About this file
This is a Response Certification Form (Exhibit A) for the Department of Information Resources Request for Offer COOP2026SOL-00008 for Cabling Services and Related Products in Texas. The form serves as the mandatory certification document that respondents must complete, sign, and submit through the Vendor Portal as part of their bid response. The document requires respondents to provide comprehensive company information including legal entity name, DUNS number, Federal Employer Identification Number, principal place of business address, and facility address responsible for fulfilling services. Respondents must also designate contact persons for RFO inquiries, negotiations, and contractual binding authority. The form requires submission of all mandatory response exhibits including the Response Certification Form (signed), Company Ability and Capacity documentation, Contract Marketing and Support Plan, Itemized Pricing, EDGAR Certification Form, PDAA Form, Texas Government Code 2161 Form (signed), Exceptions Table, and any signed addenda or letters of authorization.
The certification requires respondents to disclose conflicts of interest for current and former state employees, verify financial stability with a Duns and Bradstreet SER Score of 8 or less, and maintain current tax payment status with the Texas Comptroller of Public Accounts. Respondents must certify compliance with numerous state and federal requirements including non-debarment status, anti-terrorism restrictions, antitrust law compliance, computer equipment recycling programs, Texas residency bidder status where applicable, and compliance with prohibitions on boycotting Israel and involvement with foreign adversaries. The form explicitly prohibits late submissions and warns that failure to sign and return the certification document will result in disqualification of the response.
