About this file
This is a W-9 form for domestic vendors submitted to the Auraria Higher Education Center (AHEC) in Colorado for the on-call service providers/small construction contract opportunity AHEC-27701-R. The form serves as a taxpayer identification and certification request document that vendors must complete to establish their eligibility to do business with AHEC. The document requires vendors to provide their legal business name, federal tax classification, taxpayer identification number (either Social Security Number or Employer Identification Number), business address, and account information. Additionally, vendors must certify their U.S. citizenship status, provide accurate tax information, and confirm their status regarding IRS backup withholding requirements and FATCA reporting exemptions.
The W-9 form includes a voluntary self-certification section designed to help AHEC understand and measure its diverse vendor relationships across multiple demographic categories and business characteristics. Vendors are invited to identify ownership categories such as woman-owned, minority-owned, veteran-owned, disabled-owned, LGBTQ-owned, and small business designations. The form also requests information about business size (measured by full-time employees), certifications (SBA 8(a), HUBZone, WOSB, VOSB, SDVOSB, MBE, WBE), and business practices including health insurance offerings, paid time off, environmental sustainability, hiring of individuals with barriers to employment, living wage practices, unionization status, and social enterprise operations. This voluntary certification allows AHEC to track participation levels by women, minorities, small businesses, local Colorado-based businesses, and other impacted groups within its vendor network.
