EXHIBIT B – NON-COST EVALUATION QUESTIONS

Competitive Solicitation No.: 01519

Bidder: On-Site Environmental, Inc.

The Evaluation portion will have a maximum value of 35 points towards the total scoring. Each question is worth 5 points. Do not put your company name in the answers.

Question Answer

Provide most recent 3 Proficiency Testing results for the analyses/methods/matrix of interest

PE sample results are provided for 2017, 2018, and 2019. The files are in pdf format, and all start with “PE”.

Provide sample data packages for samples of similar types to those to be analyzed. The data packages should include initial calibration data.

Provided. Three separate lab reports are submitted. The file names are: Lab Report – general chemistry, Lab Report – metals, and Lab Report – organics.

Provide Method Detection Limit values for the analytes/methods of interest

A excel file named MDLs,PQLs is provided for all of the analyses of interest. It is broken up into three sections: General Chemistry, Metals, and Organics. MDLs are not done, and so not provided for the following analyses: Alkalinity SM 2320B, Total Solids SM 2540G, Total Suspended Solids SM 2540D, and Turbidity EPA 180.1.

Provide Initial Demonstration of Capability data for the analytes/methods of interest.

Analytical lab methods require the lab to perform an Initial Demonstration of Capability (IDC) for the analytes of interest. The IDC normally includes 4 replicates of a solution with known concentration that is near the middle of the calibration range. The lab should provide the summary data for the IDC, which includes the mean recoveries and % Relative Standard Deviations (RSDs). The lab does not need to submit any raw instrument data.

Provided. Three separate excel files are provided, with the tabs indicating the specific analyses and matrix. The file names are: ID of C

– general chemistry, ID of C – metals, and ID of C

– organics.
