Task Assignment Notification Form - GC896-04

This is a Task Assignment Notification Form (Attachment C) for DEP Contract No. GC896, issued by the State of Florida's Department of Environmental Protection (DEP) to the Board of County Commissioners of Palm Beach County. The Local Program Manager is Deborah Drum, and the DEP Contract Manager is David G. Warfel. The task assignment covers the period of July 1, 2019 through June 30, 2020, with a fixed cost amount not to exceed $497,333.00. The Local Program performs all work as specified in Attachment A of Contract GC896, managing petroleum cleanup sites for the Board of County Commissioners.

The compensation structure provides an annual fixed cost base rate of $468,200.00 for petroleum cleanup site management, with $448,200.00 allocated for petroleum cleanup site management of 137 non-Low Score Assessment (LSA) source properties and $20,000.00 for five (5) LSA source properties. Compensation varies by site phase: $4,000.00 annually for planning phase sites (site assessment, interim remedial action plan), $8,000.00 annually for remediation phase sites for the first two years, then $4,000.00 thereafter, and $2,400.00 annually for monitoring phase sites. Performance incentive payments of $750.00 are available for specific completion milestones, capped at 10% of annual non-LSA compensation. Monthly payments range from 95% to 109% of the monthly compensation amount, with retainage of 5% of non-LSA source property compensation withheld monthly. Up to 80% of withheld retainage is released monthly based on achievement of four performance measures: deliverable turnaround times, change request turnaround times, timely data entry in the Storage Tank Contamination Monitoring (STCM) database, and timely document insertion into OCULUS. A final 20% retainage is released upon verification that 98% of assigned source properties were visited or inspected during the fiscal year. The task assignment includes an attached list of 137 source properties requiring inspection during FY 19/20.
