ATTACHMENT 9
SITE VISIT
October 13, 2021 from 10:00 AM to 12:00 PM CT
a. If the vendor has not visited the site, it is strongly urged to do so. However, visitation is neither required nor a prerequisite for quote submission, and will not be considered a factor in the evaluation. However, it is considered impracticable to determine, without inspection, the exact nature of the work and the condition of the site under which the work is to be performed.
The site visit October 13, 2021 from 10:00 AM to 12:00 PM CT will provide vendors the opportunity to inspect the government furnished property; equipment, facility, and site which services are to be performed; and the general and local conditions that may affect the costs and performance of the prospective order, to the extent that the information is reasonably obtainable.
b. In no event will failure of the vendor to inspect the Government furnished property;
equipment, facility, and site which services are to be performed; and the general and local conditions of the site relieve the vendor from assuming all responsibility for properly estimating the price, difficulties, and the conditions, including safety precautions, that may be encountered during performance, nor will failure to inspect the building and site constitute grounds for a claim after order issuance.
c. Questions will not be answered at any time during the site visit. Vendors must submit their questions in writing to Kevin Schumacher, at Kevin.schumacher@nara.gov, no later than 12:01 PM CT on October 18, 2021. Subsequent to the site visit, if required, an amendment containing an abstract of the questions and answers will be issued.
d. Vendors are cautioned that, notwithstanding any remarks or clarifications given at the site visit, all terms and conditions of the solicitation remain unchanged unless they are changed by a solicitation amendment. If the answers to site visit questions or any solicitation amendment create ambiguities, it is the responsibility of the vendor to seek clarification prior to submitting a quotation.
e. In order to access the facility, vendors should be prepared to show a Government-issued photo identification card, and if driving, proof of automobile insurance and registration.
f. COVID-19 Protocols. Vendors are limited to one (1) representative and shall wear nose and mouth coverings during the entire visit. Vendor’s representative shall bring and provide to the NARA representative for the visitor a copy of the completed assessment attached.
g. Vendors are to send email to the contracting officer at kevin.schumacher@nara.gov and the COR Kevin McMurry kevin.mcmurry@nara.gov on Thursday October 7, 2021 with name of representative and to confirm that site visit has not been cancelled due to COVID.

Health Screening Self-Assessment
1. Check your temperature. All NARA employees and contractors are required to measure their temperature on days they are scheduled to report to a NARA facility. Please measure your temperature at home, before reporting for duty.
2. Complete the health screening inventory. After checking your temperature and before reporting to the workplace, answer each of the “yes” or “no” questions provided in the Health Screening Inventory below.
Health Screening Inventory
Please answer the following questions to the best of your ability:
1. Are you currently experiencing any symptoms of COVID-19? Do not include symptoms that are explained by something other than respiratory illness (e.g. allergies, asthma, an illness that has been diagnosed as something other than COVID-19, routine aches, pain, or headaches that you normally experience). The following is a list of COVID-19 symptoms recognized by the CDC as of the date of this fact sheet. Check https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/symptoms.html.

If you respond “yes” to any question on the health screening inventory:
1. Do not come to work. Contact your supervisor and tell them that you answered “yes” to at least one question. Your supervisor will contact the Contracting Officer and the Contracting Officer’s Representative (COR).
2. If you’ve been in the facility in the past two weeks, be prepared to provide your COR with information necessary for them to conduct contact tracing. Your COR will ask you:
a. Were you in the facility in the last two days before you first felt ill or were diagnosed, or in the 14 days after you came in close contact with someone diagnosed with
COVID-19?
b. Where did you spend the majority of your time in the facility?
c. Was there anyone else in the facility who you were within 6 feet of for longer than 15 minutes?
3. You will be asked to stay home for the duration of your symptoms, and no less than 14 calendar days.
4. Seek medical attention.
If you respond “no” to all questions on the health screening inventory, you should report to work as scheduled.

