FORM 1 – SAMPLE PROJECT – OFFEROR INFORMATION

SAMPLE PROJECT STATEMENT OF WORK

Provide labor, material, equipment, related services, management, and supervision required to repair and maintain the protective coating for the buildings and facilities listed below, in accordance with IDIQ specifications.

Ancillary items such as cables, conduits, brackets, placards, panels, or other minor equipment that conceal surfaces scheduled for preparation and paint shall be partially or completely removed, as appropriate, and reinstalled after painting. Securely store any removed items, including hardware, and reinstall to fully operational condition in the original location and position prior to completion of the work.

Feather all irregular surfaces and edges to provide a uniform painted surface that is consistent in texture and appearance. After removal of all loose coating, carefully feather the edges of the remaining intact coating to ensure a uniform transition between layers that will be visually indistinguishable after spot repair and over-coating.

Assume no heavy metals are present in the existing coatings.

Facility-specific requirements include the following:

Building X: Prep and paint entire stucco/CMU building using acrylic latex, including entrance on east side. Related requirements include:

· Repair cracks in stucco siding, widespread over entire surface of building.

· Repair/coat exposed rebar and spalling concrete, where encountered, especially over roll-up doors. Some areas will likely involve rebar coating/repair before reforming and installation of concrete.

· Pressure wash and repaint entire building including metal man doors, galvanized roll up doors, and entry enclosure on east side of building.

· Pressure wash only bare (unpainted) wall on northwest corner to remove mildew.

· Remove and replace deteriorated downspout.

· Remove and replace pipe hangers on west side of building and the two insulated pipe hangers on the north face of the southwest corner of the building.

· Remove facility ID signs and reinstall upon completion of paint work.

· Remove misc box and steel plates on north face of building; patch.

· Properly feather all repairs and patch all holes so as to make them visually indistinguishable from the remainder of the substrate Building Y: Pressure wash, prep and repaint entire CMU building and adjacent box structure using acrylic latex. Related requirements include:

· Re-caulk windows.

· Repair spalled concrete with rebar corrosion mitigation.

· Replace man doors with new aluminum doors and frames.

· Repair cracking in concrete where staircase connects to building.

· Add non-skid material to stair steps.

· Concrete block infill area where AC unit formerly resided in wall.

· Remove facility ID signs and reinstall upon completion of paint work.

· Replace and paint wood trim around existing AC unit.

· Repaint galvanized handrails using an epoxy and aliphatic urethane coating.

OFFEROR RESPONSE

i. PROPOSED TASK ORDER: Specify the IDIQ CLINs and coating system required to fulfill the SOW requirements for each building, as described above. Provide a narrative describing the rationale for the selections and any assumptions that were made. For a few SOW requirements, a “Non-Priced Line Item” (an item not specifically identified within the IDIQ CLIN structure) may be required. For these items, the work element should be identified in the table, with the “IDIQ Line Item” specified as “NPI”. Any “NPI” line items should also be described in the narrative

BUILDING X

Work Element from SOW
IDIQ Line Item

(CLIN and Description) Coating System (from Paint Schedule Guide)1

1 – Only required for those work elements/line items that actually involve coating application.

FACILITY X Narrative: Describe the major work elements required to complete the SOW for this building. Identify any assumptions that were made in the process of selecting appropriate IDIQ CLINs and Coating Systems. Discuss the rationale for your selected CLINs and Coating Systems. Specifically highlight any items identified as “NPIs”.

BUILDING Y

Work Element from SOW
IDIQ CLIN

(CLIN No and Description) Coating System (from Paint Schedule Guide)1

1 – Only required for those work elements/line items that actually involve coating application.

BUILDING Y Narrative: Describe how the work in the SOW will be accomplished, addressing the major work elements required to complete the SOW for this building. Identify any assumptions that were made in the process of selecting appropriate IDIQ CLINs and Coating Systems. Discuss the rationale for your selected CLINs and Coating Systems. Specifically highlight any items identified as “NPIs”.

ii. SEQUENCE OF WORK: Prepare a proposed sequence of work for the elements required to complete the Sample Project SOW (above). Each unique work element shall be reflected in the sequence. The elements shall be presented in the order they would need occur for satisfactory completion of the SOW. Both buildings shall be addressed in the sequence. The sequence shall also clearly account for any planning or submittal documentation that is required in order for work to proceed. The tasks necessary to complete both buildings shall be clearly identified in the order they will occur. Any tasks that will overlap should be noted. Provide a narrative describing any assumptions that were made.

Task No.

1 Specify the Building No (X or Y) to which the work element relates. If the item is a general requirement for the task order and relates to both buildings, “X/Y” or “General” may be specified.

2 If tasks will be completed simultaneously, or some overlap will occur, then the Task No for the overlapping elements should be cross-referenced in the column entitled “Overlap”
