Include only first tier U.S. subcontractors. If the prime Offeror (to include any U.S. small business concerns who are proposing as part of a joint venture or teaming arrangement) is itself a U.S. small business concern under NAICS ______, the Offeror’s own participation as a SB, SDB, WOSB, HUBZone, VOSB, and SDVOSB will also be considered small business participation to the extent it is qualified under NAICS ______ for the purpose of this evaluation.

(2) Small Business Participation Past Performance.
All offerors shall submit information substantiating the offerors’ past performance in complying with FAR 52.219-8, “Utilization of Small Business,” maximizing opportunities for U.S. small business subcontractors. In addition, large business concerns shall submit information substantiating the offerors’ past performance in complying with FAR 52.219-9, “Small Business Subcontracting Plan.” Offerors shall also provide a statement indicating whether any negative information has been reported in the past three (3) years concerning the offerors’ past compliance with FAR 52.219-8 and/or FAR 52.219-9 (when applicable). If any such negative information has been reported, the offeror may submit explanations or comments responding to such negative information. Offerors with no prior contracts containing FAR 52.219-8 and/or FAR 52.219-9 shall certify the same. The Government may, in evaluating the responses to this subfactor, conduct follow-up interviews with offerors and/or subcontractors, submit questionnaires to both offerors and subcontractors, as well as review offeror information contained in the Contractor Performance Assessment Reporting System (CPARS), the Past Performance Information Retrieval System (PPIRS), or the Electronic Subcontracting Reporting System (eSRS).
(3) Small Business Subcontracting Plan.
(i) Separate from the Small Business Participation Plan, other than U.S. Small Business Offerors must also submit a Small Business Subcontracting Plan meeting the requirements of FAR 52.219-9 and DFARS 252.219-7003 (DFARS 252.219-7004 for Comprehensive Subcontracting Plans). Large Businesses must submit an acceptable Subcontracting Plan to be eligible for award. Individual Subcontracting Plans shall reflect and be consistent with the commitments offered in the Small Business Participation Plan.
(ii) Per FAR 19.702(a)(1) and FAR 19.705-5(a)(5), the Small Business Subcontracting Plan is not a requirement for evaluation in source selection, but rather, a requirement for award to a large business and will be incorporated into any resultant contract. The Small Business Subcontracting Plan will not count against the page limitation for this volume. In addition to providing the Small Business Subcontracting Plan with the Offeror’s proposal submission, a copy of the plan shall be provided to the Offeror’s Administrative Contracting Officer (ACO) as soon as possible after the issuance of this solicitation but no later than the date set for receipt of proposals. The proposal shall specify name, address, phone number, and e-mail address of the ACO to which the plan was submitted. Offerors that have an approved Comprehensive or Commercial Subcontracting Plan must still submit a separate Small Business Participation Plan.
Note: Subcontracting goals are calculated as a percentage of subcontracted dollars as opposed to small business participation goals which are calculated as a percentage of the total contract dollars.
