Substation Maintenance
TECHNICAL EXPERIENCE QUESTIONNAIRE
Source Selection Sensitive (when filled-out) IAW FAR 2.101 and FAR 3.104
Instructions: Fill-in offeror name in footer. In the first table below provide a detailed description of experience/work performed for specific tasks listed in the table. Use the reference table to provide a minimum of one (1) and up to five (5) references to support the experience claimed in the first table. Limited to a thirty-five (35) page submission. Offeror must also complete teaming arrangement table.
Factor 2 Subfactor 2: Technical Experience. Minimum of five (5) years of experience within the (7) year period immediately preceding release of this Request for Quote (RFQ), providing Substation Maintenance Services as more specifically described below. The experience called out in Subfactor 2 is for work which is the same or substantially similar to the work activities and tasks described in the table below. Provide reference and contact information to support the experience claimed by offeror. If offeror intends or proposes to use an entity other than offeror to fulfill a requirement or a portion thereof (e.g. a sub-contractor or other teaming partner), please provide ALL the requested information for that entity as well.
Eligibility for award. To be eligible for award, offerors (or the entity other than offeror which offeror intends or proposes to use to fulfill a requirement or portion thereof) must have at least five (5) years of experience providing Substation Maintenance within the seven (7) year period immediately preceding release of this Request for Quote (RFQ). The required experience is for work which is the same or substantially similar to the work activities and tasks described in the Performance Work Statement.
The offeror's responses and references provided must support the experience claimed by the offeror. Based on the offeror's completed technical questionnaire, offerors shall be given an experience rating based on the following:

Substation Maintenance Experience. At least five (5) years of experience providing substation maintenance services within the seven (7) year period immediately preceding release of this RFQ. Use the narrative block to provide a detailed description of EACH task performed related to providing Substation Maintenance Services identified in the table immediately below (Reference PWS).

Sub-Contracting Arrangements (e.g. Teaming Arrangements).
If your company is not performing ALL the work, you must fill-in the contemplated sub-contracting arrangement in Table 1 below for percentage of work performed by sub-contractor(s) and then fill in the percentage of work in Table 2 that will be performed by your company as the Prime Contractor. Sub-contractor arrangements include, but are not limited to, the following types of relationships: prime/sub-contractor, mentor/protégé, joint venture, etc. If a sub-contractor arrangement is not applicable and ALL the work will be performed by your company as the Prime Contractor, please indicate 0% in Table 1 and complete all fields in Table 2 accordingly.
