About this file
This is a Business Proposal Attachment E for a Request for Proposal (RFP) issued by the State of Indiana, Department of Administration (IDOA). The document serves as the template for respondents to submit comprehensive business and organizational information as part of their proposal evaluation. While the attachment does not specify particular products or services, deadlines, or award dates, it establishes the framework requiring respondents to provide company introductions, organizational structure documentation, financial statements demonstrating stability, and evidence of business registration with the Indiana Secretary of State. Respondents must also furnish three client references through Attachment H, which references must be submitted directly to idoareferences@idoa.in.gov on the proposal submission due date specified in Section 1.24 of the RFP.
The Business Proposal attachment mandates submission of corporate financial information from the two most recently completed fiscal years, either through Dunn & Bradstreet reports or audited financial statements, along with a statement of integrity signed by the CEO and/or CFO certifying the accuracy of all financial information. Respondents must address their experience serving state governments and similar-sized clients, provide diversity subcontractor engagement documentation for Minority & Women's Business Enterprises (MBE/WBE) and Indiana Veteran Owned Small Business (IVOSB) partnerships, and indicate willingness to extend pricing to other governmental bodies. If the proposed system involves cloud-based infrastructure, respondents must review and accept or redline the Additional Terms and Conditions provided in Attachments B1, B2, and B3 for IaaS, PaaS, and SaaS solutions respectively, with the State reserving the right to reject requested deviations from the standard terms.
