5) "Financial Management Report" At a minimum, the following information shall be included in the CDRL;
All Locations:
• Summary listing of all expenditures
• Contract identification data
• Funding authorized to date
• Cumulative billings
• Actual cumulative costs to date
• Actual costs incurred since last reporting period
• Estimated costs through end of contract year
• Cost variances comparing the estimated cost of completion to amount of funds authorized and percentage of funding obligated or spent
• Summary detailing fixed price cost line items with descriptive data and total FY funding for each line item with a cumulative listing of applicable modifications affecting each item
• Quarterly FY cost projections for each cost element
• Functional description including local purchase equipment, software, supplies and services, commercial publications and software, training, engineering support study and implementation, training, travel, overtime, emergency corrective, maintenance and other O&M services. List all expenditures itemized by performance location (WOC, SOC, etc) and site to include charge code, Contract Line Item Number (CLIN), description, unit of issue, unit price, total price
• Document financial information to reflect current and previous performance year (PY) contract cost data, projected funding requirements, and performance location-specific expenditures

5) "Status Report" Provide the government with a summary of significant accomplishments, issues, and status on all projects, with start and projected end dates. Provide level of support and use of any subcontracted efforts to complete projects. Identify impact of project if not completed on time and any resources required for successful completion.
Projects are defined in Appendix A.
8) The document will be accepted/rejected by the Government within 30 calendar days after receipt. Contractor shall revise and resubmit the document within 15 calendar days after receipt unless otherwise directed by the CO.

10, 11, 12, 13) Contractor shall provide a contingency plan that provides reasonable assurance of maintaining operations at all site locations in the event of contingencies, crisis conditions, or a strike. Plan shall include contractor's strategy to retain and rotate site personnel as required to meet mission requirements in these conditions. Include in the plan procedures for supporting the Training Squadrons should they be required to operate 24/7 due to a crisis or contingency. The plan shall include for the protection, removal, or destruction of classified material in case of fire, natural disaster, civil disturbance, terrorist activities or enemy action, to minimize the risks of its compromise. Subsequent changes to the management plan shall be submitted to the CO for review and approval.

“Supportability Analysis Report.” The Supportability Analysis Report shall be used to provide recurring supportability reviews focusing on high failure rate items impacting system availability, software/hardware obsolescence, and diminishing manufacturing sources and material shortages (DMS/MS). This report shall include recommended solutions.
The Supportability Analysis Report shall include the following information:

"Top Failures."
The contractor shall identify and rank the top failure items for the UASOCS during this reporting period. If failure items are less than 10, then indicate no other failures. The contractor shall also provide the failure data for components that have more than three failures per quarter or fail more than once per quarter at WOC/SOC.

16_REMARKS_Continued: "Metrics" Utilize performance metrics listed in AFI 10-602, Attachment 11 and App M to compute failure data, supply demand data, repair data, and field-level feedback to identify high failure rate components detracting from operational availability. The Mean Time Between Critical Failure (MTBCF) & Mean Repair Time (MRT) shall include Long Haul Communications, Mission System (Appendix O), Video Dissemination Hub (VDH), and Video Teleconferencing (VTC). The contractor shall provide the MTBCF and MRT hours of each system. Format this data by WOC/SOC for each item as applicable. Graph the Operational Availability rates and MTBCF and MRT hours displaying the previous 12 full months of data from the submittal date of the report. 5) The contractor shall develop and implement a MCP IAW technical orders, commercial standards, applicable manuals, and UAS operational guidelines. The MCP shall include procedures and processes for prioritizing and controlling maintenance actions across all disciples to meet operational mission/availability. The contractor shall administer MCP policies/procedures, instructions and implement the plan at all operating locations.
The MCP shall identify the procedures used to document, schedule, and execute maintenance actions and logistics tasks to ensure all facilities and equipment are operated and maintained IAW the Performance Work Statement (PWS).
The MCP shall include procedures for scheduling, performing, and documenting Corrective Maintenance (CM), Preventative Maintenance Inspection (PMI), corrosion control program, equipment/system technology refresh program, government-approved modifications/upgrades, Help Desk function, and procedures for coordinating with customers/outside agencies (include contact information). Include procedures for administering & managing the Data Management System.
The MCP shall include a master listing of all PMIs with task references for all equipment/system, as well as an annual PMI schedule. PMIs in the annual schedule shall be categorized by site, facility, system, equipment, and frequency of inspection. Submit a PMI schedule to the CO that covers at least 30 calendar days. The MCP shall include procedures for tracking/documenting PMI schedule deviations and procedures for PMI of single point failure equipment.

Include Operational Availability (Ao) metrics by Wing/Squadron and System/Eqpt/Circuit identifier. In addition, report the number of trouble calls received and the amount of hours of telephonic troubleshooting. In the instances where restoral actions require an outside agency, notification time to the outside agency will be considered as the restoral time and used for calculation of Ao. Report equipment system/circuit outages, maintenance and status. As a minimum, provide the following information for all outages, scheduled and unscheduled maintenance: System/Equipment/Circuit ID/nomenclature, outage/maintenance narrative, Discrepancy, Estimated Time of Return to Operation (ETRO), maintenance start/stop date and times, corrective actions, status changes, date/time of outage/status change, Item(s) ordered (include part number), IMDS Job Control Number (JCN), Supply Document number and Supply status. Monthly Reachback Report shall iInclude site-by-site statistics all service summary metrics.

An equipment inventory (by WOC/SOC) shall be embedded in the document and shall report, but not limited to: equipment location, equipment manufacturer, model number, part number, purpose, serial number, and list equipment warranties/expirations. 16_REMARKS_Continued: 5c) SATCOM Relay Sites Guide shall include: system diagrams, circuit routing maps, circuit troubleshooting information, circuit restoral information and priority, circuit loss contingency plan, contact information, encryption settings, and associated C4I systems configurations to include circuit type specific profiles, timing configuration and sources. Include all Satellite terminal BaseBand information: All circuit configurations clearly indicating which values are in use (includes circuit type specific profiles and timing configuration and sources). Identify satellite link controllers, local and distant end user and points of contact for each individual distribution authorized to U.S. Government agencies and their contractors.
