About this file
This is a Certification of American-Manufactured Goods form required by the Maryland Department of General Services (DGS) for solicitation KN-000-220-002A, a Rooftop Unit Replacement (RTU) project at Western Correctional Institution. The form functions as a mandatory compliance document that requires bidders/offerors to certify the manufacturing origin of goods offered under the solicitation or contract. Bidders must select one of two options: certify that all offered goods are American-manufactured as defined in COMAR 21.11.15.04B(1), or certify that goods are not American-manufactured and identify the applicable exceptions.
For bidders selecting Option 2 (non-American-manufactured goods), the form requires identification of goods by category using two supplementary forms. Form A requires listing of goods that are non-American-manufactured but qualify as emergency life safety and property safety goods under COMAR 21.11.15.04B(2). Form B requires listing of goods that are non-American-manufactured and do not qualify as emergency life safety or property safety goods. The certification must be executed by an authorized representative of the bidder/offeror and includes spaces for signature, printed name, title/capacity, and date. Additional sheets may be attached if needed to accommodate extensive goods listings.
