About this file
This is an interoffice memorandum from the Orange County Transportation Authority (OCTA) regarding insurance and indemnification requirements for IFB 250089, a janitorial supplies procurement. The Materials Management Department requested a blanket purchase order for janitorial supplies as detailed in the associated Scope of Work. The contract has a one-year initial term with one one-year option term and is budgeted at $200,000.
The insurance requirements mandate that the selected vendor maintain Commercial General Liability coverage with minimum limits of $1,000,000 per occurrence and $2,000,000 general aggregate, including Products/Completed Operations and contractual liability; Automobile Liability Insurance with a combined single limit of $1,000,000 per accident covering owned, hired, and non-owned vehicles; Workers' Compensation coverage as required by the State of California with a Waiver of Subrogation; and Employers' Liability with minimum limits of $1,000,000 per accident and $1,000,000 per disease. All insurance must be primary and non-contributory to OCTA's coverage. Proof of insurance, including certificates and blanket additional insured endorsements, must be submitted to InsureTrack within ten calendar days of the agreement's effective date and prior to work commencement. The contractor must list Blanket Purchase Order Number B250089 and Senior Buyer Susan Rosenkranz on all certificates of insurance. The indemnification clause requires the seller to indemnify, defend, and hold OCTA harmless from all claims for losses, damages, bodily injuries, death, or property damage caused by the seller, its employees, subcontractors, or suppliers in connection with contract performance.
