INSTRUCTIONS TO OFFERORS

1. TYPE OF AWARD
The Government contemplates award of a Firm-Fixed Price (FFP), performance-based contract in accordance with the Performance Work Statement (PWS) dated 19 November 2019 to provide environmental remediation services at Barksdale Air Force Base (AFB), Louisiana;
Dyess AFB, Texas (TX); Goodfellow AFB, TX; Joint Base San Antonio (JBSA) Camp Bullis, TX;
JBSA Fort Sam Houston, TX; JBSA Lackland, TX; JBSA Randolph, TX; Laughlin AFB, TX; and Sheppard AFB, TX.
2. BASIS FOR AWARD
The award will be made to the contractor representing the best value to the Government as determined by use of the trade-off method of source selection. The source selection decision will be made based on both the technical proposal and the price proposal. The Government reserves the right to make award to other than the firm submitting the lowest price. The price to be used for evaluation purposes is the total of the offerors’ price proposal, including the Base Contract and all options.
3. GOVERNMENT FURNISHED INFORMATION
A substantial amount of Government Furnished Information (GFI) is available for the ORC TX/LA Group. Contractors may submit a request to obtain the available GFI by submitting an emailed request to the Contact Specialist at Brenda.K.Anderson@usace.army.mil . The request must provide the following: Company Name, POC, address, city, State, zip code, email, and telephone number. ONLY ONE (1) SET OF GFI PER COMPANY. Upon receipt of the request, the GFI will be provided on CDs and will be shipped in accordance with the provided information. GFI may be requested at any time until ten (10) calendar days from the proposal due date. Contractors and potential Offerors are urged to request the GFI as early as possible to allow sufficient time to review the data.
IMPORTANT NOTE: Some GFI is sensitive information, such as sampling data not yet incorporated into a report, or otherwise made publicly available.

Information provided in the PWS and as GFI is intended to assist the Contractors in developing their proposals. Contractors are responsible for performing whatever due diligence they deem prudent to examine records, documents, etc. necessary to develop a proposal, to include independent verification of the information provided in the PWS and as GFI. A reasonable effort (at the time of the Request for Proposal) has been made to supply all relevant information for the use of the Contractor.
4. PRE-PROPOSAL SITE VISITS
Pre-award site visits will be conducted by the Government and are tentatively scheduled for the second week of December 2019 in San Antonio, TX covering Texas installations and the third week of December 2019 for Barksdale, AFB in Louisiana. The tentative site visit agenda is as follows:
Tuesday 10 December Goodfellow, Dyess, Sheppard – Virtual Site Visit and Lackland Site Visit Wednesday, 11 December Fort Sam Houston and Camp Bullis Site Visit Thursday, 12 December Laughlin – Virtual Site Visit and Randolph Site Visit Tuesday, 17 December Barksdale - Site Visit
All personnel attending the site visits will need to notify the Contract Specialist (Brenda Anderson) via email to Brenda.K.Anderson@usace.army.mil with the number and names of attendees for each site visit no later than COB on 3 December 2019. No more than three personnel from each firm may physically attend the site visits. Please note that no photography will be permitted during any site visit. Further information regarding the site visit logistics will be provided via separate email to those firms signifying their intention to attend a site visit.
5. ORAL PRESENTATIONS
The Government will not conduct oral presentations as part of this procurement, but reserves the right to conduct discussions with all Offerors determined to be part of the competitive range, should the SSA deem discussions necessary.
6. SUBMISSION OF TECHNICAL PROPOSAL AND COST/PRICE PROPOSAL
HARD COPY PROPOSALS WILL NOT BE PERMITTED.
THE ONLY AUTHORIZED TRANSMISSION METHOD FOR SUBMITTING PROPOSALS IN
RESPONSE TO THIS SOLICITATION IS THROUGH THE DOD SAFE WEBSITE AT:
https://safe.apps.mil. NO OTHER TRANSMISSION METHODS (EMAIL, FACSIMILE, REGULAR MAIL, HAND CARRIED, ETC.) WILL BE ACCEPTED.
Proposal submissions shall be uploaded to the DOD SAFE website by the identified date and time. Firms are strongly encouraged to review instructions for using the DOD SAFE service to ensure they have no problems accessing the DOD SAFE site or issues uploading files and transmitting their proposals. A drop-off request must be submitted to Brenda.K.Anderson@usace.army.mil prior to creating a drop-off for uploading of files. Once the drop off request is made, you will receive an email with a link to submit your drop-off. The drop-off link will be valid for 14 days. Firms are encouraged not to wait until the proposal submission date to submit their drop-off request.

