Scope of Work This Statement of Work (SOW) is to design and build three new Paint Booth Inserts and a Paint Mixing/Cleaning room in building 674 for F22 aircraft. New Paint Booth Inserts shall be installed in aircraft docks M5, M6 and M7 in building 674 at Hill Air Force Base (AFB) as shown in Figure 1. Modify existing buildings 674 and 680 as required for new Paint Booth Inserts and associated equipment and facilities.

Project Schedule

1.1. Contractor shall complete project in 462 calendar days from notice-to-proceed as shown in schedule below. Contractor shall not start construction or equipment manufacture until 100% design is completed and approved by Government.

General Conditions

1.2. Site Visit:

1.2.1. A site visit will be held prior to proposal submittal for contractors to examine site conditions. One site visit will be held prior to proposal submittal. The contractor is responsible to investigate the general and local conditions which can affect the work or its cost in accordance with Federal Acquisition Regulation (FAR) 52.236-3. Such investigation may include but is not limited to:

1.2.1.1. The conditions bearing upon transportation, disposal, handling, and storage of materials.

1.2.1.2. The availability of labor, and necessary utilities including water, gas, and electric power.

1.2.1.3. The availability and suitability of pavements and roadways.

1.2.1.4. Prevalent weather conditions or similar physical conditions at the site.

1.2.1.5. The conformation and existing conditions of the ground, pavements and soils.

1.2.1.6. The character and condition of equipment and facilities needed during work performance.

1.2.2. The Contractor is responsible to examine and note the character, quality, and quantity of surface and subsurface materials or obstacles to be encountered insofar as that information is reasonably ascertainable from an inspection of the site. Contractor is responsible to review existing as-built drawings. Any failure of the Contractor to take the actions described and acknowledged in this paragraph will not relieve the Contractor from responsibility for estimating properly the difficulty and cost of successfully performing the work, or for proceeding to successfully perform the work without additional expense to the Government.

1.2.3. The Government assumes no responsibility for any conclusions or interpretations made by the Contractor based on the information made available by the Government. Nor does the Government assume responsibility for any understanding reached or representation made concerning conditions which can affect the work by any of its officers or agents before the execution of this contract, unless that understanding or representation is expressly stated in this contract.

1.3. Work Schedule: All work at Hill AFB shall be accomplished during normal working hours (07:00 to 16:00 Hours) exclusive of Saturdays, Sundays, and holidays unless other times are approved by the Contracting Officer, or otherwise stated in the contract. Coordinate the work schedule with the Aircraft Maintenance Group (AMXG) Project Manager.

1.4. Work in Existing Aircraft Buildings:

1.4.1. When working in the existing aircraft maintenance buildings 674 and 680, the Contractor will be required to work in the close proximity to aircraft and shall use extra caution to insure that construction activities do not endanger aircraft or personnel nor delay aircraft maintenance activities. No work will be allowed over the aircraft.

1.4.2. Because of the very strict tool control within aircraft maintenance facilities, all of the contractor’s tooling brought in the aircraft work areas shall be marked with contractor identification.

1.5. Security

1.5.1. Escorts: Contractors working inside buildings 674 and 680 shall be escorted by 309 AMXG personnel, except in area inside building 674 designated as “Building 674 Construction Zone”. 309 AMXG will only provide two escorts at any one time. Each escort will only oversee up to five Contractor personnel. Contractor personnel shall be in close proximity of and within sight of escort at all times. Contractor shall provide seven days of notice for each piece of work requiring escort.

1.5.2. Building 674 Construction Zone: Contractor shall establish a Construction Zone inside building 674 aircraft docks M5, M6 and M7. Construction Zone boundaries are shown in Figure 2. Construction Zone cannot be established until all aircraft, aircraft parts, tooling and equipment have been removed from Construction Zone area. Contractor shall provide barriers and signage designating Construction Zone. Contractor shall designate person(s) to open and close Construction Zone at the beginning and end of each work day. Contractor Representative shall be escorted by 309 AMXG personnel into and out of Construction Zone at the beginning and end of each work day.

1.5.2.1. While Construction Zone is open the following shall occur:

1.5.2.1.1. Contractor may work inside Construction Zone without an escort.

1.5.2.1.2. Construction Zone shall be open to authorized Contractor and Government personnel only.

1.5.2.1.3. Contractor may enter and exit through exterior doors into Construction Zone.

1.5.2.1.4. Interior personnel and roll-up doors shall remain shut.

1.5.2.1.5. All contractor personnel shall have controlled area badges.

1.5.2.1.6. Cell phones without cameras may be used inside Construction Zone.

1.5.2.1.7. Contractor shall continuously occupy Construction Zone.

1.5.2.1.8. Contractor Representative(s) are responsible for the security of Construction Zone. Contractor personnel shall not go into secure areas inside building 674 without an escort.

