About this file

This is a cost proposal attachment for Solicitation MH26-11, issued by the State of Utah Division of Purchasing, establishing multiple contracts for tow and wrecker services for state-owned vehicles. The RFP seeks vendors to provide light/medium duty tows (up to 26,000 lbs) and heavy duty tows (over 26,000 lbs) across all 29 Utah counties. Vendors are not required to service all counties but must commit to serving entire counties at the quoted rates if selected. The contract term is five years, with vendors required to guarantee pricing for a minimum of 24 months. Multiple awards may be granted based on technical score thresholds and total score thresholds established by the state.

The cost proposal consists of two independent categories, each worth a maximum of 200 points. For light/medium duty tows, vendors must quote a hook-up fee (not to exceed $120), mileage rate per loaded mile, accident call fee (not to exceed $120), wait time fee (not to exceed $30 per tow or $10 per 15 minutes), and clean-up fee (not to exceed $40 per tow or $10 per 15 minutes). For heavy duty tows, the maximums are higher: hook-up fee of $250, accident call fee of $250, wait time fee of $60 per tow or $20 per 15 minutes, and clean-up fee of $60 per tow or $20 per 15 minutes. Vendors must complete all required yellow fields for counties they seek to serve; incomplete fields are considered as fees included in the base pricing and cannot be added later. Cost points are awarded using a formula comparing each vendor's average county price to the lowest proposed average county price. An administrative fee of 0.25% applies to the contract. Optional value-added services including special tows, roadside assistance, jump starts, fuel assistance, and dispatch fees may be offered with defined maximum rates where applicable. No fuel surcharges should be included in the base pricing, as the Scope of Work permits up to 15% fuel surcharge when fuel exceeds $4.00 per gallon.

Instructions

ATTACHMENT C (Cost Proposal) - for Solicitation #MH26-11
PURPOSE - This Attachment is used to evaluate and assign cost points to a vendor's proposal; however, not all requested information in this attachment will be used to assign cost points. All pricing information provided by the vendor will become contractual if the vendor is awarded a contract. Awarded vendors will be required to provide a full catalog or price list, which will be included in the contract. Any % discounts will be for the life of the contract and pricing will be for the duration of the price guarantee period as stated in the RFP.
COST POINTS - Cost points available: 200 (per category). Each category is independent and scored separately. The vendor with the lowest average cost for the counties serviced in each category will receive the maximum cost points for that category. All other vendors will receive a portion of the total cost points available based on a percentage of the lowest offered price. The formula to compute the points is: Cost Points x (Lowest Proposed Average County Price/Proposed Average County Price). Cost points will be added to a Vendor's Technical Score to determine a total RFP score.
VENDOR INSTRUCTIONS:

1- Vendors should view this file at 100% to ensure information is not unintentionally cut off.

2- There are two tabs other than this tab, each tab is for a separate category (1- Light/Medium Duty Tows & 2-Heavy Duty Tows). The cost table for each category has a row for each county in Utah. Vendors are not required to service all counties. If the vendor cannot service an entire county, they should leave that county-row blank. Vendors are required to service the entire county at the costs provided in this file.

3- Vendors must complete all the yellow fields in any category AND county they are seeking an award for. YELLOW = Required; GREEN = Optional (If a Vendor cannot service a particular county, that row of yellow cells may be left blank) 4- If a fee is included, the Vendor should state "0" or type "included". Any yellow cells left blank will be considered "included" and cannot be added into the costs at a later time.

5- Do not alter this file's formatting unless instructed otherwise. Fields that are not yellow may contain formatting or formulas that make this attachment work properly. If this file does not function as designed due to the Vendor's attempt at formatting may result in their proposal being rejected.

6- Each table/tab may have specific instructions, vendors must follow instructions on each table/tab in order for their proposal to be evaluated.

REMINDER: When filling out your costs, do not add a fuel surcharge. The Scope of Work will allow a fuel surcharge of up to 15% when fuel is greater than $4.00 per gallon. Since the Scope of Work defines this fee, Vendors should not add it on top of their towing fees.

QUESTIONS - Any questions regarding the completion of this attachment must be submitted on U3P, during the public Q&A period
