Attachment 11 Asbestos Inspection Report and
Lead Based Paint Survey
DEPARTMENT OF THE AIR FORCE

ASBESTOS INSPECTION REPORT FOR BUILDING 674
NEW PAINT BOOTHS
During March, Terry Hess, with 309 MXSS requested an asbestos inspection of Building 674 to new paint booth install. The scope of this project is as follows:
Work Location: Building 674 Work Order #:56906
Statement of Work:
New paint booth inserts.
This inspection report is project specific and is not a valid report for any other project(s) associated with Building 674.
The information provided for this inspection report is based on samples collected by Neldon Anderson (Utah Certified Asbestos Inspector ASB-#0054). Samples for this survey were collected on 24 March 2014. The Science and Engineering laboratory, utilizing EPA Method 600R-93-116, analyzed all samples, which is PLM similar to NIOSH Manual of Analytical Methods 9002. Civil Engineering personnel also reviewed all available asbestos inspection reports databases of suspect asbestos containing building materials (ACM) that could potentially be encountered in this area. The information gathered from all current and previous inspections is shown below by homogenous area.
Footages are based upon the scope of work provided by the requestor.
Grey gravel/Black tar duct (100ln’) was sampled from the northeast corner of paint booths L02 and L03, the south side of paint booth L03, and the south side of paint booth L04. These sampled were analyzed as non-asbestos containing material (GM140846, GM140847, and
GM140848).
Roof flashing black (50ln’) was sampled from the north side of the blower on the north side of paint booth L01 roof, the south side of paint booth L03, and the northeast side of L02 and L03.
These samples were analyzed as non-asbestos containing material (GM140849, GM140850, and
GM140851).
Pipe insulation white/foil/yellow (500ln’) was sampled from the west side of paint booth L01, the west side of the boiler for paint booth L04, the east side boiler for paint booth L02, and the center of the west side of mechanical room 163. These samples were analyzed as non-asbestos containing material (GM140852, GM140853, and GM140854).
Pipe fitting insulation (100 ct.) was sampled from the south side of boiler for paint booth L03, the west side of paint booth L04, the northeast corner of the boiler for paint booth L04, the south

17 April 2014 09:20 Hours side of paint booth L03, and the southwest corner of Bay M7 eyewash. These samples were analyzed as non-asbestos containing material (GM140856, GM140857, GM140858, GM140859, and GM140560).
Duct adhesive red (10ln’) was sampled from the roof duct on the south side of paint booth L04, the roof duct on the north side of paint booth L04, and the roof duct on the south side of paint booth L04. These samples were analyzed as non-asbestos containing materials (GM140861, GM140862, and GM863).
Block filler paint white (25sq’) was sampled from the exterior southeast corner of room 131, the southwest corner of room 162 phase II, and the west side of Bay 6 phase II. These samples were analyzed as non-asbestos containing material (GM140864, GM140865, and GM140866).
All Hard Thermal System Insulation encountered above ceilings, behind walls, in wall chases, in attic spaces, and in mechanical rooms is assumed to be asbestos. This includes all insulation on pipe runs, pipe saddles, tanks, and pipe fittings.
All suspect ACM not identified in the body of this report must be sampled to determine asbestos content. If materials are encountered which have not been previously identified/sampled, stop work and contact Nickolas King at 586-7094.
Remember that the locations of these building materials (homogenous areas) indicate where these materials were found, but do not necessarily mean that these are the only locations where these materials may be encountered during the project. When any new suspect ACM is encountered, stop work immediately and contact 75 CES/CEOHA to identify any new suspect building materials. As per state and federal regulations, if the scope of work or the work location changes, then a new asbestos inspection and asbestos inspection report will be required.
DO NOT DISTURB ANY BUILDING MATERIALS THAT ARE SUSPECTED ACM.
CONTACT NICKOLAS KING AT 586-7094 TO ARRANGE REMOVAL OF THESE
MATERIALS IF THERE IS ANY POTENTIAL FOR THEM TO BE DISTURBED
DURING THIS PROJECT.

MEMORANDUM FOR: 309 MXSS (Terry Hess)
FROM: 75th CES/CEOHA
SUBJECT: Lead Based Paint (LBP) Survey, Building 674 (WO #56906). New paint booth install.
1. On 24 March 2014, Neldon Anderson (Utah Certified Lead Inspector #PB-1072) tested the painted surfaces found in Building 674 that would be impacted by installing new paint booths. This building was constructed in N/A. A Niton XL3t 300 (Serial #30660) was used to test the painted surfaces of these components. Lead based paint was non-detected. Please refer to specific results of our survey; see attachment 1.
2. Limited lead-based paint identification has been completed for Building 674.
It is possible that lead based paint could be encountered during any renovation project.
Additional identification for lead based paint on other building components may be required. The facility manager should ensure that these components are surveyed for lead as well, to meet OSHA Standard 29 CFR 1926.62 construction requirements for lead.
3. The U.S. Department of Housing and Urban Development defines Lead-based paint as any paint, vanish, stain, or other applied coating that has 1mg/cm2 as measured by an X-ray Fluorescence (XRF) Analyzer or laboratory analysis or 0.5 percent by weight (5,000ug/g by dry weight) by laboratory analysis, or more of lead. All other components tested were less than 1 mg/cm2.
4. Any effort to disturb lead paint can create lead dust. Please ensure that appropriate abatement, cleanup, and disposal will be accomplished and that the appropriate safety measures are taken IAW 29 CFR 1926.62. If you have any questions please feel free to call Nickolas King at 586-7094.
