About this file
This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form for the City of North Carolina's On-Call Sustainability Services contract opportunity. The certification requires bidders, contractors, and subcontractors to verify that neither they nor their principals—including officers, directors, managers, and persons holding greater than ten percent equity interest—are currently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from covered transactions by any federal or state department or agency. Bidders must also confirm that within the three-year period preceding the proposal, they have not been convicted of or had civil judgments rendered against them for fraud, criminal offenses in connection with public transactions, violations of federal or state anti-trust or procurement statutes, embezzlement, theft, forgery, bribery, falsification of records, false statements, or receipt of stolen property.
The certification further requires bidders to attest that they are not presently indicted for or otherwise criminally or civilly charged by any government entity with commission of the offenses enumerated in the preceding section, and that they have not had one or more public transactions terminated for cause or default within the three-year period preceding the application. Bidders must sign and date the certification with their printed name and title. The form includes a checkbox option for bidders unable to certify to one or more statements to attach an explanation. False statements on this certification may result in rejection of the proposal, termination of the award, or criminal prosecution.
