About this file
This is a Responsibility Inquiry form (Attachment G) from the State of Oregon, Department of Administrative Services, for OregonBuys Bid S-25800-00014144 concerning the procurement of two 10-passenger crew carriers for the SPIRE program. The solicitation was posted on July 28, 2025, with proposals due by August 15, 2025, providing bidders approximately 18 days for submission. The form serves as a pre-award evaluation tool to assess proposer financial capability, past performance, and organizational suitability prior to contract execution. Key evaluation areas include proposer access to appropriate financial, material, equipment, facility, and personnel resources; contract performance history over the past five years regarding budget and schedule adherence; any criminal or civil charges within the past three years related to public contracts, antitrust violations, or financial crimes; contract termination history for default or litigation involving creditors or contract disputes; outstanding judgments; financial distress or liquidity concerns; bankruptcy filings within the past three years; and current licensure, insurance, and authorization to conduct business in Oregon.
The Responsibility Inquiry requires proposers to certify their compliance through affirmative responses to a series of yes-or-no questions with supporting documentation. Proposers employing 50 or more full-time workers on contracts exceeding $500,000 must provide a current, unexpired Pay Equity Certificate from the Department of Administrative Services, unless the certificate was previously submitted with the bid or proposal. Authorized representatives must sign and certify that all responses are complete, accurate, and not misleading. The Agency reserves the right to request additional documentation including profit-and-loss statements, balance sheets, cash flow information, assets-to-liabilities ratios, financing availability, bonding capacity, insurability reports, credit information, personnel qualifications, and prior contract performance records. Failure to promptly provide requested information or failure to demonstrate responsibility may result in a finding of non-responsibility and proposal rejection.
