About this file

This is a Request for Proposals issued by the City and County of San Francisco Department of Public Health for an Ending the HIV Epidemic Intensive Case Management program. The solicitation seeks qualified public or nonprofit entities (or private for-profit entities if they are the sole available provider) to operate intensive case management services for people with HIV who have complex behavioral health issues, unstable housing, and high acuity needs that prevent success in traditional Ryan White-funded case management. The program will provide multidisciplinary, agency- and community-based case management services to clients re-entering the HIV system of care through low-barrier, transitional medical and navigation services. The RFP was issued on August 10, 2026, with a pre-proposal conference scheduled for August 17, 2026 at 11:00 a.m. PT via Microsoft Teams. The deadline for submitting questions is August 24, 2026 at 5:00 p.m. PT, with proposals due September 9, 2026 at 2:00 p.m. PT through the San Francisco City Partner website. Short-listing notifications and interviews are estimated for October 2026, with contract award announcement also anticipated in October 2026 and a Health Commission presentation in November 2026. The contract will commence March 1, 2027, with an anticipated five-year initial term and five one-year renewal options at the City's sole discretion.

The estimated contract value exceeds $400,000 but does not exceed $10 million. The program is primarily funded through the federal Ending the HIV Epidemic in the U.S. initiative and must comply with Ryan White HIV/AIDS Program requirements and Uniform Administrative Requirements. A Local Business Enterprise rating bonus of up to 10 percent is available for Small or Micro LBEs and 5 percent for SBA LBEs; the LBE subcontracting requirement has been waived. Proposers must meet three mandatory minimum qualifications: at least five years of cumulative experience within the past ten years providing medical case management to people with HIV with moderate or intensive needs; at least three years of experience working with a consortium of HIV service providers; and the ability to employ or recruit appropriately licensed or license-eligible staff. Contractors must comply with San Francisco prevailing wage, Minimum Compensation Ordinance, Health Care Accountability Ordinance, First Source Hiring, and Chapter 12L nonprofit governance requirements. Protected health information and Business Associate Agreement compliance is mandatory. The primary contact is Lucinda Huang at 628-271-6174 or lucinda.huang@sfdph.org.

1.1 Request for Proposals:

The Department of Public Health (hereinafter “City” or “DPH”) is issuing this Request for Proposals (hereinafter “RFP” or “Solicitation”) to seek proposals (hereinafter "Proposals”) from qualified and experienced firms (hereinafter “Proposers”) to operate an intensive case management program to support people with HIV whose complex behavioral health issues and unstable housing challenge them from achieving and maintaining durable viral suppression, engagement in care and support services, community stability and overall wellness. The program will provide intensive, multidisciplinary, agency- and community-based case management services to clients who have re-entered into the HIV system of care through a low-barrier, transitional “Ending the HIV Epidemic” medical and navigation service and are too high acuity to be successful in traditional Ryan White-funded case management and navigation services.

The program is primarily funded by the federal Ending the HIV Epidemic in the U.S. initiative. In accordance with Ryan White HIV/AIDS Program requirements, services are to be provided by public or nonprofit entities or by private for-profit entities if they are the only available provider of quality HIV care in the area.

1.2 Contract Term:

The anticipated term of the contract will be a five-year term, with five one-year options to extend at the City’s sole, absolute discretion. The City reserves the right, in its sole discretion, to modify the anticipated contract term, including but not limited to the length of the initial term and any option periods, if the City determines that such modification is in its best interest.

2.1 Obtaining RFP Documents and Addenda:

A. RFP documents, including addenda, can be found on the San Francisco City Partner website: San Francisco City Partner Event Search B. Proposers will then need to search the Sourcing Event ID listed above for all related documents. It is the Proposer’s responsibility to check the San Francisco City Partner website regularly to stay current on the documents that are available as this is the primary communication website for this RFP.

C. The City may modify this Solicitation, prior to the Proposal due date, by issuing an addendum to the Solicitation, which will be posted on the San Francisco City Partner website. Every addendum will create a new version of the sourcing event and Proposers must monitor the event for new versions. The Proposer shall be responsible for ensuring that its Proposal reflects any and all addenda issued by the City prior to the Proposal due date regardless of when the Proposal is submitted. Therefore, the City recommends that the Proposer consult the website frequently, including shortly before the Proposal due date, to determine if the Proposer has downloaded all Solicitation addenda. It is the responsibility of the Proposer to check for any addenda, and updates, which may be posted to the subject Solicitation.

