About this file

This document is an NYS Required Certifications attachment for a state contract bid, containing multiple mandatory certifications required by New York State. The certifications cover various legal and ethical requirements, including nondiscrimination in employment in Northern Ireland (MacBride Fair Employment Principles), non-collusive bidding, diesel emission reduction, prohibition on certain telecommunications equipment, and compliance with Executive Order No. 177 regarding workplace harassment and discrimination.

The document does not specify specific goods or services under contract, but rather serves as a standard compliance form that bidders must complete to be eligible for state contracts. The certifications require bidders to affirm they have no institutional policies that fail to address discrimination, have implemented sexual harassment prevention policies, comply with diesel emission reduction regulations, and will not use prohibited telecommunications equipment. Bidders must sign and attest to the accuracy of these statements under penalty of perjury, with provisions for explaining any inability to fully certify compliance.
