ATTACHMENT 2

Quality Assurance Surveillance Plan

Tribal Training and Technical Assistance Center

Required Tasks
Performance Standards
Method of Surveillance

Quality Assurance Standard to be met/allowed deviation

Overall Task Order Management, including cost and time

Contractor adheres to guidance provided by COR.
COR and Contracting Officer monitoring; deliverables; budget estimates; invoices and reports

Fewer than 6 customer complaints during each contract year, including COR comments.

Customer Satisfaction
Contractor maintains high level of quality assurance, responsiveness to COR & Contracting Officer; reliability, completeness of tasks; contacts COR immediately with any issues or problems. Contractor remains within or below cost estimates; meets time frames identified by COR; notifies COR immediately of any budget issues; no cost overruns.

Contractor responds within 24 hours to COR inquiries 95% of the time.

COR & Contracting Officer monitoring deliverables, budget estimates, invoices and reports.

COR receives no more than 6 valid complaints per 12 months. No more than 2 invoices per 12 months requiring suspension or disallowance due to mistakes, incompleteness, or unallowable costs. If deficiencies are found, Contractor will provide COR solutions for implementation within 60 days.

Task 1 Plan of Performance
Timely, comprehensive kickoff meeting, Plan of Performance, and revisions that are consistent with the contract purpose, goals, and time frames and that incorporate feedback from the Contracting Officer’s Representative (COR)/Alternate COR and other Federal staff initially and on an ongoing basis.
COR/Alternate COR oversee deliverables
Contractor submits a Plan of Performance that is 85% complete by no later than one (1) week after the CED. Contractor responds to all inquiries from the COR within one business day. Additional plans are provided that are 85% complete and are consistent with the requirements in the SOW.

Task 2

Deliver Training and Technical Assistance

Timely and comprehensive training and technical assistance (TTA) to broad, focused, and intensive audiences. TTA shall include on-site and virtual TTA such as phone calls, conference call, emails, and webinars. In addition there shall be Gatherings of Native Americans (GONA), Learning Communities, Tribal Action Plan Trainings, emails, conference calls, phone consultations, webinars, on-site TA, obtaining consultants, readiness assessments and community engagement, infrastructure development assistance and support, sustainability and mentorship activities.
COR/Alternate COR oversee deliverables
TTA is developed and delivered consistent with the SOW. COR receives no more than 4 valid complaints from TTA recipients; products are of high quality and need few corrections.
Task 3 - Information Dissemination
Timely undertaking and completion of the production and dissemination of work products and resources, comprehensive IT plan including IT Security Plan, Website Migration Plan, and Content Development and Management Plan. Provide maintenance, and management, listserv development and maintenance, document preparation, and graphic development tasks
COR/Alternate COR oversee deliverables
Task activities and deliverables are accurately met 85% of time. Contractor submits products that are complete, accurate, timely (as specified in the SOW), and clearly written with minimal errors found.
Task 4 Data Collection and Analysis
Timely undertaking and completion of data collection and analysis utilizing SAMHSA-approved GPRA measures including but not limited to immediate outcomes, intermediate outcomes, and long-term (6 months and beyond) outcomes.
COR/Alternate COR oversee deliverables
Data is collected consistently using SAMHSA’s OMB-approved GPRA measures. Reports are comprehensive and submitted on time. Data is submitted to a SAMHSA-supported data collection platform semi-annually.
Task 5 Reporting
Timely completion of reports that are accurate (85 percent error-free), complete, and incorporate COR/Alternate COR comments and revisions.

Task activities and deliverables are accurately met 85% of time. Contractor submits reports that are complete, accurate, timely (as specified in the SOW), and clearly written with minimal errors found
Task 6 Transition Plan and Turnover at the End of Contract
Timely submission of transition plan; delivery of all publications, materials, software, equipment, data, correspondence, supplies and all other items procured under this contract to the government or new contractor.
COR/Alternate COR oversee deliverables
Contractor provides for the transition of all previous contract activities (e.g., website, training, and technical assistance) so there is no lapse in service. 100% of requested information, data and materials, is provided to COR. Transition activities are completed as specified in the SOW.
Optional Task 7 Planning, Training, and Meeting Logistics and Support for SAMHSA’s Tribal Technical Advisory Committee (STTAC)
Timely and comprehensive planning, coordination, and execution of meetings for SAMHSA’s Tribal Technical Advisory Committee members including logistics, event evaluation, and preparing meeting summaries
COR/Alternate COR oversee deliverables
Meetings are planned and executed in a way consistent with the SOW. Logistics are executed with minimal errors and COR receives no more than 2 valid complaints from attendees. Meeting summaries and event evaluations are prepared with 90% accuracy.
Optional Task 8 Circles of Care (COC) Grantee Technical Assistance
Timely and comprehensive provision and planning of individualized TA, cross-site/group TA, COC grantee meeting, and coordination with COC evaluation TA contractor
COR/Alternate COR oversee deliverables
Plans and TTA are developed and delivered consistent with the SOW. COR receives no more than 4 valid complaints about TA from grantees; contractor meets delivery schedule; products are high quality and need few if any corrections. Contractor responds to all inquiries from the COR within two business days. Task activities and deliverables related to the grantee meeting are accurately met 85% of time.
Optional Task 9 Support for Annual Circles of Care Grantee Meeting
Timely and comprehensive support the annual grantee symposium, including working with the evaluation contractor, planning with the COR/ACOR, and task leads, preparing all meeting materials, providing logistic support, and post-meeting debriefing and reports.
COR/Alternate COR oversee deliverables

Task activities and deliverables are accurately met 85% of time.

Optional Task 10 Meeting of Tribal Leadership for Intensive TTA Sites
Timely and comprehensive support for the meeting, including planning, execution and evaluation, and providing a summary of the event.

Optional Task 11 Plan and Conduct a Gathering of Native Americans/Gathering of Alaska Natives (GONA/GOAN):

Timely and comprehensive planning and delivery of GONA/GOAN to include logistical arrangements, facilitation, and evaluation, and reporting of the event.
COR/Alternate COR oversee deliverables
GONA/GOAN is developed and delivered consistent with the SOW. COR receives no more than 4 valid complaints from TTA recipients; reports are of high quality and need few corrections.

Optional Task 12

Deliver Training and Technical Assistance

Timely and comprehensive training and technical assistance to a broad audience. TTA shall include on-site and virtual TTA such as phone calls, conference call, emails, and webinars.
COR/Alternate COR oversee deliverables
TTA is developed and delivered consistent with the SOW. COR receives no more than 4 valid complaints from TTA recipients; products are of high quality and need few corrections.
