About this file
This document is a price increase percentage form for a contract with the Arizona Department of Transportation (ADOT), specifically Solicitation No. BPM04763. The form provides guidelines for potential price increases during contract renewal periods, allowing the offeror to specify maximum percentage increases for up to four renewal periods.
The contract allows for incremental price increases, starting with a 2% maximum increase in the first renewal period and escalating to an 8% maximum increase in the fourth renewal period. All percentage increases are calculated against the original contract price, and ADOT emphasizes that price increases are not automatically granted. Vendors must provide documentation justifying the need for any price increase at the time of renewal. The document includes an example table demonstrating how the percentage increases would be applied across different renewal periods, with the original contract price serving as the baseline for calculations.