6.1 Electronic File Submission Organization:
Offerors shall submit their proposals in response to this RFP via DOD SAFE in two (2) separate Volumes to the identified USACE receivers in paragraph 6.2 below.
a. The first will consist of all electronic files relating to Volume 1 – Technical Proposal.
Offerors shall ensure their company name is within the file name of each document transmitted. Both pdf and native file versions of documents shall be submitted (i.e., Word, MS Project, etc.).
b. The second will consist of all electronic files relating to Volume 2 – Cost/Price and Contract Information. Offerors shall ensure their company name is within the file name of each document transmitted. Both pdf and native file versions of documents shall be submitted (i.e., Word, Excel, etc.)
All of the proposal files may be compressed (zipped) into one file entitled proposal zip using the latest version of WinZip or 7-Zip, or as separate uploads. The USACE utilizes Microsoft Office 2013. Ensure files are compatible and viewable in Microsoft Office 2013 format. Self-extracting exe files are not acceptable.
*NOTE: In accordance with FAR Subpart 4.8, "Government Contract Files," the government will retain one copy of all unsuccessful proposals.

6.3 Proposal Submission Date/Time:
Proposals shall be received not later than 1500 hours (3:00 p.m.) Central Standard Time on 30 January 2020.
(NOTE: Offerors are cautioned to allow sufficient time for creation and submittal of their drop-off containing their proposal files, and for the Receivers identified in Section 6.2 to receive the appropriate email notification. Therefore, waiting until the last minute to attempt transmission of your proposal could result in your proposal being deemed late due to the website’s slow performance, internet connection issues, etc.)
6.4 Receipt of Proposal
For the purposes of establishing whether a proposal is considered late, the government considers (the earlier of) the date and time of receipt of the DOD SAFE website email notification of the last proposal submission to the Contract Specialist and Contracting Officer, NOT the date and time of uploading of the proposal into the DOD SAFE website as the time of proposal submission.
Do not assume that electronic communication is instantaneous. It can take several minutes or even hours in some cases. Per Federal Acquisition Regulation (FAR) 15.208 any proposal, modification, or revision, thereof that is received after the exact time specified for receipt of proposals is “late” and will not be considered.

The government will not be responsible for proposals delivered to any location or to anyone other than those designated to receive proposals on its behalf. Offerors are responsible for ensuring that proposals are submitted so as to reach the designated recipient of proposals.
Offerors are responsible for allowing sufficient time for the proposal to be received in accordance with the instructions provided.
6.5 Page Format Restrictions and Limitations
6.5.1 Page Limitations
Page limitations shall include typewritten text pages, charts, graphs, figures, diagrams, schematics, etc., aside from those items specifically stated as exempt from the page count.
Outlines and other similar information documents shall be included as numbered pages - no exceptions. The total number of pages for the Volume 1 submittal shall not exceed one hundred (100) pages. Compliance with page count limits shall be verified via the pdf version of the Volume 1 submission. If exceeded, the excess pages will not be read or considered in the evaluation of the proposal. The same page limitations shall also be placed on responses to Evaluation Notices (ENs), if issued.
6.5.2 Pages Counted
Each page shall be counted except the following: cover letter, cover pages, executive summary, table of contents, title page, list of tables/figures/acronyms, list of appendices, glossaries, separator tabs, dividers, blank pages, Past Performance Questionnaires, Resumes, Teaming Agreements, Integrated Master Schedule (IMS), Milestone Pricing Schedule (MPS) without cost (Table L-1), Milestone Payment Schedule (without cost), and Volume 2 in its entirety.
NOTE: Proposals that exceed one hundred (100) pages will have only those pages that are within the page count limits evaluated. All information appearing thereafter will not be evaluated.
6.5.3 Page Size
Unless otherwise stated, a “page” is defined as being one (1) 8.5” x 11” sheet of paper. Pages shall be typewritten except for the reproduced sections of the solicitation document and the font size shall be no less than 10 point and fully legible. A smaller font size (minimum of 8 point and fully legible) may be used on charts, graphs, figures, diagrams, and schematics to accommodate a “make to fit” software capability; however, all text shall be legible and easily read by a reasonable person. Margins on all four edges of each sheet will be at least one inch (margins for figures, maps and schedules may be reduced to no less than ½-inch). Proprietary statements, security markings, and page numbers can fall within the defined margin area.
