INSTRUCTIONS TO OFFERORS

CONSTRUCTION MANAGEMENT SUPPLEMENTAL SERVICES ID/IQ
LEAVENWORTH, KANSAS
ISSUE DATE: May 11. 2023 OFFER DUE DATE: June 1, 2023, by 10:00 A.M. EST OFFERS SHALL BE SENT ELECTRONICALLY TO: mrmorgan@bop.gov
These instructions are provided as a guide for use in preparing a proposal in response to this solicitation. Failure to furnish all required information or any rejection of or addition to the terms and conditions of the solicitation may cause your offer to be considered as non-responsive and ineligible for award. Do not include any extraneous information/documentation outside of the solicitation requirements.
Offerors are reminded to ensure their SAM registration is complete and current.
REQUIRED SUBMISSIONS
1. One (1) copy of your proposal
2. CM-1 and CM-2 Questionnaires
3. Brief narrative describing how the requirement s will be fulfilled as shown in the
Statement of Work and how they will benefit the project.
4. CMSS Staffing Schedule
SECTION 1:
Solicitation Form Page 1, Standard Form 1449 Complete blocks 17a, 30(a), 30(b, 30(c). Ensure the address in block 17a exactly matches the address contained in your SAM registration to include the firm’s legal business.
SECTION 2:
Supplies or Services and Prices/Costs Item B.1 Utilize Exhibit C – Cost Proposal Section J.

1. FAR Provision 52.204-24, Representation Regarding Certain Telecommunication and Video Surveillance Services or Equipment (Nov 2021)
2. FAR Provision 52.209-7, Information Regarding Responsibility Matters (Oct 2018)
3. FAR Provision 52.209-12, Certification Regarding Tax Matter (Oct 2020)
4. FAR Provision 52.212-3, Offeror Representations and Certifications-Commercial Products and Commercial Services (Dec 2022)
Attached to this document is a performance questionnaire. While it is not mandatory, we ask that contractors provide the documents prior to the proposal opening. The Past Performance Questionnaire should be forwarded to at 5 companies/Government agencies who you have current work or have completed work within the last 5 years.
The references submitted should be in the same range as your proposal price. Please submit reference forms for projects of a similar nature or as similar as possible. If you do not have references similar to what you are proposing for this project, please indicate so by including a separate letter with your proposal stating so. Not having references similar will not determine your proposal non-responsive.
You are responsible to ensure the questionnaires are delivered to your references, and that they respond via forwarding the completed reference to mrmorgan@bop.gov. The completed reference documents must come from your reference, and not from an email address in your firm.
