Attachment C Proposal Submission Instructions and Evaluation Criteria

Data Management Tools

RFP No. 25-43232300-RFP

Document Submission: If a Solution meets the requirements of multiple Service Categories, the applicable Attachment L, Service Category Technical Response(s), and Attachment B, Cost Proposal, for each applicable Service Category shall be submitted as follows:

Attachment L

Combine all Attachment L, Service Category Technical Response(s), documents into a single .zip folder for submission. The .zip folder should follow the naming convention below:

• AttachmentL_RespondentName

Within the .zip folder, Service Category Attachment L file(s) for each solution should follow the naming convention below:

• AttachmentL_RespondentName_ServiceCategory#_SolutionName

Attachment B

Combine all Attachment B, Cost Proposal, documents into a single .zip folder for submission.

The .zip folder should follow the naming convention below:

• AttachmentB_RespondentName

Within the .zip folder, Service Category Attachment B file(s) for each solution should follow the naming convention below:

• AttachmentB_RespondentName_ServiceCategory#_SolutionName

Cost Proposal Scoring Methodology: For each Solution proposed for a Service Category, the Respondent may be awarded up to 0.75 points for its Cost Proposal. Pricing for Add-On Services will not be evaluated. Using the scale below, the Procurement Officer will determine if the Cost Proposal receives a score of 0.25, 0.50, 0.75 or zero (0) points.

SCORING Assessment Assessment Criteria 0 Fail Respondent failed to submit a Cost Proposal that complies with the instructions in Attachment B, Cost Proposal.

+0.25 Pass Respondent submitted a Cost Proposal that complies with the mandatory requirements in Attachment B, Cost Proposal.

+0.25 Tiered Pricing Respondent’s submitted Cost Proposal provides tiered pricing for the proposed Solution.

+0.25 Not-to-

Exceed Pricing

Respondent’s submitted Cost Proposal provides not-to-exceed pricing for the proposed Solution.

Experience Response Scoring Methodology: The Experience Response will be independently reviewed and scored. Using the scale below, each Evaluator will determine if the experience scores one quarter point (0.25) or zero (0) points.

0 Fail Respondent failed to submit an Experience Response.

0.25 Pass Respondent submitted an Experience Response.

All Evaluator scores for a Respondent’s Experience Response will be totaled and averaged by the Procurement Officer to create a Respondent’s total experience response score. The points assessed for this total experience response will be part of the calculation for a Respondent’s total final score for a proposed Solution.

Technical Response Scoring Methodology: Attachments L1-L4 contain varying number of prompts requesting descriptions of a Respondent’s proposed Solution. A technical response for a proposed Solution should provide a response to all applicable prompts. Each prompt that has a response will be independently reviewed and scored by each Evaluator using the following scoring methodology:

0 Fail The Solution proposed fails to satisfy the requirements of the Prompt.

1 Poor The Solution proposed poorly addresses or demonstrates a very limited ability to perform the requirements of the Prompt.

2 Unsatisfactory The Solution proposed unsatisfactorily addresses and demonstrates the limited ability to perform the requirements of the Prompt.

3 Satisfactory The Solution proposed satisfactorily addresses and demonstrates the ability to perform the requirements of the Prompt.

4 Good The Solution proposed extensively addresses and demonstrates the ability in performing the requirements of the Prompt. The response would provide enhanced value to the State.

5 Superior The Solution proposed exhaustively addresses and demonstrates the ability to exceed performing the requirements of the Prompt and provides additional benefits above the requirements and enhanced value to the State.

The Evaluator’s Technical Response Score for each proposed Solution(s) will be the average of the scored prompts by the Evaluator. Unfilled prompts will not be included in the average calculation. The averages of the Evaluators’ Technical Response Scores will be averaged together by the Procurement Officer for the Solution’s final Technical Response Score.

The Procurement Officer will calculate the Solution’s overall score in accordance with section 2.8 and awards will be made in accordance with section 2.9 of the RFP.
