About this file
This is a Past Performance Questionnaire (PPQ) Form template used to evaluate contractor performance on federal contracts. The form is designed to be completed by government or private contracting officials to assess a contractor's past performance within the preceding four years, with ratings and evaluations that will inform federal contract award decisions.
The form uses five adjectival rating scales—Exceptional, Very Good, Satisfactory, Marginal, and Unsatisfactory—to evaluate contractors across five key performance categories: Quality (planning, technical program execution, engineering support, and interface management), Schedule (timeliness of task completion and milestone delivery), Cost Control (forecasting, managing, and controlling contract costs), Management (coordination, problem identification, corrective actions, and subcontract management), and Small Business Subcontracting (compliance with FAR Subpart 19.7 requirements and statutory obligations to provide opportunities for small businesses, including disadvantaged, women-owned, HUBZone, and veteran-owned entities). Part I requires the offeror to provide contract identification details including contract number, contractor name, CAGE code, program title, NAICS code, prime contractor name, and contract dates. Part II identifies the offeror's representative with contact information. Part III is completed by the contracting official respondent, who must provide detailed comments for any ratings other than "Satisfactory" and certify the accuracy of the information provided. The government reserves the right to contact the contracting official to verify any information submitted on the form.

PPQ
Past Performance Questionnaire (PPQ) Form Attachment 0004 Amendment 01 The following questions pertain to the Contractor's record of past (within the past four years) and current performance for the contract identified below. The information that you provide as the Contracting (Government/Private) Official will be used in the awarding of a federal contract. Therefore, it is important that our information be as factual and accurate as possible. Please provide examples and/or explanations (use additional pages if necessary) sign the form, and return to individual identified in Part II: "Identification of Offerors Representative".
The following adjectival ratings shall be used in your reponse.
Exceptional: Performance meets contractual requirements and exceeds many to the Government’s/Customer's benefit. The contractual performance of the element or sub-element being evaluated was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
Very Good: Performance meets contractual requirements and exceeds some to the Government’s/Customer's benefit. The contractual performance of the element or sub-element being evaluated was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
Satisfactory: Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
Marginal: Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being evaluated reflects a serious problem for which the contractor has not yet identified corrective actions. The contractors proposed actions appear only marginally effective or were not fully implemented.
Unsatisfactory: Performance does not meet most contractual requirements, and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
