About this file

This is a Drug-Free Workplace Report Coversheet form issued by the State of West Virginia Purchasing Division in compliance with West Virginia Code 21-1D-7b. The form serves as a template for contractors to submit certified drug-free workplace reports to the appropriate public authority that issued their contract. Contractors are required to submit this report no less than once per year or upon project completion. The coversheet requires vendors to document their organization's drug-free workplace compliance and attach supporting documentation detailing education and training services provided to meet state requirements.

The required report content must include the name of the United States Department of Health and Human Services-certified laboratory performing drug tests, the average number of employees engaged in connection with the construction on public improvements, and comprehensive drug test results organized by category including pre-employment and new hires, reasonable suspicion, post-accident, and random testing with both positive and negative test counts for each category. Submission requirements vary based on contract value: contracts exceeding $25,000 must be mailed to the West Virginia Purchasing Division at 2019 Washington Street East, Charleston, WV 25305, while contracts of $25,000 or less should be submitted directly to the public authority issuing the contract.