Pages shall be numbered sequentially by volume. These page format restrictions shall apply to responses to EN responses (if discussions are required), as well as to the initial proposal.
6.5.4 Indexing
Each Volume shall contain a more detailed table of contents to delineate the subparagraphs within that Volume. Tab indexing shall be used to identify sections.
6.5.5 Larger Pages
Legible tables, charts, graphs and figures shall be used wherever practical to depict organizations, systems and layout, implementation schedules, plans, etc. These displays shall be uncomplicated, legible and shall not exceed 11” x 17” in size. Each page larger than 8½” x 11” will count as two pages. For tables, charts, graphs and figures, the font shall be no smaller than 8 point and fully legible; font style utilized is the Offeror’s choice. All information
(except documentation identification, solicitation number, and page numbers) shall be contained within an image area of the previously stated margins.
6.6 Volume/Page Labeling and Preparation
Volumes 1 and 2 must be submitted separately. Each Volume shall include, at a minimum, the following:
a. Volume number and Volume name on proposal cover (e.g. Volume 1 – Technical Proposal).
b. The prime, consortium, or joint venture’s name, address, and a telephone number shall appear in the lower left corner of the proposal cover, title page, and table of contents of any document/volume to be evaluated. Signature of the official with authority to bind the firm shall appear on the title page. Proposal volume cover format is at the Offeror’s discretion.
c. A Table of Contents, which provides a cross-reference of the Offeror’s proposal to the applicable sections of the solicitation.
d. List of tables/figures.
e. Volume number, section, and date submitted shall appear in the bottom right corner of each page (along with the revision number for the amended page, if necessary).
f. The Offeror shall ensure that a complete Volume 1 and Volume 2 is submitted, separately, as a .pdf document, each inclusive of all documents and attachments for the respective Volume.
Be sure to apply all appropriate markings, as applicable, including those prescribed in accordance with FAR 52.215-1(e), "Restriction on Disclosure and Use of Data."
7. PROPOSAL INSTRUCTIONS
The proposal shall be clear, concise and include sufficient detail for effective evaluation and for substantiation of the validity of stated claims. Below are the Assembly Instructions.

7.1 SPECIFIC SECTION INSTRUCTIONS
Cover Letter: The Cover Letter shall be submitted with Volume 1 and identify the point of contact (POC) [name, phone number and e-mail address] for any questions regarding the submitted proposal. The identified POC shall be the Offeror’s representative(s) who has authority to negotiate and bind the company. It is recommended that two POCs are identified.
DFARS 252.209-7999 Representation by Corporations Certificate: The Offerors shall submit the completed “Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction under any Federal Law” certificate in Volume 2.
7.2 Pages Counted
Each page shall be counted except the following: cover letter/pages, executive summary, table of contents, title page, list of tables/figures/acronyms, list of appendices, glossaries, separator tabs, dividers, blank pages, figures, exhibits, Past Performance Questionnaires, Resumes, Project’s Schedule (MPS without Cost and Pricing Schedule without Cost)/IMS/WBS and Volume 2 in its entirety.

Offerors are cautioned that “parroting” of the Technical requirements or the PWS with a statement of intent to perform does not reflect an understanding of the requirement or capability to perform. Offerors are responsible for including sufficient details to permit a complete and accurate evaluation of each proposal.
8. VOLUME 1 – TECHNICAL PROPOSAL (FACTORS 1 THRU 5)
8.1 Volume 1, Tab 1, Technical Approach (FACTOR 1):
The Offeror shall develop and submit a comprehensive cost effective technical approach to demonstrate achievement of the Air Force site-specific performance objectives in Table 1 of the PWS during the Period of Performance (PoP). Proposed approaches shall demonstrate an in-depth understanding of Air Force and regulatory requirements at each site that is fully supported, explained, and based on logical, feasible, attainable strategies and rationale. As part of their technical approach, the Offeror’s proposal should include a description of specific plans and steps anticipated to implement site specific technical approaches. The technical approach shall identify any risks along with mitigation strategies associated with the proposed technical approach(s). Risk mitigation strategies shall also identify decision logic, thresholds for determining implementing decisions, and timing if the proposed approach(s) proves incapable of achieving the Performance Objectives within the Performance Periods.
