About this file
This is a Qualifying Project (QP) Form template used for evaluating contractor experience and performance on prior projects under the MAPS IDIQ contract. The form is divided into two parts: Part 1 captures project identification and evaluation criteria, while Part 2 contains the legacy version of the form for reference.
Part 1 requires offerors to submit one of three qualifying projects (numbered 001, 002, or 003) with supporting contract documentation and relevant statements of work, performance work statements, or task guidance letters. Offerors must designate the domain(s) against which the QP will be evaluated: Engineering, Logistic & Operational Domain; Management & Advisory Domain; RDT&E Domain; Emerging IT Domain; or Foundational IT Domain. For evaluation purposes, the form captures: the contracting party point of contact information, specific contract/agreement numbers with copies, period of performance (LOE-based QPs require at least one year of completed performance within three years prior to May 1, 2026; outcome-based QPs must be fully completed within that same timeframe), alignment with technical capabilities in Section 3.0 of the PWS, applicable NAICS codes, and CPARS or PPQ ratings. For LOE-based QPs, offerors must provide vacancy rate and time-to-fill rate calculations for the last full year of performance. For outcome-based QPs, offerors must demonstrate schedule adherence (100% on-time delivery), completeness against project requirements (100% requirements fulfilled), and document customer-recognized efficiencies such as cost savings, operational improvements, or quality enhancements. All QPs must include the subcontractor passthrough rate calculated as total incurred subcontractor labor dollars divided by total incurred labor dollars. The form requires certification by both the contracting party point of contact and the offeror, and the government reserves the right to contact references to verify submitted information.

| 9 | If no, identify which permissible NAICS Code would it best align to per RFP Section L.2.3.2: |
| 9.a | Please provide a brief explination on how this NAICS Code relates to your QP: |
| 10 | Does this QP have a CPARS or PPQ per RFP Section L.2.3.2 |
| 11 | Dollar Value: |
| Items 12-17 can be completed by the Offeror or Contracting Party Point of Contact Listed Above. |
Item 18 shall be digitally or hard signed by the Contracting Party POC.
If this QP is identified as a LOE QP, Items 13 and 14 shall be completed, with Items 15 and 16 being N/A.
If this QP is identified as an Outcome Based QP, Items 15 and 16 shall be completed, with Items 13 and 14 being N/A.
| 12 | Is this QP a LOE-Based QP or an Outcome-Based QP? |
| 13 | Vacancy Rate |
(per RFP Section M.5.7.1)
The Government will evaluate the QPs to determine the vacancy rate for the last full year of performance on the QP.
The vacancy rate shall be determined by the following: the number of vacant positions on the QP, divided by the total number of proposed positions, multiplied by 100.
(# of vacant position at the end of the last full year of performance of the QP / # of positions proposed)*100
If the QP had a reduction of labor due to an official descope of the requirements by the requirements owner, and as a result the number of positions was reduced, the Offeror may update the total number of employees proposed to reflect this reduction in labor.
14 Time to Fill Rate (per RFP Section M.5.7.2)
The Government will evaluate the QPs to determine the average time to fill for any job vacancy on the QP for the last full year of performance on the QP.
The Time to Fill shall be calculated by the Date of Offer Acceptance − Date Vacancy Opened for each vacancy on the QP over the last full year of performance. If more than one vacancy was filled on the QP over the last full year of performance, the average time to fill shall be calculated by using the mean average of all filled positions.
Date Of Offer Acceptance – Date Vacancy Opened = Time to Fill
(Time to Fill Vacancy 1 + Time to Fill Vacancy 2 + Time to Fill Vacancy 3 (etc.) / Total Number of Vacancies Filled for the Last Year of Performance = Mean Average 15 Schedule (per RFP Section M.5.7.3)
The Government will evaluate each QP to determine the timeliness/delivery of the service outcome as identified in the contract/agreement.
In order for a QP to earn points for this criteria, the QP shall meet the Acceptable Quality Level (AQL) which is 100% of the Overall Project Schedule has been met.
Calculation: (Actual Delivery/Contractual Delivery) *100%
* Unit of measure (i.e. days, months, years) is dependent on terms of the QP.
16 Completeness (per RFP Section M.5.7.4)
The Government will evaluate each QP to determine its completeness against the project requirements as identified in the contract/agreement, as well as customer recognized efficiencies. A customer recognized efficiency must have been experienced during the performance of the QP and show measurable improvements tied to results, not just level of effort. These efficiencies may include, but are not limited to cost efficiencies, administrative efficiencies, performance and operations efficiencies, improved reliability/stability, reduced oversight burden, better quality outputs, and contractor-initiated improvements. Efficiencies related to schedule shall not be attributable to this section but are in paragraph M.5.7.3 above.
In order for a QP to earn points for this criteria, the QP shall meet the AQL which is 100% of the requirements fulfilled.
17 Passthrough Rate (per RFP Section M.5.6)
The passthrough rate will be calculated by the Total Incurred Subcontractor Labor Dollars divided by the Total Incurred Labor Dollars for each QP. Labor Dollars shall be fully burdened.
(Total Incurred Subcontractor Labor Dollars/Total Incurred Labor Dollars)*100 = Subcontracted Amount 18 Certification of Contracting Official (Government or Private Official):
