About this file
This is an Exhibit 14 General Certifications form for the Los Angeles County Metropolitan Transportation Authority (LACMTA), required for bidders and proposers on state and local contract opportunities. The document serves as a certification document rather than a project specification or scope of work. Bidders and first-tier subcontractors proposing over $100,000 must respond "Yes" or "No" to four certification requirements and provide full explanations for any "No" responses. The form does not specify project scope, quantities, response dates, or contract terms, as it functions solely as a compliance and certification instrument that must accompany bid submissions.
The four mandatory certifications address nondiscrimination in employment practices in compliance with California's Fair Employment and Housing Act (Gov. Code §12900), Iran investment activities restrictions for contracts valued at $1,000,000 or more under Public Contract Code §2202.5 and §2203, prohibition on fraudulent use of Disadvantaged Business Enterprise (DBE) fronts as defined in LACMTA's Contract Compliance Manual, and compliance with California Labor Code §1102.5 whistleblower requirements. Bidders must execute the form under penalty of perjury, certifying the accuracy of their responses. The document includes contact information for reporting fraud, waste, abuse, or mismanagement to LACMTA's Small Business Diversity and Labor Compliance Office, the LACMTA Inspector General Office, and federal oversight agencies. No pricing, set-asides, or funding information is included in this certification document.