Additional specific proposal requirements for Factor 1 include:
Optimization and LCC Estimate: Optimization is defined as the reduction of contaminant mass and concentrations, reduction in LCC, regulatory action, or any combination of activities prompting acceleration to RC, as a result of remedial implementation the contractor undertakes, above and beyond what is required to maintain compliance in accordance with an approved decision document. In accordance with PWS paragraphs 1.2, 1.2.5, and 4.5.1, as part of their technical approach, offerors shall propose optimization strategies for all RAO and LTM sites.
Proposed optimization strategies must be based on logical, feasible, attainable, and cost-effective rationale that will result in reduced LCC and/or reduce the time required to achieve SC.
The Offeror should indicate the percentage degree to which their optimization approach will reduce cost and when. LCC reductions based on proposed optimization efforts accrued during the PoP must be reflected in the Offeror’s Volume 2 cost proposal (e.g., reduced LTM costs on a site by site basis in year 7 compared to year 2).
The Offeror shall include a total post-PoP life-cycle cost estimate with proposed activities, activity duration, and assumptions. At a minimum, sufficiently detailed assumptions must be provided for the anticipated activities beginning with estimated completion of the proposed performance objective or the contract PoP, whichever comes first, and carry them through Offeror’s projected Site Closeout date (or FY50 for sites with indefinite closeout dates) for the Government to validate the Offeror’s post-PoP life-cycle cost estimate. Post-PoP Life-Cycle cost estimate must be on a FY20 cost basis, un-escalated, and provided by site and by fiscal year.
Note: The Offeror shall provide the total LCC in the cost volume and not in the technical volume. The Government will compute the total LCC by adding the post-PoP LCC and the proposed price included under Factor 6.
Technical Summary Table: The Offeror shall provide a Technical Summary Table for all sites that reflects their proposed technical approach. The first page of this tab shall include a summary table which provides the following information for each site included in the PWS:
Installation Site ID, Site Name, Performance Objective, Duration of Achieving Performance Objective (from obligation of CLIN/Sub-CLIN), Brief Description of the Technical Approach, and
Site Performance Objective Completion Date. Site ID and Site Name should be copied from PWS Table 1. Performance Objective is the performance objective to be achieved within the PoP and shall meet the performance objectives listed in the PWS. Based on the offeror’s technical approach, a Projected Site Response Complete (RC) Date or Site Closeout (SC) Date shall be provided for all sites with RC/SC performance objectives to be achieved within the PoP.
Business Case Analysis: The end-state objective of a site at the end of the contract should be well defined and should be justified via development of a business case analysis as the optimum technical solution when considering technology and cost. The Offeror shall develop a business case analysis to support each proposed performance objective for sites listed in Table 1 of the PWS and to demonstrate any tangible benefits to the Air Force through reduction in life-cycle cost (LCC).
8.2 Volume 1, Tab 2, Work Breakdown Structure, Integrated Master Schedule, Milestone Payment Schedule without Cost, and Pricing Schedule without Cost (Factor 2)
The offeror's proposal shall include a Work Breakdown Structure (WBS), Integrated Master Schedule (IMS), and a Milestone Payment Schedule (MPS) (without cost).
8.2.1 WBS
The WBS shall demonstrate a comprehensive understanding of the ORC process and work required to achieve each Performance Objective (PO); this would be demonstrated by a thorough work decomposition of each task and effective application of a WBS structure. The WBS shall align with the Contract Line Item Number (CLIN)/Sub-CLIN Structure provided with the RFP, consistent with the Offeror’s proposed approach, and in accordance with the requirements in PWS Section 4.1.
8.2.2 IMS
The IMS shall be built from the WBS above. The IMS must be consistent with the Offeror’s proposed approach, and in accordance with the requirements in PWS Section 4.2. At a minimum, the offeror’s IMS shall:
• Present a realistic sequence and timing of all activities and documents necessary to demonstrate achievement of the PO within the PP.
• Include Sub-CLIN activity detail at a level that demonstrates a comprehensive understanding of the work and project execution
• Demonstrate logical incorporation of realistic Government and Regulatory review periods as defined in the PWS.
• Identify timing of decision points addressing risk mitigation and/or contingency strategies.
Offerors shall identify and discuss schedule assumptions, schedule risks and mitigations, and the means and methodologies they will employ to maintain and update their proposed IMS throughout the PoP in accordance with PWS Section 4.2. The narrative information does count toward the page limit; however, the IMS is excluded from the page count.
