About this file

This document is a Customer Feedback Survey for Design Professionals issued by the State of Utah Division of Purchasing to evaluate contractors applying for the Utah Division of Purchasing Pre-qualification list for Design Professionals. The survey is distributed to clients who have previously engaged with contractors to gather feedback on completed projects and vendor performance. Respondents are asked to confirm the contractor's name, provide the approximate project completion date, describe the services rendered, and rate their overall satisfaction on a five-point scale ranging from extremely dissatisfied to extremely satisfied with above and beyond service. The evaluation process includes questions regarding contractor recommendation to others, consistency of work quality relative to project pricing, and optional space for additional feedback relevant to the vendor evaluation process. All responses are kept confidential and are reviewed only by the evaluator, with results used solely for assessing vendor applicants for the pre-qualification list.

The survey is designed to ensure the highest level of quality and integrity among vendors seeking approval on the pre-qualified vendor list for Design Professional Services, which has a 12-month approval period with requalification required thereafter. The feedback mechanism serves as a quality assurance tool for the State of Utah's procurement process, allowing the Division of Purchasing to make informed decisions about vendor eligibility. Respondent information, including names, is optional and not shared with the public, the contract, or other entities, maintaining confidentiality throughout the evaluation process.
