This is a price quotation and bid response submitted by Bureau Veritas North America, Inc. to the Washington State Department of Enterprise Services (DES) in response to Competitive Solicitation No. 01519 for Analytical Laboratory Services. The proposal is dated September 9, 2019, and includes supporting documentation such as the Bidder's Certification (Exhibit A-1), Bidder's Profile (Exhibit A-2), Non-Cost Evaluation Questions (Exhibit B), and pricing worksheets (Exhibit C). The contract is structured as an indefinite delivery vehicle with a 24-month initial contract period and optional renewal periods extending to a maximum of 72 months. The contract will be available to eligible purchasers in the State of Oregon through the ORCPP partnership at no additional cost variance.
Bureau Veritas proposes to provide analytical laboratory services for air sample analysis, including industrial hygiene testing and air toxics analysis. The company operates two laboratory locations: a facility in Novi, Michigan that performs air toxics analysis using EPA TO-15 methodology for volatiles and ion chromatography methods for inorganic acids, and a facility in Kennesaw, Georgia that specializes in asbestos and microscopy testing including phase-contract microscopy, polarized light microscopy, and transmission electron microscopy. Pricing for air toxics testing ranges from $58.00 to $225.00 per sample depending on the specific method, with standard turnaround times of 5 business days and expedited options available. Asbestos and fiber testing prices range from $13.60 to $150.00 per sample. The company maintains AIHA-LAP accreditation since 1974, TNI/NELAP, and NVLAP certifications, and complies with ISO 17025:2017 standards. Bureau Veritas does not utilize subcontractors and has over 40 years of experience in industrial hygiene analysis with notable clients including the National Institute for Occupational Safety and Health (NIOSH), federal government agencies, and state government entities. Sampling media is provided at no additional charge unless overnight delivery is required, and the client bears responsibility for return shipping costs to the laboratory.

Mr. Richard Worthy Contract Specialist
WASHINGTON STATE DEPARTMENT OF ENTERPRISE SERVICES
Olympia, WA Emailed to: DESCPRMEnvironProc@des.wa.gov
Subject: RFP 01519 – Analytical Laboratory Services
Dear Mr. Worthy:
Bureau Veritas North America, Inc. (Bureau Veritas) is pleased to present this price quotation to Washington State Department of Enterprise Services (DES) to provide analysis for air samples.
The initial contract is for 24 months and with option years, the maximum option is 72 months.
Note that the Bureau Veritas laboratories for industrial hygiene (IH) analytical services were doing business as (d/b/a) Maxxam Analytics, as a brand name. However, in June we resumed using our legal name, Bureau Veritas North America, and the corresponding logo. Therefore, a few of our attachments still contain the Maxxam name.
Bureau Veritas brings the following advantages to industrial hygiene projects.
• Experience supporting industrial hygiene programs for a variety of industrial hygiene clients from state government, federal government, and the private sector
• A laboratory with 45 years of accreditation from the American Industrial Hygiene Association Laboratory Accreditation Program (AIHA-LAP,LLC) since the inception of the accreditation program
• Accreditation specifically for the requested industrial hygiene methods under the AIHA-LAP Industrial Hygiene Program (IHLAP)
• Compliance with the quality system requirements of the international standard, ISO 17025:2017
• Broad scope of services, performing a large majority of the industrial hygiene methods published by the Occupational Safety and Health Administration (OSHA) and the National Institute for Occupational Safety and Health (NIOSH)
• More than 13 years of experience as the contract laboratory for the Chemical Exposure Monitoring Branch (CEMB) for NIOSH, enabling us to remain on the cutting edge of new analytical methods and issues in chemical exposure sampling
We have worked with several clients for more than a decade and we value our long-term partnerships.

Proposal Content and Attachments
This letter summarizes our overall capabilities and a few pricing policies, since edits could not be made to the pricing pages. These should be considered clarifications on our bid response. The attachments to this proposal response are the following.