8.2.3 MPS (without cost)
The MPS (without cost information) shall be based on the WBS above, consistent with the Offeror’s proposed approach and the IMS, and in accordance with the requirements in PWS
Section 4.3 and the draft MPS (without cost) template provided in Section J. At a minimum, the Offeror’s MPS (without cost information) shall:
• Present appropriate milestone payments considered integral and necessary to the achievement of the POs.
• Include milestone payments per CLIN/Sub-CLIN based on the scope and anticipated duration to achieve CLIN/Sub-CLIN objective and/or PO.
• Does not include milestone payments representing a “progress” payment or a monthly payment for level of effort expended as well as management or overhead type costs.
• Include final CLIN/Sub-CLIN milestone payments demonstrating achievement of the CLIN/Sub-CLIN objective which is a minimum of 20% of the total CLIN/Sub-CLIN value.
The Offeror’s proposal shall also include the Pricing Schedule (without cost) which is identified as Attachment L-1 showing the ORC CLIN/sub-CLIN structure. The Pricing Schedule (without cost) cannot be modified and serves as the streamlined roll-up of the MPS without cost.
8.3 Volume 1, Tab 3, Management Approach and Key Personnel (FACTOR 3) The Offeror’s technical capabilities and management approach will be evaluated based on the qualifications of their proposed Key Personnel and management approach, which will include organization structure and overall management approach.
8.3.1 Management Approach
The Offeror shall submit a detailed management plan that clearly demonstrates their organizational approach for achieving the proposed site-specific performance objectives and milestones as scheduled, managing the requirements, ensuring quality services and deliverables, lines of authority for Key Personnel, and organizational relationships with any proposed subcontractors/team members. The Offeror shall include whether they anticipate the use of their own resources totally, a joint venture, a teaming arrangement, or other subcontracting arrangements. The Offeror shall include names and addresses of team subcontractors as well as a summary of the contractual agreements and/or corporate commitments binding the firms to the contract. If these agreements are in place, provide a certified copy of the contractual agreement(s) in Volume 2, Tab 4, which will not be included in the total page count. A certified copy is defined as a copy of a document or record, signed and certified as a true copy by the officer to whose custody the original is entrusted.
The Offeror’s Management Plan shall demonstrate the requirements in the following order:
• Efficient and effective management of team members and subcontractors including clear identification of roles, responsibilities, and communication strategies for key personnel, subcontractors, and other relevant team members o Provide an organizational chart depicting the above elements o Key personnel demonstrating applicable qualifications in PWS Section 10.0
• Effective and efficient management of the work in order to meet the requirements of the PWS including management of multiple and concurrent events across multiple installations.
• Quality Control procedures/processes to ensure services that result in submission of high-quality technical documents and deliverables. At a minimum, the contractor shall include procedures/processes that address the following:
• change management
• prime and subcontractor staffing management
• sampling and analysis
• health and safety
• An effective communication strategy between stakeholders (Air Force, USACE, Contractor, Regulators, and Community).
• Procedures/processes to identify, monitor, mitigate, and/or address management risks.
8.3.2 Key Personnel and Resumes
The evaluation of Key Personnel will consider education, professional qualifications, complexity and range of experience, and accomplishments/successes in executing and providing environmental services in accordance with the PWS.
The Offeror shall provide resumes only for individuals being proposed for the Key Personnel roles identified in Section 10.0 of the PWS. Resumes must clearly demonstrate how the proposed individual possesses not only the required education and qualifications, but also the required years of experience in the stated Program areas required by his/her respective Key Personnel role stated in Section 10.0 of the PWS to successfully execute the requirements of this contract. Resumes are excluded from the 100-page count limit for Volume 1 but are limited to two (2) pages each.

Offerors shall ensure that each resume (example format below) provides detailed information on specific education, professional registration (if applicable), required years of experience (not as a declarative statement but as demonstrated through listed projects on the resume), accomplishments, and achievements relevant to the proposed Key Personnel role for which the individual is being proposed. The resumes shall also include appropriate cross-references of the proposed individual’s work on submitted Past Experience projects, if applicable (i.e. if the individual worked in that role on any of the submitted Past Experience projects). 8.4.1. Past Experience
Past Experience will be evaluated based on the breath, depth, and relevance of the projects submitted by each Offeror; these submitted projects must demonstrate the Offeror’s Past Experience performing the Environmental Remediation Services identified in the PWS of the solicitation.
