About this file
This document is a process flowchart depicting the Dig Permit Process for excavation work. The process begins when a completed JMRHH-1254 form is submitted via email or in person to the Excavation Permit Coordinator (EPC). The EPC reviews the form for proper information, documentation, and schematics, determining whether it was filed out properly.
If the form is properly completed, the EPC assesses whether the work constitutes an emergency. For non-emergency requests, the EPC communicates receipt within ten business days and schedules execution with the requestor. For emergency requests, the EPC immediately contacts the O&M Chief or Director for coordination and requires O&M Leadership to contact Miss Utilities and the National Electrical Code (NEC) immediately. The EPC then emails the 1254 documentation to the Shop Foreman and NEC for execution, with the Army Material Command (ArMA) EPC creating a case and image. The Government Fleet Evaluation Board (GFEBS) IW28 EPC converts the notification to a Work Order and assigns it to the appropriate Work Center. Simultaneously, the EPC cross-references the permit under the relevant Service Order/Work Order/Project field, adds information to the Dig Permit Tracker on SharePoint, and reviews daily for status updates. Once all trades are complete, the EPC follows up with NEC and O&M personnel, inputs the permit expiration date (30 days from issuance), emails the requestor an approved Excavation Permit, and updates the Dig Permit Tracker. The requestor is responsible for upkeep of markings. Extension requests follow a separate process where the requestor submits an extension request, the EPC submits a memorandum to the Branch Chief for review and approval, and the Branch Chief approves and returns approval via email, with total extensions not exceeding 180 days.
