About this file
This is a Respondent Information Form attachment for the Managed Network Services (MNS) Request for Offer (RFO) DIR-CPO-STS-609 issued by the Texas Department of Information Resources (DIR). The form serves as the primary documentation vehicle for collecting detailed information from all organizations bidding on this state of Texas procurement opportunity. Respondents must complete all sections of the form and obtain signature authorization from an officer or agent with contractual binding authority. The form does not specify project timelines, response deadlines, site visits, bidder conferences, award dates, or contract terms within its content, as it functions as a standardized information collection document rather than a project specification or timeline document.
The form requires respondents to disclose entity information including legal name, DUNS number, Federal Employer Identification Number, principal place of business, and multiple points of contact for communication throughout the procurement process. Respondents must also complete comprehensive conflict-of-interest disclosures identifying any current or former state employees who will dedicate 20 percent or more of their time to the resulting contract, as well as any proposed personnel related within two degrees of consanguinity to current or former state employees. Additional requirements include disclosure of any former state executives employed by the respondent and certifications regarding financial stability, tax payment status, compliance with federal procurement restrictions, debarment status, and adherence to antitrust laws. The form incorporates multiple statutory compliance certifications under Texas Government Code and federal regulations, including terrorism financing restrictions, foreign adversary ownership restrictions, and service contract requirements for Texas-produced products.