1.5.2.2. Contractors Representative and 309 AMXG escort shall conduct a sweep of area before closing Construction Zone for the day. While Construction Zone is closed the following shall occur:

1.5.2.2.1. Contractor shall be escorted.

1.5.2.2.2. All exterior doors shall be shut and secured.

1.5.2.2.3. No intrusion detection system motion sensors shall be blocked.

1.5.3. Staging Area: Contractor may establish a staging area for job trailer, portable toilet facilities, parking vehicles, storage of equipment and materials. Staging area shall be located on building 674 hangar approach ramp south of aircraft docks M5, M6 and M7 as shown in Figure 3. Contactor shall provide and maintain barriers and ropes around perimeter of staging area. Staging area and travel paths shall be kept clean.

Figure 3. Building 674 Site Plan Access to Project Site: Contractor shall access project site through vehicle and personnel gates east of building 674. Contactors shall stay in designated vehicle routes shown in Figure 3. No Privately Owned Vehicles (POVs) are allowed inside flight line fence. Contractor Company Owned Vehicles (COVs) will be allowed inside flight line fence. COVs shall have company logos on both sides of vehicles. COVs shall be parked within Contractors staging area. Contractor personnel may park POVs in parking lot east of building 674.

1.5.4. Hangar and Rollup Doors: Hangar and exterior rollup doors shall only be open as needed to prevent birds and rodents from entering building. Only one hangar door shall be open at one time. Birds and rodents set off motion sensors when intrusion detection system is secured.

1.5.5. US Citizenship: Contractors working inside buildings 674 or 680 shall be United States citizens.

1.5.6. Controlled Area Badges: Contractors working inside flight line fence and inside buildings 674 and 680 will need Air Force (AF) Form 1199 Entry Control Badge with Area Z or be escorted. Contractors with 1199 will escort their own workers by signing for Visitor Badges from Aircraft Security Office in building 100. 309 AMXG Security Office will arrange for AF Form 1199 badges for Contractors who will be working inside flight line fence and inside buildings 674 and 680 for more than one month.

1.5.7. Cell Phones: Cell phones are not permitted inside building 674 or 680 except inside building 674 construction zone when construction zone is open.

1.5.8. Cameras: Contractor shall not use or possess cameras inside flight line fence and inside buildings. This includes cell phones with camera.

1.5.9. Toilet Facilities: Contractor shall provide their own toilet facilities.

1.6. Storage of Materials:

1.6.1. Materials and equipment shall be stored in staging area or inside building 674 construction zone.

1.6.2. Store materials to avoid hindering the aircraft maintenance activities and to avoid damage of materials. All materials and equipment shall be stored and handled to preclude the inclusion of foreign matter and damage by water or breakage. Store packaged materials in original containers until ready for use. Materials showing evidence of water or other damage shall be rejected and removed from the base. All materials shall be stored as recommended by the manufacturer, unless specifically noted otherwise in the contract documents. Store in accordance with general provisions entitled Operations and Storage Areas (FAR 52.236-10).

1.6.3. When pipe and conduit are stored on the site, they shall be stored in racks or blocked to prevent rolling.

1.6.4. When combustible materials are stored on the site they shall be stored in an Occupational Safety and Health Administration (OSHA) approved combustible materials locker at least 50 feet (15 meters) from all buildings unless otherwise specifically indicated by the Contracting Officer.

1.6.5. It is the responsibility of the Contractor to secure all property within the construction site. If government property is included, the Contractor must secure it also, and notify the Contracting Officer and the AMXG Project Manager.

1.7. Covering of Debris: The Contractor shall be responsible to cover all loads of debris before hauling them off base. The Contractor shall provide a covering to keep the debris on the truck; however, in the event debris does fall out of the truck while hauling they shall immediately clean up all debris.

1.8. Removals and Salvage:

1.8.1. Contractor shall be responsible for the removal and/or dismantling of items, which are required for proper completion of work as applicable to each section.

1.8.2. All materials and equipment resulting from this work not designated for reuse in new work nor designated to be salvaged for the government, become property of the Contractor, and shall be removed from site as the materials and equipment are accumulated and disposed off base. All excavated material (dirt, asphalt, concrete, etc.) and construction/demolition debris shall be managed in accordance with Environmental Protection paragraph of this Section of SOW.

1.9. Protection of Government Property and Personnel:

1.9.1. The Contractor shall conduct his work so the Government property and personnel, other personnel, and work areas, shall be protected at all times from inconvenience, damage of any nature, or injury caused by this work until completion of the contract.

1.9.2. The Contractor shall furnish and spread drop cloths in the work areas as required to protect surfaces, etc.

1.9.3. In the event of damages of any nature caused by this work due to improper protection, precaution of safety measures, such damages shall be repaired or such property shall be replaced by the Contractor at no expense, cost, or charge to the Government. In the event the Contractor does not satisfactorily repair or replace such damage caused by the work of this contract, the Government reserves the right to make the necessary corrections and deduct from the contract price the cost to the Government for inconveniences, labor, materials, etc. involved. Reference General Provisions Protection of Existing Vegetation, Structures, Equipment, Utilities and Improvements (FAR 52.236-9).

1.9.4. The Contractor shall furnish, place, and maintain all required barricades as directed by the Safety Office and the AMXG Project Manager, and access drives and doors will remain clear at all times.

