About this file

This is a Business Proposal Template attachment for RFP 26-86378 issued by the State of Indiana (Department of Administration) for Community Supports IT Systems. The RFP requests comprehensive proposals from respondents for IT system products and/or services, with respondents required to submit detailed business proposals addressing organizational structure, financial stability, company integrity, contract terms, and client references. The proposal template requires three professional references from clients who have received similar products or services, with reference information to be submitted directly to the state at idoareferences@idoa.in.gov no more than ten business days after the proposal submission deadline specified in RFP Section 1.24.

Respondents must demonstrate financial responsibility through recent audited financial statements or Dunn & Bradstreet Business Reports for the two most recently completed fiscal years, with CEO and/or CFO certification of financial information accuracy. The template requires documentation of Indiana Secretary of State registration status, proof of signatory authority through corporate bylaws or board resolution, and detailed disclosure of experience serving state governments and similar-sized clients. Additionally, respondents must address their engagement efforts with Indiana Veteran Owned Small Business (IVOSB) subcontractors through the Division of Supplier Diversity, provide general company information including employee count and years of experience, and confirm the existence of disaster recovery plans and data security protocols. Proposed systems must align with one or more of the state's cloud-based service models (IaaS, PaaS, or SaaS) outlined in Attachments B1, B2, and B3, with respondents required to accept or provide detailed redlined edits with supporting rationale for any deviations from the state's Additional Terms and Conditions.
