About this file
This is a Contractor Affidavit form required by the School District of Manatee County (SDMC), Florida, pursuant to Section 787.06(13) of the Florida Statutes. The affidavit must be completed and signed by an authorized representative of any nongovernmental vendor executing, renewing, or extending a contract with the school district. The form requires contractors to certify under penalty of perjury that they do not engage in coercive labor practices, including the use or threat of physical force, unlawful restraint or confinement, exploitative debt arrangements, confiscation of identification documents, threats of financial harm, fraudulent enticement, or provision of controlled substances for exploitation purposes. This affidavit submission is a mandatory compliance requirement for all vendors and must be completed prior to contract execution.
The affidavit serves as a compliance certification document with no pricing, set-asides, or funding implications. It is a standard form that applies universally to all contractor submissions for the SDMC, including the ongoing Tillman Elementary Campus-Wide Renovation project (RFQ 2025-09). The document requires basic contractor identification information, authorized representative details, and a signed declaration attesting to compliance with Florida's anti-coercion labor statutes. No renewal options, incumbency considerations, or contract-specific terms are addressed within this affidavit form.