1.9.5. It shall be the responsibility of the Contractor to prevent any damage to existing pavements beyond the limits of indicated pavement removal. Existing pavements that are to remain and are damaged by the Contractor shall be replaced in kind or repair as directed by the Contracting Officer at no expense to the Government.

1.9.6. The Contractor shall conduct operations to offer the least possible obstruction and inconvenience to public traffic, and all traffic shall be permitted to pass through work with as little delay as possible. Where the nature of construction operations in progress and the equipment and machinery in use are of such character as to endanger passing traffic, the Contractor shall provide such lights and signs, erect such fences or barriers, and station such guards as may be necessary to give adequate warning and to avoid damage or injury to passing traffic. Signs, flags, lights and other warning and safety devices shall conform to applicable city, county, and state requirements.

1.10. Housekeeping and Daily Cleanup: The Contractor shall keep the total construction site, structures, and access ways free of debris and obstructions at all times. Work will not be allowed in those areas that, in the opinion of the AMXG Project Manager, have unsatisfactory cleanup and housekeeping at the end of the preceding days' normal work shift. At least once each day all areas shall be cleaned by the Contractor.

1.11. Submittals & Shop Drawings:

1.11.1. Design Submittals: See Design Section of this SOW.

1.11.2. Equipment Submittals and Shop Drawings: Provide two copies and contain sufficient literature, catalog cuts, brochures, etc., to show compliance with the design specifications and plans. Submit with standard AF form 3000, dated, signed and identify by individual specification section the item submitting for. Incomplete AF3000 or insufficient documentation of submittal items on same will be rejected and have to be re-submitted. Items of related equipment or materials shall be submitted at one time. Each copy shall be in a separate binder. Mark all submittals to show choices and applicable options. Equipment submittals shall be given to the AMXG Project Manager as soon as possible, but in no case shall submittals exceed twenty one (21) days after the construction start date. No payments will be authorized for materials or work, which do not have approved submittal requirements.

The government will within 14 calendar days return a minimum of one copy of the submittal marked to indicate approval or disapproval or approved as noted. The Contractor shall make any corrections indicated on the submittals. If the Contractor considers any correction to constitute a change to the contract drawings or specifications, written notice will be given to the Contracting Officer. Disapproved items shall require resubmission for approval within 14 calendar days of Contractor’s receipt. The Contractor will not be allowed to claim for time because of disapproved submittals.

1.11.2.1. Submittals approved by the Government shall not relieve the Contractor from responsibility for complying with the requirements of this contract (See FAR 52.236-5). No substitutions for approved items, which meet contract requirements, will be allowed without approval of the Contracting Officer. If submittals show variations from the contract requirements, the Contractor shall describe such variations in writing at the time of submission.

1.12. Materials and Workmanship:

1.12.1. All equipment, material, and articles incorporated into the work covered by this SOW shall be new and of the most suitable grade for the purpose intended, unless otherwise specifically provided in this contract. References in this SOW to equipment, material, articles, or patented processes by trade name, make, or catalog number, shall be regarded as establishing a standard of quality and shall not be construed as limiting competition. The Contractor may, at its option, use any equipment, material, article, or process that, in the judgment of the Contracting Officer, is equal to that named in the SOW.

1.12.2. All work under this contract shall be performed in a skillful and workmanlike manner. The Contracting Officer may require, in writing, that the Contractor remove from the work any employee the Contracting Officer deems incompetent, careless, or otherwise objectionable.

1.12.3. In accordance with FAR 52.236-6, at all times during performance of this contract and until the work is completed and accepted, the Contractor shall have on the worksite a competent superintendent who is satisfactory to the Contracting Officer and has authority to act for the Contractor.

1.13. Weekly Construction Meetings: During construction, Contractor shall conduct a weekly meeting to discuss work progress and issues. Meetings shall be held at building 674 at the same time each week. Contractor shall provide agendas and keep minutes for each meeting.

1.14. Cold Weather Precautions:

1.14.1. Contractor shall be responsible for proper application of weather sensitive materials and shall not apply any such materials when weather conditions fall below the minimums recommended by the material manufacturer.

1.14.2. Contractor shall cover all concrete installed in weather below 32 °F with approved insulating blankets.

1.14.3. Contractor is responsible in weather below 40 °F that water lines, including fire protection systems, do not freeze.

1.14.4. Contractor shall maintain interior portions of building under construction above 50 °F.

1.14.5. The only types of temporary heating devices permitted are Underwriters Laboratories (UL) approved electric heaters. The Contractor shall physically monitor all temporary heating devices at least every four hours. Exception to this requires written approval from the Base Fire Department.

1.15. Use of Utilities:

1.15.1. The contractor will be allowed the use of electrical and natural gas utilities without reimbursement while performing work under this contract.

1.15.2. Toilet Facilities: Contractor shall provide their own toilet facilities. Contactor toilet facilities shall be located in staging area.

1.16. Excavation Permits:

1.16.1. No excavation shall be done prior to receiving a Base Civil Engineering (CE) Excavation Permit. Permits are available from the Red Stake Office in Building 593 South. If excavation is started without obtaining a permit, the Contractor shall be held liable for repairs of any broken utility lines or other damage resulting from the broken lines.

