About this file

This is an Affidavit of Eligibility Form for the Opportunity Zone Program administered by the Central Florida Tourism Oversight District in Florida. The form certifies that a business's main office is located within designated Qualified Opportunity Zones in Orange, Osceola, Lake, Polk, or Seminole counties. The Opportunity Zone Program is a federal initiative created by the Federal Tax Cuts and Jobs Act of 2017 designed to foster economic development and job creation in economically distressed communities by encouraging businesses, developers, and financial institutions to invest long-term capital in low-income census tract areas. Eligible businesses must complete this affidavit when responding to solicitations if they wish to participate in the program and qualify for any associated preferences or set-asides.

The form requires businesses to provide their legal name, main office address, and the applicable Opportunity Zone county from the designated counties. Submission of this affidavit is required for businesses seeking to claim Opportunity Zone status in response to solicitations, such as the Landscape Maintenance Services contract opportunity referenced. Applicants must verify that their main office location corresponds to the Opportunity Zones maps maintained by FloridaJobs.org, and the form serves as certification of eligibility for program participation and potential contracting preferences.
