blasting activities and minimize the overall risk of cost growth and identify any resources that are available to overcome unforeseen maintenance or production issues.

Management Plan (Tab 1e). Provide a clear and concise management plan and discussion on how resources are to be handled, along with a clear organizational chart indicating the respective roles of the team to be used for this project.

Tunnel Plug Approach (Tab 1f). Provide a plan that shows a good understanding of the requirements and the approach to be taken in construction of the tunnel plug that will minimize voids and provide appropriate protection of workers.

2.2 SECTION 2: EXPERIENCE (Tab 2)

Prime Contractor Experience (Tab 2a). Provide information regarding projects undertaken that demonstrating experience in large grouting projects to depths in excess of 300 feet using balanced stabilized grouts and ability to achieve very low (1 lugeon) residual permeability.

Individual Personnel Experience (Tab 2b). Identify any personnel that have experience in grouting projects that are of significant depth, used balanced stabilized grouts, and able to achieve very low residual permeabilities and with roles consistent with this experience and shown on the organizational chart.

Subcontractor Experience (Tab 2c). Identify all subcontractors that have experience in grouting projects that are of significant depth, used balanced stabilized grouts, and able to achieve very low residual permeabilities and with roles consistent with this experience and shown on the organizational chart.

2.3 SECTION 3: PAST PERFORMANCE (Tab 3)

The offeror shall provide a listing of three relevant projects, with detailed project description of each, along with a Point of Contact Name and Telephone Number. References will be interviewed and requested to provide a rating based on the Offeror’s: a) Quality of Work; b) Timely Performance; c) Effectiveness of Management; d) Compliance with Safety Standards;

and e) Compliance with Labor Standards.

3. BASIS FOR AWARD.

Proposals must meet the criteria stated in the RFP in order to be eligible for award, to include responsiveness, technical acceptability and responsibility.

In order to determine which proposal represents the best overall value, the Government may compare proposals to one another. The Government will award a contract to the responsible Offeror whose technical submittal and price proposal contains the combination of those criteria described in this document offering the best overall value to the Government. Best value will be determined by a comparative assessment of proposals against all source selection criteria in this RFP.

The Government reserves the right to accept other than the lowest priced offer. The right is also reserved to reject any and all offers. The basis of award will be a conforming offer, the price or cost of which may or may not be the lowest. If other than the lowest offer, it must be sufficiently more advantageous than the lowest offer to justify the payment of additional amounts. Any award price must be determined to be fair and reasonable.

Offerors are reminded to include their best technical and price terms in their initial offer and not to automatically assume that they will have an opportunity to participate in discussions or be asked to submit a revised offer. The Government may make an award of a conforming proposal without discussions, if deemed to be within the best interests of the Government.

The Government intends to award a contract without discussions based on initial proposals received; therefore, the proposals shall contain the offeror’s best terms from a cost and technical standpoint. However, the Government reserves the right to conduct discussions in accordance with FAR 52.215-1. Should discussions be necessary after evaluations, the Government will establish a competitive range of the offerors that are the most highly rated. The Government reserves the right to address any pertinent issues in the proposals.

4. SMALL BUSINESS SUBCONTRACTING PLAN.

Prior to the Contracting Officer awarding the contract, if the successful offeror is a large business they will be required to submit a Small Business Subcontracting Plan in accordance with the provision of FAR Clause 52.219-9, “Small Business Subcontracting Plan.” This requirement applies to all business concerns other than small businesses. The Contracting Officer must approve the Plan before an award can be made; the Plan will be made apart of the resultant contract.

Offerors shall not include a small business subcontracting plan with its proposal package.

The successful offeror shall make a good faith effort to apply the following percentages to its intended amount of subcontracts in order to satisfy the Small Business Subcontracting goals of the Chicago District. The District Goals are identified below:

(1) at least 70.0% with small businesses (SB)

(2) at least 6.2% with small disadvantaged businesses (SDB)

(3) at least 7.0% with women-owned SB (WOSB)

(4) at least 0.9% with service-disabled veteran-owned SB

(5) at least 3.0% veteran-owned SB

(6) at least 9.8% with HUBZone SB