1.16.2. The Contractor shall request a permit 30 calendar days prior to scheduled start of digging.

1.16.3. Hand digging shall be required to locate utilities shown on the contract drawings, Excavation Permit, or three feet on either side of locations identified by Base Maintenance Shops.

1.16.4. The Contractor shall protect from damage all existing improvements and utilities at or near the work site, the locations of which are made known to or should be known by the Contractor.

1.16.5. The Contractor shall repair any damage to those facilities, resulting from failure to obtain necessary permits and utility marking or failure to exercise reasonable care in performing the work. If the Contractor fails or refuses to repair the damage promptly, the Contracting Officer may have the necessary work performed and charge the cost to the Contractor.

1.16.6. The Contractor shall verify the elevations of existing piping, utilities, and any type of underground obstruction not indicated or specified to be removed but indicated in locations to be crossed by piping, ducts, and other work to be installed. Verify elevations before installing new work closer than nearest manhole or other structure at which an adjustment in grade can be made.

1.17. Utility Outages: All utility outages shall be scheduled during off duty hours and weekends unless otherwise approved by the AMXG Project Manager. All outages shall be scheduled as far in advance as possible with the AMXG Project Manager and in no case less than 24 calendar days before the outage. The Contractor shall obtain in writing from the AMXG Project Manager a statement or schedule giving the permissible times for the outage or closure for particular installations and the maximum time allowed for such outage or closure. The Contractor shall strictly observe such schedules and will be held responsible for any violations.

1.18. Burning Permits: Open Burning of any sort is strictly prohibited.

1.19. Welding, Cutting and Brazing: AF Form 592 is required daily for all welding, cutting, brazing, soldering and similar hot work. The form shall be properly filled out and displayed while all hot work is underway. The Contractor will be required to attend the Welding, Cutting, Brazing certification class prior to construction startup. This class is conducted on the first Thursday of every month at the Base Fire Station Building 9.

1.20. Airfield Construction Waiver: An air field construction waiver shall be approved before and in-place while any construction occurs.

1.20.1. Contractor shall request an air field waiver at least 90 calendar days prior to start of construction. AMXG Project Manager will complete and request approval of airfield construction waiver by CE Facility Planning Committee. Contactor shall provide AMXG Project Manager with the information required for the air field construction waiver. Minimum information for the air field construction waiver includes: start and finish dates of construction, crane height, dimensions of staging area, and type of barriers used around staging area. Contractor shall abide by constrains of airfield construction waiver.

1.20.2. Cranes shall have air field obstruction flags on end of boom. Crane boom shall be lowered below building 674 roof height at the end of each day. Contractor shall maintain barriers around staging area on hangar approach ramp.

1.21. Safety Requirements: The Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1, and the OSHA Standards for Construction (Title 29, Code of Federal Regulations (CFR) Part 1926 and Part 1910) and AF OSHA standards are all applicable to this contract. In case of conflict the most stringent requirements of the standards is applicable. If recurring violations and/or gross violation indicate that the safety performance is unsatisfactory, corrective action shall be taken as directed by the Contracting Officer or the AMXG Project Manager.

1.22. Environmental Protection: The Contractor is required to comply with all applicable local, state, federal, and AF laws and regulations dealing with environmental and workplace management and protection. In the event of a conflict between any laws or regulations, the most stringent shall apply. These items include, but are not limited to, the following:

1.22.1. Vehicles and other powered equipment will meet local (Davis County) air quality regulations for emissions. They will be mechanically sound and have all seals, grease rings, and other containment systems in place to ensure against leakage. Drip pans will be maintained such that overflow and spills do no occur. There will be no activities for equipment maintenance including oil changes, filter replacements, or hydraulic equipment repair performed on base.

1.22.2. All solid waste shall be collected and disposed of on a daily basis. There will be no accumulation of solid waste unless properly containerized or put within a fenced area to prevent material being blown about by the wind. No liquids will be put into base dumpsters. This includes but not limited to paints, oils, chemicals, compressed gas, or pesticides.

1.22.2.1. Paint Cans: Only empty paint cans shall be allowed in dumpsters. Empty containers have to comply with the “Resource Conservation and Recovery Act (RCRA) empty rule” before they become a non-hazardous solid waste. The rule defines empty as removing all pour-able, pump-able or aspirate-able contents. For paint cans a best management practice is to use all the paint, let the residue dry, then put the empty can in the dumpster. For cans of spray paint, spray until no more paint comes out, and then put the empty can in the dumpster. If the nozzle breaks and the contents cannot be sprayed, the can may be turned in to the Hazardous Waste Control Facility. 1.22.2.2. Asphalt: Asphalt debris shall be managed by disposing in an off-base Class I, II, III, or V permitted landfill.

1.22.2.3. Concrete and Excavated Soils: Concrete and excavated soils that have been determined by the pre-contract laboratory analysis to be “clean” shall be managed by disposing in an off-base Class I, II, III, IV or V permitted landfill.

1.22.2.4. Wood: Wood shall be managed by disposing in an off-base Class I, II, III, IV or V permitted landfill.

1.22.2.5. All solid waste other than clean concrete, asphalt, excavated soil, and wood shall be sent to off base permitted facilities. Other questions regarding the disposal of solid waste should be directed to the AMXG Project Manager.