• Form A-1: Bidder Certification
• Form A-2: Bidder’s Profile o Certificate of Insurance (COI) is also attached o Please note that our three References should be kept confidential. Our laboratory accreditations require that we respect client confidentiality and their contact information is provided only for the bid evaluators.
• Form for Exhibit B: Evaluation Questions o Attachment 1: Performance Testing (PT) results o Attachment 2: Data package examples (Level IV) --note that our standard deliverable is a Level 2 (analytical results and QC summary) o Attachment 3: Method Detection Limits (MDLs), Reporting Limits (RLs), and Reporting Units o Attachment 4: Initial Demonstration of Capabilities (IDCs) – quality control (QC) charts and IDC forms from our quality system o Attachment 5: Accreditations – summary list and selected certificates
• Form for Exhibit C: Price Table (Excel) o Air Toxics price worksheet o Industrial Hygiene price worksheet for fiber tests (no bid for the microbiological fungi/mold o All other worksheets are labeled No Bid
The attachments demonstrate our many years of experience and consistent quality.
Laboratory Locations and Accreditations
All tests we are bidding are performed out of one of two laboratories in our network. No subcontractors are being proposed.
The laboratory for the tests requested on the Air Toxics price worksheet is located in Novi (Detroit), Michigan. This facility analyzes volatiles by EPA TO-15 and inorganic acids by a variety of ion chromatography methods. The tests requested on the Industrial Hygiene price worksheet are analyzed at the laboratory located in Kennesaw (Atlanta), Georgia. They specialize in asbestos and microscopy, including phase-contract microscopy (PCM), polarized light microscopy (PLM), and transmission electron microscopy (TEM).
Note that our laboratories offer a much wider range of industrial hygiene tests, beyond those requested on the worksheets. We analyze air samples and some wipes for contaminants, including metals, hexavalent chromium, crystalline silica, particulate, organic solvents, aldehydes, and isocyanates.
Each client can decide if they want to ship all samples to one laboratory and have us forward to the appropriate facility via overnight courier (at no additional charge) or if the client wants to deal with the individual laboratories, according to the testing required. We have contracts that work both ways, depending on the client’s preference. Please note that the turnaround time (TAT) calculation starts the day that the samples reach the laboratory performing the analysis.
The two laboratories in this proposal are accredited by AIHA-LAP, LLC in the pertinent programs and fields of testing. All the accreditation certificates are provided in this proposal, in Form Exhibit B, Attachment 5. The certificates show the specific fields of testing (FoT) and methods.
Our laboratories are compliant with the requirements of ISO 17025:2017, through our accreditation from AIHA-LAP. The laboratory quality system is thoroughly documented in the Bureau Veritas Quality Manual. Our contract from NIOSH also keeps us on the forefront of emerging issues and sampling methods.
Pricing Policies
Following are some key notes about pricing, since the Exhibit C Price Sheet could not be edited.
• On the Air Toxics price worksheet, the price for volatiles is for a standard analyte list for method EPA TO-15. If Tentatively Identified Compounds (TICs) are also needed, there is an additional fee.
• For Method EPA TO-15, any canisters not returned within 30 days will incur replacement fees, on the order of $600 per canister, unless arrangements for longer retention have been made at the beginning of the project.
• The sampling media for the various acid methods are included in the cost of the analysis.
• Expedited services option: Rush turnaround time (TAT) must be scheduled in advance of sending samples to the laboratory. We quoted a rush turnaround time of 3 business days in Exhibit C. Turnaround times of 2 business days, 1 business day, and same-day results are available on a sliding scale of surcharges.
• Sampling media is shipped at no additional charge, unless overnight delivery is required.
In those cases, we invoice at FedEx shipping rates or the client can provide us with a FedEx number to use.
• The client is responsible for the fees to return media to the laboratory for analysis.
Industrial hygiene samples are small and portable, so shipping is reasonably simple.
