About this file
Attachment K is a Foreign Country of Concern Attestation form issued by the Florida Department of Management Services for the Data Management Tools RFP (No. 25-43232300-RFP). The form is a mandatory certification that must be completed by an officer or representative of an entity submitting a bid, proposal, or entering into a contract that would provide access to Personal Identifying Information. The attestation requires vendors to confirm they are not owned by, organized under the laws of, or have a principal place of business in a Foreign Country of Concern, and that no such government has a controlling interest in the entity.
The form is part of a comprehensive procurement process for a State Term Contract for Data Management Tools, which aims to establish multiple vendor awards across four service categories: Data Management, Data Governance, Data Analytics, and Data Interoperability and Integration. Vendors must declare under penalties of perjury that the statements made are true, and the form requires a printed name, title, signature, and date. This attestation is designed to ensure compliance with state regulations and protect sensitive government information by screening out potential vendors with connections to countries deemed to be of concern.