1.22.3. All hazardous wastes such as sandblast media, chlorinated solvents, paints and paint thinners, and fuels will be labeled and an inventory management system initiated to ensure timely removal and proper disposal. There shall be no on-base disposal allowed, including use of drains (sanitary, storm, or industrial) or the ground. The contractor is responsible for properly collecting and disposing contractor generated hazardous waste. Exceptions may be made for small amounts of hazardous waste on a case by case basis. With approval, Environmental Management Division (75 GEG/CEV) may opt to provide containers and take possession of the waste and arrange disposal. Contractor shall include waste handling and disposal costs in their budget because exceptions are not guaranteed. The contractor shall follow the basic requirements which are in the Base Hazardous Waste Management Plan for site requirements, signage and site management, not to include supplies, and services designed for the Base and other costs mentioned above.

1.22.3.1. All drums/containers must be labeled with a hazardous waste label. The proper Department of Transportation (DOT) shipping name, UN numbers, Environmental Protection Agency (EPA) waste number, generator information, and accumulation start date on the label must be filled out. The label must be placed in the upper third of the drum/container. Drums/containers shall be kept closed except when adding waste. Hazardous waste drums must be stored in an area authorized by 75 CEG/CEV.

1.22.3.2. All drums used to store hazardous waste must be non-leaking and safe to handle. Drums that are rusty, dented, or leaking should be over-packed. Drums and/or over-packs must be purchased by the Contractor. All drums purchased by the Contractor must be DOT approved for containing Hazardous Waste which may include the following specification numbers 5B, 17E or 17H. The specification numbers are stamped on the bottom of the drum. The contractor shall be responsible to see that proper containers are used. All hazardous waste sites upon the date of receiving hazardous waste must be inspected weekly. Hazardous waste containers can only be stored for 90 days after the accumulation start date

1.22.3.3. The Contractor shall coordinate with a Hazardous Waste Customer Support Person and obtain from 75 CEG/CEV, north end of Building 5, a site book, a container log and a weekly inspection form. The contractor shall fill out an inspection checklist each week the containers are on the site. Completed checklists must be forwarded to 75 CEG/CEV (customer support) every Friday until the drums are properly disposed of by the Contractor. The contractor shall provide a properly filled out Hazardous Waste Manifest to 75 CEG/CEV for review prior to the removal of any Hazardous Waste from the Base and shall only use a contractor authorized to haul Hazardous Waste to transport the Hazardous waste off Base while meeting all the requirements of 49 CFR.

1.22.4. Petroleum Oils Lubricants (POL) storage larger than 55 gallons requires secondary containment. This may be accomplished by a double-walled container or by a catch basin. Siting of this will be approved by the Base Fire Department and the 75 CEG/CEV.

1.22.5. Storage of oils, greases, chemicals, or other liquids will require a fully contained facility for spill prevention and security. A minimum of 40 pounds (18 kg) of absorbent material per 44 gallons (167 liters) of chemical/oil/grease will be on hand for spill control.

1.22.6. All spills shall be reported by dialing 911 and giving information as to spill location, type of material and estimated quantity, and if the spill is contained. The Contractor will ensure appropriate personnel protective equipment (PPE) is available to take care of spill cleanup and handling of residue.

1.22.7. Spray painting shall utilize high-transfer efficiency equipment with low-volatile organic compound (VOC) paints or water base paints. The VOC content of low-VOC paint shall be 3.5 pounds per gallon or less for primers, topcoats, and specialty coatings--for clear coats, 4.3 pounds or less. If a low-VOC paint is not available for the application, a paint waiver shall be obtained from 75 CEG/CEV by contacting the CE Project Manager. Uncontrolled spray painting with high-VOC paints shall not be performed.

1.22.8. Contractor sites will be maintained at all times. Damage due to erosion and control of fugitive dust will be the responsibility of the Contractor. An on-site review will be conducted by the Project Manager, the Construction Engineer, and the 75 CEG/CEV prior to use of any location for contractor setup. A follow-up program for site overview will be maintained by all parties. Release of the site by the Contractor will be accompanied by a final site review, at which time site deficiencies will be noted. The Contractor will have 14 calendar days to correct deficiencies. Final contract payment will be withheld pending completion of the deficiency list.

1.22.9. Asbestos and Lead-Based Paint: The Contractor is cautioned that materials in and around this project may contain asbestos or be coated with Lead-Based Paint (LBP). The government will make every effort to locate and identify all Asbestos Containing Materials (ACM) and LBP prior to bidding; however this is not always possible. These materials are often hidden and cannot be discovered until after demolition has begun. The failure of the government to identify all ACM and LBP in no way relieves the Contractor from his legal obligation to comply with state and federal regulations regarding the handling of asbestos, lead, or LBP. If suspected asbestos containing materials or LBP surfaces are encountered, immediately cease work and notify the Contracting Officer and the project manager immediately. Do not conduct or continue with any work that will violate any AF, local, state or federal regulations regarding asbestos, lead, or LBP. If suspected materials or surfaces have not been disturbed, secure and post signs in the area where the materials are located to ensure that they are not disturbed. If the suspected materials have already been disturbed, secure and post signs in the area where the material was originally located, any areas to which materials have been moved, and any containers that suspect materials may have been placed in. Take all necessary steps to ensure that materials are not further disturbed, moved, or disposed of until directed to do so by the Contracting Officer. Failure to notify the government promptly or failure to comply with state and federal regulations will be grounds for termination of this contract and may result in other appropriate civil and/or criminal actions. The Contractor will be fully responsible for any and all fines or other penalties resulting from his acts and /or omissions pursuant to law and regulation. At the Pre-Construction Conference, the contractor will be required to sign the "Contractor's Notification of Hazardous Materials Requirements" at the end of this Section.

