ATTACHMENT L-4: PAST PERFORMANCE QUESTIONNAIRE SOLICITATION NUMBER: FA9300-11-R-0002 WHEN FILLED IN THIS DOCUMENT IS SOURCE SELECTION SENSITIVE INFORMATION IAW FAR 3.104 The Air Force Flight Test Center (AFFTC/PKTB), Edwards AFB, CA is conducting a source selection for the Advanced Research and Engineering Services (ARES III) contract. Please complete and return this questionnaire within 5 workdays. Handwritten responses are sufficient. Do not send a copy of the completed questionnaire to the contractor being assessed. You may be contacted by our office to review the results of the questionnaire. The questionnaire information will be used in the evaluation of the Past Performance Factor contained in the Instructions to Offerors of the solicitation. Completed questionnaires will become part of the office source selection records and protected in accordance with FAR 3.104.

SECTION 4: EVALUATION

The following is an explanation of the evaluation codes:

CODEPERFORMANCE LEVEL
BBLUE/OUTSTANDING – The contractor’s performance meets contractual requirements and exceeds many (requirements) to the Government’s benefit. The contractual performance was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
PPURPLE/GOOD – The contractor’s performance meets contractual requirements and exceeds some (requirements) to the Government’s benefit. The contractual performance was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
GGREEN/ACCEPTABLE – The contractor’s performance meets contractual requirements. The contractual performance contained some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
YYELLOW/MARGINAL – Performance does not meet some contractual requirements. The contractual performance reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented.
RRED/UNACCEPTABLE – Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
NNOT APPLICABLE – Unable to provide a score.

EVALUATION AREAS
