About this file
This document is an Itemized Service Budget (ISB) template for the Community Services for Families Program (CSFP) administered by an Arizona state agency, with contract oversight involving ADES (Arizona Department of Economic Security) and CCSD (likely a county or regional service delivery entity). The ISB form is designed for service providers entering into a state contract to detail anticipated costs for delivering CSFP services during the contract period of July 1, 2025 through September 30, 2025, with instructions referencing a full contract year of July 1, 2025 to June 30, 2026.
The budget template categorizes allowable costs across ten budget lines: Personnel (including FTE allocations for direct and administrative functions), Employee Related Expenses (fringe benefits), Professional and Outside Services (subcontractor services), Travel (mileage reimbursement at federal rates), Space/Occupancy (facilities costs), Equipment (purchases or leases of items costing $1,000 or more), Materials and Supplies (consumable items), Operating Services (telephone, training, insurance), Direct Costs, and Indirect Costs (administrative overhead). Offerors are required to identify all revenue sources, distinguish between total service costs and amounts charged specifically to CCSD, ensure costs are necessary and reasonable, and break out expenses by category with all costs rounded to the nearest dollar. The template provided shows zero dollars allocated across all budget categories, indicating it serves as a blank form awaiting completion by service providers during the procurement process.

Instructions
| ISB Instructions |
| An Itemized Service Budget (ISB) for the first contract period anticipated to be July 1, 2025 to June 30, 2026, as follows: |
* Utilize the ISB form to identify Total Service Costs and ADES Costs allocated to the Service Delivery Area. Total Service Cost column is to be utilized to account for all expenses involved in carrying out the service. CCSD Cost column is to be utilized to account for only the expenses that an Offeror will be charging to CCSD to carry out the service.
* Ensure that costs are necessary and reasonable for the delivery of the service(s);
* Place a zero (0) in the column(s) for any budget category where no costs are anticipated;
* Round all costs to the nearest whole dollar;
* Breakout the costs under each budget category;
* Identify all revenue sources (Offeror is to list out all the revenue sources include ADES and the amounts they are receiving from each source to provide this service); and
* Utilize the information in the following table to determine appropriate costs for each budget category.
| BUDGET CATEGORY | ALLOWABLE TYPES OF COSTS |
| 1. Personnel | Applicant’s employees’ salaries that relate directly to the delivery of projects. |
| · FTE Level: Is the percentage of an employee's time that will be allocated to the contracted service. For example if you have an eligibility specialist that works with different programs, you may estimate as an example that the employee spends 30% of their time providing the contracted service, therefore the FTE level would be .3 on the budget. | |
| · *A = % Administrative Functions: the percentage of expenses that are budgeted towards indirectly supporting the delivery of a service. | |
| · D = % Direct Service Functions Expenses that are budgeted towards the actual delivery of a service. | |
| 2. Employee Related Expenses (ERE) | Applicant’s fringe benefits to employees, including but not limited to: Social Security (FICA); unemployment insurance; worker’s compensation; health and life insurance; and retirement (Apply only to that portion of employees’ salaries directly related to the agreement.) |
| 3. Professional & Outside Services | Subcontractor services for direct service provision and/or support services (e.g., interpreter services) that relate directly to the proposed project plans, but are not provided by agency staff. Services may be provided by individuals or organizations. |
| 4. Travel | Mileage reimbursement not to exceed the federal rate as may be amended. |
| 5. Space | Facilities costs such as: rent; depreciation or use allowance; building services and utilities such as: fuel; electricity; water; trash removal; maintenance (subcontracted services only); sewer; insurance for buildings and contents. |
| 6. Equipment | Purchase and/or rental/lease of equipment costing $1,000 or more (each); depreciation; equipment maintenance and repair costs. |
| 7. Materials & Supplies | Consumable supplies such as: general office, kitchen, cleaning, hygiene, and program supplies; postage; reproduction and subcontracted printing services. |
| 8. Operating Services | Telephone; (ADES pre-approved) training for employees; library purchases; general liability insurance. |
| 10. Indirect Costs | Costs incurred for the benefit of multiple programs, functions, or other cost objectives and therefore cannot be identified readily and specifically with a particular program or other cost objective. Indirect costs typically support administrative overhead functions such as fringe benefits, accounting, payroll, purchasing, facilities management, utilities, etc. |