1.22.10. Standards for Demolition and Renovation: For demolition and renovation the contractor shall comply with 40 CFR 61.145 and the Utah Administrative Code R307-801. The contractor shall complete the applicable Utah Division of Air Quality notification form with the assistance of the government's project manager and the Base Asbestos Manager. The contractor shall submit the applicable form to the State with a copy sent to the government's Project Manager. Forms are available at the web site www.eq.state.ut.us/eqair/haps/asbestos/forms.htm.

1.22.11. All tanks used for fuel storage must have spill containment for 110% of stored fuel. Any tanks needed for chemicals, oils, and other liquids must have spill containment for 110% of stored product.

1.22.12. If at any time, an authorized inspector for 75 CEG/CEV feels that regulatory requirements listed above are not being followed, 75 CEG/CEV reserves the right to notify the Contracting Officer and recommend corrective action.

1.22.13. Banned Ozone Depleting Substances (ODS): Heating, Ventilating and Air Conditioning equipment which use chlorofluorocarbon (CFC) refrigerants are strictly prohibited. This includes but is not limited to R-11, R-12, R-13, R111, R-112, R-113, R-114, R-115, R-211, R-212, R-213, R-214, R-215, R-216 and R-217. Fire protection systems using Halon 1211, 1301 or 2402 are also prohibited. Other substances banned from use on the work site include carbon tetrachloride, methyl chloroform and methyl bromide.

1.22.14. Storm Water Permitting: Contractor is prohibited from pouring or washing water into the storm drain system.

1.22.15. Sanitary and Industrial Waste Water:

1.22.15.1. Some types of waste are prohibited from entering any drain. Contractor shall get permission before dumping any waste down a drain. Permission must be obtained from the government project manager in all cases and from Base waste water managers when appropriate.

1.22.15.2. All drains (storm, sanitary and industrial) shall be protected from accepting spilled materials during the entire project.

1.22.15.3. Floors shall be cleaned by sweeping, not washing.

1.23. Contractor’s Base Credentials:

1.23.1. The procedure for obtaining base identification passes for contractor personnel to work on any Air Force Materiel Command (AFMC) base shall be as follows:

1.23.1.1. The Contractor shall submit a written request on company letterhead stationery, if available, to the Program Manager specifying:

1.23.1.1.1. Contract number.

1.23.1.1.2. Location of work site:

1.23.1.1.3. Date entry to the base must begin and contemplated termination date of entry.

1.23.1.1.4. Names of contractor and subcontractor employee requiring access to the base.

1.23.1.1.5. The name of the individual who will submit the Request of Identification Credentials for each employee for whom identification credentials are needed.

1.23.1.2. The Program Manager will:

1.23.1.2.1. Endorse the request.

1.23.1.2.2. Attach a copy of the contract cover page and any other pages that provide performance information, such as the need for and duration of access to the work site.

1.23.1.2.3. Forward this request to the Security Forces, Pass and Registration Office of the installation where the work is to be performed.

1.23.1.2.4. Provide blank Application for AFMC Identification Card, AFMC Form 496 and Unescorted Entry Authorization Certificate, AF Form 2586 to the Prime Contractor.

1.23.1.3. The Prime Contractor shall be required to complete and submit AFMC Form 496, for each of the firm's employees and for each subcontractor employee who must have access to the installation.

1.23.1.4. Controlled Area Badges: Contractors working inside flight line fence and inside buildings 674 and 680 will need AF Form 1199 Entry Control Badge with Area Z or be escorted. Contractors with 1199 will escort their own workers by signing for Visitor Badges from Aircraft Security Office in building 100. 309 AMXG Security Office will arrange for AF Form 1199 badges for Contractors who will be working inside flight line fence and inside buildings 674 and 680 for more than one month.

1.23.1.5. The Prime Contractor shall also request AF Form 75, Visitor/Vehicle Pass when the AFMC Form 496 is submitted. To obtain the vehicle decal from the Security Forces, Pass and Registration Office, the Contractor shall produce:

1.23.1.5.1. A valid driver’s license.

1.23.1.5.2. Proof of financial responsibility or insurance, which meets the minimum requirements of the contract clause, entitled "Required Insurance."

1.23.1.5.3. Current vehicle registration.

1.23.1.5.4. Proof of Davis County emissions certification if the vehicle is not registered in a county within the State of Utah, which requires emissions testing.