Each Offeror’s Past Experience will be rated based on the quality of each project’s narrative describing the work performed (i.e. how well the project’s scope is described) and how relevant the work is to the PWS, as well as the depth and breadth of each project’s scope.
The complexity of a submitted project and innovativeness of the Offeror’s technical approach to achieve the performance objectives or to overcome obstacles, either anticipated or unforeseen, will be considered if presented by the Offeror in the project’s narrative.
The description of each project shall indicate the Offeror’s role in the execution of that project (i.e. prime or sub-contractor), as well as the specific work elements performed by the Offeror (reference the project information form below). References, to include name, address, telephone number and email address of the customer, shall be provided for each submitted project.
Confidential clients will not be accepted as references.
8.4.2. Past Experience Project Criteria
Criteria for the Offeror’s Factor 4 – Past Experience:
a. Submit no more or less than five (5) projects to demonstrate the organization’s/team’s relevant Past Experience as it relates to contract type (i.e. FFP Performance Based), magnitude (i.e.
number of installations/sites), technical scope, and complexity (both technical and regulatory);
b. At least three (3) of the five (5) submitted projects must be performed by the Offeror as the Prime;
c. It is not a requirement for Offerors to demonstrate specific experience at any installation identified in the PWS. However, submitted past experience must demonstrate the Offeror’s ability to successfully perform their proposed effort for the TX/LA Group;
d. Ensure each submitted project meets the relevance requirements to the PWS of this RFP;
e. Ensure each submitted project meets the definition of a ‘project’, as defined below; AND
f. Submitted projects must be at least 75% physically completed (not fiscally completed or the amount invoiced), within the last 5 years prior to the proposal due date. 75% physically completed means at least 75% of the contractual requirements have been accomplished.
For the purpose of the past experience evaluation, offerors shall be defined as business arrangements and relationships, such as Joint Venture participants, teaming partners, and major subcontractors.
8.4.3. ‘Project’ Definition In order to be deemed a ‘project’, it must meet one, and only one, of the following:
a. 1 single Task Order against an IDC (a “D” contract) covering work performed at one single site;
b. 1 single Task Order against an IDC (a “D” contract) covering work at multiple sites within one single installation or facility;
c. 1 single “C” type contract (not a “D” contract) covering work performed at one single site (the following also will qualify as a non-‘D’-type: a Cooperative Agreement or Grant;
single BPA/BOA Call; single order against a GSA Schedule; or Purchase Order);
d. 1 single “C” type contract (not a “D” contract) covering work performed at multiple sites within one single installation or facility (the following also will qualify as a non-‘D’-type: a Cooperative Agreement or Grant; single BPA/BOA Call; single order against a GSA Schedule; or Purchase Order) ;

8.5 Volume 1, Tab 5, Past Performance (FACTOR 5):
The Offeror shall make every possible effort to submit Past Performance information, to include a Past Performance Questionnaire (PPQ), for each project submitted in Factor 4 – Past Experience. The Government will evaluate the Offeror’s Past Performance information to determine performance risk associated with the Offeror’s likelihood of unsuccessful performance in performing the requirements set forth in the solicitation. The recency (was it performed within the last 5 years prior to the proposal due date) and relevancy (how relevant to the PWS) of the submitted Past Performance information, the source of the information, context of the data, and general trends in the Offeror’s performance may be considered.
For the purpose of the past performance evaluation, an ‘Offeror’ shall be defined as business arrangements and relationships, such as Joint Venture participants, teaming partners, and major subcontractors.
The Offeror may provide a narrative for each submitted Past Experience project that addresses its’ past performance information such as if it was completed on-time/budget, whether or not the Offeror encountered any performance issues or challenges, and key and/or best management practices the Offeror employed to sustain successful performance for each past performance project submitted. If performance issues, customer dissatisfaction or challenges did arise, provide information on what corrective action was implemented or how they were addressed and outcomes. The narrative information does count toward the page limit; however, the PPQs are excluded from the page count.
For each example project submitted for Factor 4, the Offeror shall provide a PPQ that includes:
Project # - Title (matching past experience); Contract and task order number as applicable;
Client Point of Contact including email and phone number, and a description of the performance on that project. The Offeror shall send the PPQ, found as Attachment L-3 below, to those Government agencies or private sector companies’ Point of Contact identified for each project in Factor 4 – Past Experience for completion. These agency or company contacts must have detailed information concerning the performance of the project in order to accurately complete the PPQ for the Offeror. The government may contact PPQ POCs to further assess the scope of work performed performance; therefore, it is imperative to ensure correct telephone and email addresses are identified for the client POCs on each PPQ.