D. The submittal of a response to this Solicitation shall explicitly stipulate acceptance by Proposers of the terms found in this Solicitation, any and all addenda issued to this Solicitation, and the proposed contract terms.

2.2 Submission of Proposals:

A. How to Register as a City Supplier

The following requirements pertain only to Proposers not currently registered with the City as a Supplier.

Step 1: Register as a Bidder on the San Francisco City Partner website: San Francisco City Partner Bidder Registration Step 2: Follow the step-by-step instructions on SF.gov for converting your Bidder ID to a Supplier ID. This will require you to register with the City Tax Collector’s Office and submit the online 12B Declaration for Article 131 (Equal Benefits Program) compliance through the City Partner website. Once these forms have been completed, submitted, and processed, you will be notified via email with your organization's new Supplier ID. That email will also provide instructions for completing your Supplier registration.

1. City Business Tax Registration Inquiries: For questions regarding business tax registration procedures and requirements, contact the Tax Collector’s Office at (415) 554- 4400 or, if calling from within the City and County of San Francisco, 311.

4. Proposals must be received by the due date and time shown in the RFP. Proposals that are submitted by email will not be accepted.

5. Proposals must be submitted through the San Francisco City Partner website. However, if the website is not functioning, Proposers must notify the Procurement Officer listed in Section 1.4 (Procurement Officer and Delivery Address) and provide proof of the issue.

(CRA). CRA may be performed for each entity manufacturing the product, performing technical functions related to the product’s performance, and/or accessing City’s networks and systems. Where a prime Proposer or reseller plays an active role in each of these activities, CRA may also be required for the prime Proposer or reseller.

B. To conduct a CRA, City may collect as part of this Solicitation process one of the following two reports:

1. SOC-2 Type 2 Report: Report on Controls at a Service Organization Relevant to

3. The above reports may be requested at such time City has selected or is considering a potential Proposer. The reports will be evaluated by the soliciting Department and the City’s Department of Technology to identify existing or potential cyber risks to City.

Should such risks be identified, City may afford a potential Proposer an opportunity to cure such risk within a period of time deemed reasonable to City. Such remediation and continuing compliance shall be subject to City’s on-going review and audit through industry-standard methodologies, including but not limited to: on-site visits, review of the entities’ cybersecurity program, penetration testing, and/or code reviews.

3.8 Solicitation Errors and Omissions:

Proposers are responsible for reviewing all portions of this Solicitation. Proposers are to promptly notify the City, in writing and to the Procurement Officer if the Proposer discovers any ambiguity, discrepancy, omission, or other error in the Solicitation. Any such notification should be directed to the City promptly after discovery, but in no event later than the deadline for questions.

Modifications and clarifications will be made by addenda.

3.9 Objections to Solicitation Terms:

Should a Proposer object on any ground to any provision or legal requirement set forth in this Solicitation, the Proposer must, no later than the deadline for questions, provide written notice to the City setting forth with specificity the grounds for the objection. The failure of a Proposer to object in the manner set forth in this paragraph shall constitute a complete and irrevocable waiver of any such objection.

3.10 Protest Procedures:

A. Protest of Solicitation Content

Any Proposer who contends that the terms, conditions, or specifications of this Solicitation are unduly restrictive, improperly limit competition, or appear to be written in a manner that favors a particular vendor, may submit a written protest of the Solicitation content. Such protest must be received by the City no later than the deadline for submission of questions as set forth in the Solicitation schedule. The written protest must clearly identify each allegedly restrictive specification, explain in detail why the specification is deemed restrictive or biased, and include all supporting facts, evidence, and any applicable law, rule, ordinance, or procedure. The protest must be signed by an individual authorized to represent the Proposer. Failure to submit a timely protest will be deemed a waiver of the Proposer’s right to challenge the content of the Solicitation.

