PROPOSAL ITEM LIST
Salem VAMC, Salem, VA
Renovate Patient Privacy Wing-J; Project No. 658-20-101
PROPOSAL ITEM I (BASE PROPOSAL): The Contractor shall furnish all labor, materials, tools, equipment, transportation, and qualified supervision necessary to perform the general construction, alterations, and certain other items as graphically illustrated on the contract drawings, narratively described in the contract specification. The work is for the upgrading patient privacy rooms in Building 143 4th floor Wing-J. All work shall be performed at the Salem VA Medical Center, Salem, Virginia 24153.
During all phases of construction, the contractor will be required to maintain Life Safety passages per code as outlined in any phasing documents provided in the Final Construction Documents.
Infectious Control Risk Assessment (ICRA) containment systems shall be erected as required in and around the construction zone as required to contain any debris, dust and particulates caused by construction activities. Contractors will coordinate all activities with the Salem VAMC FMS Contracting Officer Representative.
Since the major expansion occurs over an occupied building, the Contractor shall plan to work weekends and off-hours for certain demolition and construction efforts being performed. Normal business hours for adjacent clinical operations are typically between 0700-1700 M-F. The contractor will be required to submit a detail phasing plan along with a construction schedule for approval by the Contracting Officer prior to commencing construction. This work will be constructed in successive phases as indicated on the specifications and drawings. Phasing dates shall be scheduled to ensure accomplishment of this work per plan, within the 420-calendar day period of performance, and that are mutually agreeable to the Medical Center Director, COR and the Contractor. Plans and specifications have been written with those requirements in mind. The VA is willing to discuss and consider alternate execution phasing so long as it does not prolong construction activities, increase cost to the project, or place any disruption to clinical service.
Other than unforeseen conditions driving phasing changes during the execution of the project, all proposed phasing changes must be agreed to by the VA prior to issuing Notice to Proceed.
If selected, the construction time allocated for completion of the work associated with Bid Item I
(Base Bid), as described above, is 420 calendar days from notice to proceed (NTP).