1.23.2. Contractor employees, at all times while on a military installation, shall wear visible contractor-provided identification either as a part of, or attached to, their outer clothing. The identification shall clearly identify the individual as being a contractor employee.

1.23.3. During performance of the contract, the Contractor shall be responsible for obtaining required identification for newly assigned personnel, and for prompt return of credentials to the Security Forces, Pass and Registration Office, for any employee who no longer requires access to the work site.

1.23.4. At the termination or completion of the contract, or upon the expiration of credentials (if any such expirations are specified), the Contractor must be sure that all base identification credentials and vehicle registration decals for all contractor and subcontractor employees are returned to the Security Forces, Pass and Registration Office.

1.23.5. Prior to submitting an invoice for final payment, the Prime Contractor shall obtain a clearance certification from the issuing Pass and Registration Office which states that all base identification credentials and vehicle decals have been returned or "accounted for." This certification shall be attached to the final invoice at the time of submittal for payment. Failure to comply with these requirements will result in withholding final payment until compliance is effected.

1.24. Construction Inspection: Contractor shall provide construction inspection and comply with FAR 52.246-12 Inspection of Construction.

1.25. Testing:

1.25.1. All tests required by this contract are the sole responsibility of the contractor unless expressly stated otherwise. The Contractor shall notify the AMXG Engineering Project Manager at least 24 hours in advance of any testing.

1.25.2. The Contractor shall submit test reports to the AMXG Project Manager in no more than seven days following test completion.

1.25.3. The Contractor shall not proceed with any work, which would cover up the work being tested until the work being tested has been approved by the Contracting Officer.

1.26. Final Cleaning: Perform final cleaning prior to final inspection

1.26.1. Execute prior to final inspection.

1.26.2. Clean surfaces exposed to view. Remove temporary labels, stains, and foreign substances: polish transparent and glossy surfaces, wet mop tile or sealed concrete floors, vacuum carpeted floors, broom clean other floors. Clean equipment and fixtures to a sanitary condition, clean or replace filters of mechanical equipment. Any additional final cleaning as specified in other sections.

1.26.3. Clean site of all construction related and project related debris.

1.26.4. Remove waste and surplus materials, rubbish, and construction facilities from the work areas and the site.

1.27. Warranties: This project shall be under warranty as set forth in the General Provision entitled Warranty of Construction (FAR 52.246-21).

1.28. Operation & Maintenance (O&M) Manuals and Training:

1.28.1. Provide O&M manuals for Paint Booth Inserts. Provide one electronic and four hard copies of O&M manuals. Hard copies of O&M manuals shall be bound in as many three ring (straight ring, not curved) binders as needed. Each binder shall be labeled with project title. O&M Manuals to be submitted and approved. At a minimum O&M manuals shall include:

1.28.1.1. Index.

1.28.1.2. Complete drawings including wiring diagrams.

1.28.1.3. Sequence of operation.

1.28.1.4. Human Machine Interface (HMI) screens.

1.28.1.5. Alarm list with cause and effect.

1.28.1.6. Preventative maintenance lists and schedules.

1.28.1.7. Spare parts list.

1.28.1.8. Recommended spares list.

1.28.2. Provide training, including training manuals, as required by other sections of this SOW.

1.29. As-Built Drawings:

1.29.1. Maintain a complete set of full-size prints of the design drawings for record mark-up purposes throughout the construction period. Whenever changes occur the contractor shall immediately mark-up these drawings in red, to show both changes and the actual installation in sufficient detail to form a complete record of work accomplished. The marked-up drawings shall be complete, legible and precise. Whenever a red-mark is made the contractor shall ensure related sections, details, legend, plans, profiles, schedules and notes are marked accordingly to avoid conflicting data on all other sheets. Particular attention should be given to work, which will be concealed and difficult to measure and record after construction, and work which may require servicing or replacement during the life of the facility.

1.29.2. The contractor shall transfer the changes from the working mark-up prints to the original electronic AutoCAD files. Final as-built drawings shall be prepared after the completion of Construction. Provide one electronic and one hard copy of record drawings. Electronic copy shall be on compact disc in adobe acrobat (pdf) format and AutoCAD version 2013 format. Hard copy shall be½ size 11” x 17” drawings fastened together is proper order.

1.29.3. The government inspectors will review the contractor’s marked-up drawings when verifying the performance shown by progress reports. If the drawings are not current, the progress report will be adjusted downward to reflect the contractor’s non-compliance. Final payment will not be authorized until complete and accurate as-built drawings are accepted by the AMXG Project Manager.

1.30. Transfer and Acceptance of Military Real Property:

1.30.1. At the completion of the project the contractor shall prepare Real Property Data utilizing the DD1354 Form. Instructions to complete the DD Form 1354 are contained in Unified Facilities Criteria (UFC) 1-300-08.

1.31. Closeout Procedures:

1.31.1. Pre-final Inspections. The Contractor shall notify the Contracting Officer at least five working days in advance of a pre-final inspection. Before a pre-final inspection will be scheduled all work must be complete and the following items submitted for review:

1.31.1.1. Commissioning Completed and report including test & balance submitted.

1.31.1.2. O&M Manuals.

1.31.1.3. Training completed.

1.31.1.4. DD Form 1354.

1.31.1.5. List of installed equipment furnished under this contract. Include name plate information, location (room number), model number, serial number, capacity, manufacturer, equipment supplier, spare parts list, warranty etc.

