About this file

This document is an Itemized Service Budget (ISB) template for TEFAP (The Emergency Food Assistance Program) service delivery contracts. The template is designed for use by service providers contracted with state and local agencies to deliver TEFAP services for the contract period of July 1, 2025 through September 30, 2025, with an anticipated full contract period running from July 1, 2025 to June 30, 2026. The document establishes the budgeting framework that contractors must use to identify and allocate all service-related costs and to distinguish between costs charged to ADES (Arizona Department of Economic Security) and those charged to CCSD (likely a specific service delivery area).

The ISB template requires contractors to itemize costs across ten budget categories: Personnel (including FTE levels for direct and administrative functions), Employee Related Expenses, Professional and Outside Services, Travel, Space/Occupancy, Equipment, Materials and Supplies, Operating Services, Direct Costs, and Indirect Costs. Contractors must identify all revenue sources funding the service delivery, ensure costs are necessary and reasonable, round to the nearest whole dollar, and provide detailed breakouts within each category. The document establishes allowable cost types for each budget category, including employee salaries directly related to service delivery, fringe benefits, subcontractor services, mileage reimbursement at federal rates, facility costs, equipment purchases or leases of $1,000 or more, consumable supplies, and administrative overhead expenses. All budget line items in the provided template are currently showing zero allocations, indicating this is a blank form awaiting completion by the service provider.

Instructions

ISB Instructions
An Itemized Service Budget (ISB) for the first contract period anticipated to be July 1, 2025 to June 30, 2026, as follows:

* Utilize the ISB form to identify Total Service Costs and ADES Costs allocated to the Service Delivery Area. Total Service Cost column is to be utilized to account for all expenses involved in carrying out the service. CCSD Cost column is to be utilized to account for only the expenses that an Offeror will be charging to CCSD to carry out the service.

* Ensure that costs are necessary and reasonable for the delivery of the service(s);

* Place a zero (0) in the column(s) for any budget category where no costs are anticipated;

* Round all costs to the nearest whole dollar;

* Breakout the costs under each budget category;

* Identify all revenue sources (Offeror is to list out all the revenue sources include ADES and the amounts they are receiving from each source to provide this service); and

* Utilize the information in the following table to determine appropriate costs for each budget category.

BUDGET CATEGORYALLOWABLE TYPES OF COSTS

· FTE Level: Is the percentage of an employee's time that will be allocated to the contracted service. For example if you have an eligibility specialist that works with different programs, you may estimate as an example that the employee spends 30% of their time providing the contracted service, therefore the FTE level would be .3 on the budget.

2. 6. EquipmentPurchase and/or rental/lease of equipment costing $1,000 or more (each); depreciation; equipment maintenance and repair costs.
7. Materials & SuppliesConsumable supplies such as: general office, kitchen, cleaning, hygiene, and program supplies; postage; reproduction and subcontracted printing services.
