About this file

This is a Disqualification Disclosure form required by the City of Pasadena, California, in accordance with Public Contract Code Section 10162, for prospective bidders on the On-Call Structural Engineering Services opportunity. The form serves as a qualification questionnaire that prospective bidders, including their officers and employees with proprietary interest, must complete under penalty of perjury. The disclosure specifically addresses whether the bidder or any associated personnel has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations.

Bidders are required to indicate whether they have such disqualifications by checking either "Yes" or "No" on the form. Should a bidder answer affirmatively, they must provide a detailed explanation of the circumstances and attach supporting documentation to the disclosure. This certification is a mandatory component of the proposal submission process for the City of Pasadena's on-call structural engineering services contract, which offers potential five-year agreements with a compensation cap of $1,000,000 per firm and requires minimum five years of comparable experience in structural engineering services.