Message from the Chief Acquisition Officer to NARA Contractors on Updated
COVID-19 Health and Safety Protocols (Vaccination Attestation and COVID-19 Testing)
As the Chief Acquisition Officer, I am reaching out to all contractors who have employees working in NARA facilities. I want to make sure you are aware of NARA’s updated policies written to protect the health and safety of staff and contractors in the NARA workplace and to prepare for future reopening of NARA facilities.
Consistent with recent guidance from President Biden’s Administration, in coordination with the
Centers for Disease Control and Prevention (CDC) and the Safer Federal Workforce Task Force, the National Archives and Records Administration (NARA) is implementing new policies and protocols for the federal workplace with respect to mask wearing, social distancing, vaccination attestation, and COVID-19 testing for screening purposes only. As such, NARA must ascertain the vaccination status of every individual who enters a NARA facility.
Once the new protocols become effective, individuals may not be allowed entry into NARA facilities unless they are in compliance with these protocols. This communication outlines vaccine attestation and COVID-19 testing requirements for onsite contractors. As described in more detail below, all NARA on-site contractors will be asked to attest to their vaccination status.
Vaccination Attestation
Beginning September 20, 2021, contractors entering a NARA facility will be required to attest to their vaccination status and individuals who are not fully vaccinated or decline to disclose their vaccination status must provide a negative COVID-19 test result prior to entering a NARA facility. NARA has been directed to provide onsite contractors with the U.S. Office of
Management and Budget's (OMB) Certification of Vaccination form (attached), which must be completed as a requirement to enter the NARA worksite. NARA will not collect and maintain
Certification of Vaccination forms completed by contractor personnel. Instead, contractor personnel should keep a copy of the completed form with them while at the worksite and should be prepared to show the completed form to the security officers when entering the facility and at the request of the Contracting Officer’s Representative (COR) or federal designee. Security officers will only verify that the contractor has completed a Certification of Vaccination form and has the required paperwork for entry into the facility.
If your employee fails to present a signed Certification of Vaccination form and, if required, show proof of a negative COVID-19 test result, they will be denied entry to the NARA worksite.
Once completed, the Certification of Vaccination form will serve as your employees’ vaccination passport as it allows them to certify whether they (1) are fully vaccinated, (2) are not yet fully vaccination, (3) have not been vaccinated, or (4) decline to respond. Individuals who are fully vaccinated must comply with applicable agency guidance when they are in any NARA facility, including a requirement to wear a mask in areas of high or substantial COVID-19 transmission.
Any individual who provides false information on the certification form may be subject to adverse action, including removal from the contract.
COVID-19 Testing
Those individuals who are not fully vaccinated or decline to respond, will be required to comply with the CDC and NARA guidance for unvaccinated individuals. This will include mandatory mask wearing, physical distancing, submitting to COVID-19 testing and demonstrating negative test results prior to entering a NARA facility. NARA requires that individuals who require
COVID-19 testing provide a test result from a valid, FDA-approved viral test (no antibody tests), from a laboratory, clinic, or similar health care facility (no at-home tests), that is no more than three days old. Three days old means that the specimen collection date was no more than three whole days (which may be greater than 72 hours) before the date of entry to the NARA facility.
Contractors will be required to update their Certification of Vaccination form when there is a change to their vaccination status. Contractors who are not fully vaccinated or who do not report their vaccination status will be barred from NARA facilities if they do not provide a negative
COVID-19 test result from a valid, FDA approved test or if they fail to follow NARA procedures for face coverings and physical distancing .NARA will not provide testing or reimburse contractor employees for COVID-19 tests, nor will NARA provide duty time to contractor employees for COVID-19 tests.
The Centers for Disease Control and Prevention (CDC) COVID-19 website remains the authoritative source for facts about how to protect yourself and your family from COVID-19, and how to recognize symptoms of the virus. The site is frequently updated with virus guidance and provides updated travel advisories. It is wise to review the CDC website regularly for updated guidance at https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/symptoms.html.

Certification of Vaccination The purpose of this form is to take steps to prevent the spread of COVID-19, to protect the health and safety of all Federal employees, onsite contractors, visitors to Federal buildings or Federally controlled indoor workspaces, and other individuals interacting with the Federal workforce. If you fail to submit this signed attestation or any required negative COVID-19 test, you may be denied entry to a Federal facility.

1 The Centers for Disease Control and Prevention considers an individual fully vaccinated if they are:
• 2 weeks after their second dose in a 2-dose series, such as the Pfizer or Moderna vaccines, or
