About this file
This is a Safety, Risk Assessment, Accident Prevention, and Quality Control proposal form for West Texas A&M University's roof replacement project, administered through SSC Services for Education Facilities Services. The project encompasses roof replacement services for three campus facilities—the Jack B. Kelly Student Center, Classroom Center, and Cornette Library—located in Canyon, Texas. The scope of work includes polyisocyanurate and modified bitumen roof replacement, shingle roof replacement, polycarbonate panel replacement, and roof coating applications. A pre-proposal meeting was held on February 12, 2026, at 10:00 AM, with proposals due by February 26, 2026, at 2:00 PM. The project was awarded to CS Advantage with a base bid of $3,580,000.00 and a proposed construction schedule of 150 days. All contractors must comply with NRCA, SMACNA, and ANSI/SPRI standards and manufacturer guidelines, with gas lines tested and certified by master or licensed plumbers. Warranty provisions include a twenty-year non-delegable limited manufacturer's warranty and two-year contractor's warranty for most roofing systems, with five-year warranties required for acrylic coating applications.
This proposal form requires contractors to submit detailed safety and quality control documentation as mandatory compliance elements upon contract award. Contractors must provide Workers Compensation Experience Modification Rates for the previous three years with supporting documentation from their insurance agent, OSHA recordable incident rates with supporting logs or documentation, and evidence of regular project safety meetings and inspections. A written safety program must be provided along with a complete site-specific risk assessment program and quality control program addressing surveillance, acceptance, rejection, documentation, deficiency resolution, trend analysis, and corrective action procedures. A Historically Underutilized Business Subcontracting Plan is required, with monthly HUB Progress Assessment Reports mandated with each pay application. Technical inquiries should be directed to A/E Contact Greg Carroll at Armko Industries, Inc., and HUB-related questions to Bryon McCafferty at WTAMU, while submission process questions should be directed to SSC Project Manager Stephanie Young.