1.31.2. Final Inspections. Contractor shall remedy all deficiencies identified in the pre-final inspection and notify the Contracting Officer a minimum of five working days in advance of a final inspection. Upon completion of final inspection, and all deficiencies have been corrected, the Contractor shall have five working days to submit the following:

1.31.2.1.Warranty information
1.31.2.2.Complete As-Built Record Drawings.

1.31.3. The Contracting Officer will notify the Contractor in writing of additional items to be completed or corrected and any deficiencies remaining on the project. The contract will not be closed nor final payment made until all items have been received.

Design

1.32. Provide a complete Architect-Engineer (A-E) design for work covered by this SOW. The contractor shall ensure design solutions meet the requirements SOW. All design services shall be performed under the direct supervision of architects and engineers licensed in the respective disciplines. A-E shall be responsible for the professional quality, technical accuracy, and the coordination of all designs, drawings, specifications, and other services furnished under this contract. A-E shall, without additional compensation, correct or revise any errors or deficiencies in its design drawings, specifications and other services.

1.33. Design shall include but not be limited to the following:

1.33.1. Architectural:

1.33.1.1. Floor plan and sections.

1.33.1.2. Relocation of existing fixtures, furniture, equipment, etc.

1.33.1.3. Roof and wall penetration details.

1.33.2. Structural:

1.33.2.1. Floor and footing support for Paint Booth Inserts.

1.33.2.2. Roof and wall penetration.

1.33.2.3. Guy wire roof anchors.

1.33.3. Mechanical/Plumbing:

1.33.3.1. Removal of existing radiant heating.

1.33.3.2. New Paint Booth utility connections,

1.33.3.3. Relocation of existing fixtures, equipment and piping.

1.33.3.4. Modifications to existing wash water system.

1.33.4. Fire Suppression:

1.33.4.1. Wet sprinkler systems.

1.33.4.2. High expansion foam systems.

1.33.5. Electrical:

1.33.5.1. New Paint Booth power connections.

1.33.5.2. Relocation of existing fixtures, furniture, devices, equipment, conduit and wiring.

1.33.5.3. Lightning Protection.

1.33.5.4. Lighting in hangar.

1.33.5.5. Intrusion Detection System (IDS).

1.33.5.6. Telephone/Date.

1.33.5.7. Public address system.

1.33.5.8. Fire alarm and mass notification systems.

1.33.6. Paint Booth Inserts:

1.33.6.1. Paint Booth structure.

1.33.6.2. Catwalk system.

1.33.6.3. Heating, Ventilation, and Air Conditioning (HVAC) systems.

1.33.6.4. Electrical power and lighting.

1.33.6.5. Controls.

1.34. Design Submittals: Submit the designs show below to the Government for Review. Government shall review each design submittal and provide written comments of A-E. A-E shall conduct a design review meeting for 65% and 95% design submittal to discuss Government comments. Design meetings shall be at Hill AFB in facility provided by Government. A-E shall submit Government review comments annotated with actions or responses with the next design submittal.

1.34.1. 30% Concept Design: Purpose of the concept design shall be to verify project requirements and explain the designer’s approach. A-E shall present concept design at a design charrette meeting. A-E shall record verbal comments provide during design charrette. Concept design shall consist of floor plan sketches, building code survey, any potential conflicts, and descriptions of Paint Booth Inserts, architectural, structural, mechanical, plumbing, fire suppression and electrical features. Provide one electronic and ten hard copies of concept design.

1.34.2. 65% Preliminary Design: General direction and details of the design shall be firmly established and clearly indicated. Drawings shall include basic floor plans and sections, rough details, equipment layouts, schedules, tables and other information required to clearly illustrate the design. Specifications shall be a rough draft of all specifications sections. Design analysis shall include complete design calculations, product and equipment data, and any other information required to clearly illustrate and describe the design. Provide one electronic and six hard copies of concept design.

1.34.3. 95% Pre-Final Design: All design work shall be complete at this pre-final design stage. Drawings shall include all floor plans, sections, details, schedules and tables. Specifications and design analysis shall be completely edited and final. Provide one electronic and six hard copies of Pre-Final design.

1.34.4. 100% Final Design: Corrected design with all design review comments resolved. Design shall be ready for manufacture and installation of Paint Booths. Provide one electronic and three hard copies of Final design.

1.35. Design Deliverables:

1.35.1. Electronic: Electronic deliverables shall be provided on compact disc in plastic case. Compact discs shall be labeled with project title and design submittal. Provide all design submittal in adobe acrobat (pdf) format. Final design submittal drawings shall also be provided in AutoCAD version 2013 format. Final design submittal specifications shall be provided in Microsoft WORD format.

1.35.2. Hard Copies: Plans shall be ½ size 11” x 17” drawings fastened together is proper order. Specifications, design analysis and other materials shall be bound in as many three ring (straight ring, not curved) binders as needed.