Completed Past Performance Questionnaires shall be submitted with your proposal. Offerors shall follow-up with clients/references to ensure timely submittal of questionnaires. If the Offeror’s client requests, he/she may submit the questionnaire directly to the Contract Specialist, Brenda Anderson, Brenda.K.Anderson@usace.army.mil prior to proposal closing date.
PPQs previously completed by the example project's POC may be submitted in lieu of requesting the POC to re-complete the form; however, Offerors shall not incorporate by reference into their proposal PPQs previously submitted for other RFPs. However, this does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation. CPARS evaluations shall not be submitted by the Offeror in place of a
PPQ.
In the event that one or more of the client POCs do not respond to the Offeror’s request to complete the PPQ, the Government may elect to contact them utilizing contact information provided with each project submitted for Factor 4. Therefore, the Offeror must ensure the information provided for each past experience project is accurate (i.e., POC name, telephone number, email address, etc.).
During the past performance evaluation, the Government reserves the right to obtain past performance information from any available source. The Government will consider information submitted by the Offeror, as well as any other relevant and reliable information obtained from any other source (including information from Government personnel and databases). The Government may review and consider any other sources of information for evaluating past performance. Other sources may include, but are not limited to, past performance information retrieved through the Past Performance Information Retrieval System (PPIRS), including Contractor Performance Assessment Reporting System (CPARS), using all CAGE/DUNS numbers of team members (partnership, joint venture, teaming arrangement, or parent company/subsidiary/affiliate) identified in the Offeror’s proposal, inquiries of owner representative(s), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), and any other known sources not provided by the Offeror. In evaluating past performance, the Government may consider available past performance information on predecessor companies, employment histories of key personnel, or major subcontractors performing key elements of the project.
While the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate and complete past performance information rests with the Offeror.
A significant achievement, unexplained or unresolved problem, or lack of relevant data may significantly impact the rating by the Government. Therefore, it is incumbent upon the Offeror to include all relevant information, including demonstrated corrective actions, in its proposal.
In addition to the project information requested in this section, the Offeror shall also provide notification and information concerning any projects where it’s right to proceed was terminated for the convenience of the government or for default/cause during the last five (5) years, or so state that there were none. If any contracts were terminated, whether for convenience or default/cause, the Offeror shall list each, providing the contract number, title, brief type of work effort, and the reason for the termination.
9. VOLUME 2 – COST/PRICE (FACTOR 6):
The Offeror shall submit, in Volume 2, the cost/price information described below. Volume 2 will be evaluated to determine the reasonableness of each Offeror’s cost proposal over the life of the contract, and the adequacy and value of the cost data in accordance with FAR 15.404-1(b) and FAR 15.404-1(c) to determine best value to the Government.
The Offeror shall carefully read the instructions for Volume 2 to ensure all requirements are complied with. Since the evaluation of the cost proposal represents a portion of the total evaluation as described in Section M, it is possible that an Offeror’s proposal might not be selected for award because of an Offeror’s oversight, unreasonable, unrealistic, incomplete or inaccurate information. Specifically-identified omissions will result in the Offeror’s Cost/Price Proposal to be deemed “Unreasonable” and, therefore, ineligible for further consideration for award. REMEMBER, the Government intends to award without discussions. The Cost/Price Proposal shall consist of the information identified below.
There is no page limitation for Volume 2.
9.1 Volume 2, Tab 1, Cost/Price (FACTOR 6):
MILESTONE PAYMENT SCHEDULE (with Cost) and PRICING SCHEDULE (with Cost)
Volume 2, Tab 1, shall consist of the Offeror’s proposed pricing by identifying a Firm-Fixed-Price (FFP) for each Contract Line Item (CLIN)/Sub-CLIN identified on the Pricing Schedule with Cost and in accordance with PWS paragraph 4.3 and the draft MPS (with cost) template provided in Section J. Each proposed price for each CLIN/Sub-CLIN and the total proposed price shall be inclusive of all labor, materials, sub-contracts, other direct costs, other indirect costs and profit to complete the performance based PWS. The Base Award items and all Options shall be included in the cost/price calculation.