Protest of Non-Responsible Determination Within three business days of the City's issuance of a Notice of Non-Responsibility, a Proposer may submit a written Notice of Protest of Non-Responsibility. The Notice of Protest must include a written statement specifying in detail each and every one of the grounds asserted for the protest. The Notice of Protest must be signed by an individual authorized to represent the Proposer, and must cite the law, rule, local ordinance, procedure or Solicitation provision on which the protest is based. In addition, the Notice of Protest must specify facts and evidence sufficient for the City to determine the validity of the protest.

In addition, the Notice of Protest must specify facts and evidence sufficient for the City to determine the validity of the protest. Please note that a disagreement with how an individual evaluator scored a Proposal does not constitute valid grounds for a protest.

E. Delivery of Protests A Notice of Protest must be delivered to the Procurement Officer whose name and contact information appears in this Solicitation, and to My Lan Do Nguyen, Manager of Contractual Pre-Award, at mylando.nguyen@sfdph.org. A Notice of Protest must be written; protests made orally (e.g., by telephone) will not be considered. It must be transmitted by a means that will objectively establish the date the City received the Notice of Protest, and received by the due dates stated above. If a Notice of Protest is mailed, the protestor bears the risk of non-delivery within the deadlines specified herein.

3.11 Proposal Term:

Submission of a Proposal signifies that the proposed products, services and prices are valid for 180 calendar days from the Proposal due date and that the quoted prices are genuine and not the result of collusion or any other anti-competitive activity. At Proposer’s election, the Proposal may remain valid beyond the 180-day period. The City reserves the right to award additional contracts under this Solicitation if additional funding becomes available or programmatic needs change. Such awards may be made to Proposers that submitted Proposals in response to this Solicitation, whether or not they were selected for an initial award, provided the Proposers met the Minimum Qualifications and their Proposals were determined to be responsive. Any additional awards must occur during the initial contract term identified in Section 1.2 (Contract Term) of this Solicitation (excluding any options to extend) and will be limited to the scope of services, products, and pricing proposed by the Proposers as of the deadline identified in Section

SECTION 4 – SCOPE OF WORK

4.1 Background:

The San Francisco Department of Public Health, HIV Health Services (hereinafter “HHS”) is soliciting proposals to provide Ending the HIV Epidemic Intensive Case Management (hereinafter “EHE ICM”) services to HIV-positive clients with moderate to intensive need as defined in the scope of work.

San Francisco’s comprehensive HIV System of Care consists of integrated medical and support services designed to support people with HIV, particularly low-income and medically underserved populations. EHE ICM will address the critical need in San Francisco’s traditional Ryan White system of care, specifically regarding people whose care engagement patterns and HIV viral suppression is challenged by homelessness, serious mental illness, and dysregulated substance use. The duration of program enrollment is not time-limited, and the program should be designed in a way that seamlessly supports clients.

The program is primarily funded by the federal Ending the HIV Epidemic in the U.S. initiative. In accordance with the Ryan White HIV/AIDS Program requirements, services are to be provided by public or nonprofit entities or by private for-profit entities if they are the only available provider of quality HIV care in the area.

4.2 Scope of Work:

A. Target Population / Priority Population

The sole eligibility requirement for enrollment in the EHE ICM program is a documented HIV diagnosis. Eligibility for EHE ICM services may not be restricted to clients of one HIV primary care clinic but must be provided to clients of any primary care clinic in the City and County of San Francisco. Services must serve clients across the entire HIV system of care and wrap-around support services and must strive to avoid unnecessary service duplication.

The Contractor shall assess prospective clients for program suitability based on the presence of one or more moderate to intensive needs as listed below. See Attachment J (HIV Referral Acuity Scale).

1. Uncontrolled HIV: CD4<200; history of or living with multiple opportunistic infections and co-occurring disorders (e.g., HCV, cancer, diabetes); and/or current HIV viremia;

viral rebound since starting ART after initial diagnosis; and/or not on ART or taking oral ART inconsistently.

2. Serious mental illness and chronic substance use disorder with limited to no insight as to the severity or impact of these conditions on self, friends/family, and community and not engaged with a behavioral health provider (e.g., LCSW, LMFT, psychiatrist, psych

NP).

3. Extended history of unsheltered homelessness and housing instability without…
