MEMORANDUM FOR POTENTIAL OFFERORS
FROM: ACC AMIC/PKCA
SUBJECT: Solicitation FA4890-11-C-0005, T-38 Aircraft Maintenance Program (AMP) Langley Operating Location Site Visit, and General Information
1. The T-38 AMP Langley Operating Location (OL) site visit is scheduled for Thursday, 15 October 2015 from 1000-1130. Government personnel will meet approved contractor personnel attending the site visit at 1000 at the LaSalle gate visitor center, LaSalle Ave, Hampton, VA 23665. The government will provide transportation to/from the visitor center and Langley OL.
2. Registration: Each Offeror is limited to a total of three (3) personnel to attend. Please submit the following information no later than 28 Sep 2015 to allow security screening before the event start date.
i. Name of Company (Include main telephone number and email, if any)
ii. CAGE Code
iii. DUNS Number
iv. Company Size for NAICS 448190 (SBA Size Standard $32.5M)
v. Company’s anticipated role for T-38 AMP (Prime or sub-contractor)
vi. Personal Information for each attendee
· Full name: (Title) First Middle Last
· Gender
· Contact number and email address
· SSAN
· Date of Birth
· State Driver’s License (State and #)

3. The site visit to the work locations is limited to three (3) personnel per offeror due to limited transportation and government escort restrictions. Government escorts WILL NOT answer questions regarding current contract performance. Phones, cameras, or any other type of recording devices are not allowed during the site visit. Personnel identified using or possessing phones/camera will be separated from the group and removed from the facility and/or tour by USAF Security Forces personnel.
4. The Government will accept written questions at the conclusion of the tours and provide oral answers as best possible on the site and will follow-up with written answers published on FBO. Written questions will also be accepted post-site visit. .