IMPORTANT NOTES:
1. Do NOT link the Unit Prices of the MPS with Cost to any Attachment L-2 or Supplemental Pricing Information spreadsheets’ totals.
2. Ensure the Unit Prices are entered as whole-dollar only.
3. Offerors MUST submit the Excel version of the Milestone Payment Schedule with
Cost.
The Offeror’s proposal shall also include the Pricing Schedule (with cost) which is identified as Attachment L-2 and shows the ORC CLIN/sub-CLIN structure. The Pricing Schedule (with cost) serves as the streamlined roll-up of the MPS with cost, and cannot be modified. Reference Notes on the Attachment L-2 must be complied with. Offerors MUST also submit the Excel version of the Pricing Schedule with Cost.
9.2. Volume 2, Tab 2, Cost/Price (FACTOR 6):
ATTACHMENT L-4, SUPPLEMENTAL PRICING INFORMATION
The Offeror shall utilize Attachment L-4 to identify the required Supplemental Pricing Information. Attachment L-4 is provided in Excel format, the document SHALL NOT be modified in any manner whatsoever (i.e. formulas, adding/deleting lines, adding/deleting cells, etc.); Offerors are to enter the required information/data within the identified cells ONLY.
THE EXCEL VERSION OF THE ATTACHMENT L-4 MUST BE SUBMITTED WITH THE
OFFEROR’S PROPOSAL.
In order to evaluate competing approaches, supplemental pricing information (e.g., “data other than certified cost or pricing data”) is required in accordance with FAR 15.403-3(b) and for each sub-CLIN and CLIN (when sub-CLINs are not present). Each identified price must be inclusive of overhead and profit.
For each CLIN or Sub-CILN:
a) Provide total proposed labor cost
b) Provide total proposed “Other Direct Costs” (inclusive of travel).
c) Provide total proposed sub-contractor(s) costs (inclusive of any sub-contractor handling fees).
d) Include all profit
Note: The Basis of Estimate (BOE) narrative required in Tab 3 shall provide an explanation of how it was determined that the profit and prices proposed by the prospective subcontractor(s) or estimated by the Offeror are fair and reasonable. While subcontractor quotes/cost buildup/supplemental information are not required, the Offeror must clearly identify the work to be performed by the subcontractor.
d) Do not submit any list of exceptions to the PWS. Offers including exceptions will not be included in the evaluation.
9.3. Volume 2, Tab 3, Cost/Price (FACTOR 6):
BASIS OF ESTIMATE
It is important for the Offeror to identify the performance objectives and list and discuss in detail all assumptions on a site by site basis to further the Government’s understanding during review of the Offeror’s cost/price proposal. This shall be accomplished by submitting a BOE narrative written for each Sub-CLIN and CLIN (when Sub-CLINs are not present). The BOE must identify/list and explain the Offeror’s pricing assumptions in sufficient detail to substantiate their proposed prices identified on the Milestone Payment Schedule with Cost, and Attachment L-4, in order to allow the Government evaluators to conduct an adequate analysis in accordance with Section M.
9.4 Volume 2, Tab 4, DFARS 252.209-7999, Representation by Corporation Certificate:

(E) Exceptional Performance meets contractual requirements and exceeds many to the Government/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor was highly effective.
An Exceptional rating is appropriate when the Contractor successfully performed multiple significant events that were of benefit to the Government/Owner. A singular benefit, however, could be of such magnitude that it alone constitutes an Exceptional rating. Also, there should have been NO significant weaknesses identified.
(VG) Very Good Performance meets contractual requirements and exceeds some to the Government’s/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
A Very Good rating is appropriate when the Contractor successfully performed a significant event that was a benefit to the Government/Owner. There should have been no significant weaknesses identified.
(S) Satisfactory Performance meets minimum contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
A Satisfactory rating is appropriate when there were only minor problems, or major problems that the contractor recovered from without impact to the contract. There should have been NO significant weaknesses identified. Per DOD policy, a fundamental principle of assigning ratings is that contractors will not be assessed a rating lower than Satisfactory solely for not performing beyond the requirements of the contract.
(M) Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective or were not fully implemented.
A Marginal is appropriate when a significant event occurred that the contractor had trouble overcoming which impacted the Government/Owner.
(U) Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
An Unsatisfactory rating is appropriate when multiple significant events occurred that the contractor had trouble overcoming and which impacted the Government/Owner. A singular problem, however, could be of such serious magnitude that it alone constitutes an unsatisfactory rating.