These pricing policies are defined due to our unique scope. Many of the tests that we offer require sample collection media, which is not required for environmental (water/soil/sediment) tests.
We appreciate an opportunity to support industrial hygiene and other air sampling projects from the states of Washington and Oregon. If you have any questions about this proposal, please contact me at 248-344-2649 or at laura.parker@bvlabs.com.
Sincerely, Laura Parker IH Consulting Scientist, Manager Bureau Veritas Laboratories Division

01519 Analytical Laboratory Services Exhibit B Evaluation Questions
EXHIBIT B – NON-COST EVALUATION QUESTIONS
Competitive Solicitation No.: 01519
Bidder: Bureau Veritas North America, Inc.
The Evaluation portion will have a maximum value of 35 points towards the total scoring. Each question is worth 5 points. Do not put your company name in the answers.
Question Answer
Provide most recent 3 Proficiency Testing results for the analyses/methods/matrix of interest
See Attachment 1. For each set of PTs, the most recent is shown first. Note that PTs are not available for all of the tests that we are bidding. A list of the PTs provided is contained on the first page of Attachment 1.
Provide sample data packages for samples of similar types to those to be analyzed. The data packages should include initial calibration data.
See Attachment 2, where we have provided representative data packages for different types of tests. The first page of Attachment 2 provides a list of the packages submitted.
Provide Method Detection Limit values for the analytes/methods of interest
See Attachment 3, which is an Excel sheet with 2 tabs. Note that some of the tests we are bidding do not require MDLs, as explained in the spreadsheet footnotes.
Provide Initial Demonstration of Capability data for the analytes/methods of interest.
Analytical lab methods require the lab to perform an Initial Demonstration of Capability (IDC) for the analytes of interest. The IDC normally includes 4 replicates of a solution with known concentration that is near the middle of the calibration range. The lab should provide the summary data for the IDC, which includes the mean recoveries and % Relative Standard Deviations (RSDs). The lab does not need to submit any raw instrument data.
See Attachment 4. Note that we have provided the quoted tests for many years and multiple analysts can do these tests. Therefore, instead of choosing a random analyst to submit an IDC, we have assembled quality control (QC) control chart data. This represents the QC for all analysts performing a selected test, across multiple batches. Following that data, we have included the forms we use to document IDCs for the tests being bid. Obviously, the criteria for chemistry tests (Air Toxics worksheet) are different from the criteria for the microscopy tests (Industrial Hygiene worksheet). The attached documents are explained further on the first page of Attachment 4.
01519 Analytical Laboratory Services Exhibit B Evaluation Questions
Question Answer
How many years of experience analyzing samples of similar types to those to be analyzed
We have more than 40 years providing full-service industrial hygiene analyses and fibers/asbestos to clients in state governments, federal government, and the private sector.
We have also been the contract laboratory for the National Institute for Occupational Safety and Health (NIOSH) for more than 10 years.
They are named as one of our references on form A-2 (Bidder Profile). Our laboratory has been accredited by AIHA-LAP since the inception of the program in 1974. Our staff consists of chemists and scientists with advanced degrees, two Certified Industrial Hygienists (CIH), and we have a professional geologist (P.G.) responsible for our fiber/minerals laboratory. Note that the IH Laboratories’ Quality Specialist has been overseeing our quality program and accreditations for the last 12 years. See the cover letter that accompanies this proposal for additional information.
Provide your Limit of Quantitation including reporting units for each Method bid.
See Attachment 3, which also contains the MDL data.
Provide copies of current Accreditation certificates and fields of testing.
See Attachment 4. Note that the tests on the Air Toxics price worksheet will be performed in the Michigan laboratory. The tests that we bid on the Industrial Hygiene price worksheet will be performed in the Georgia laboratory.
Therefore, we are submitting the pertinent accreditations for both laboratories from AIHA- LAP, NVLAP, and TNI/NELAP. Our accreditations demonstrate that we are compliant with ISO 17025 in the available fields of testing.
