2.1 Project Description

See Section C.

Phased Construction

1. Phase 0a: IT Re‐cabling.

2. Phase 0b: HVAC Replacement.

3. Phase 1: Remodel Record/Reception Area. Work includes installation of temporary reception counter and partitions; demolition and renovation of the divider wall between Reception and

Lobby; Remodel of the Reception Area including new casework, HVAC, power, and data; and demolition and installation of new entry doors and storefront.

4. Phase 2a: Temporary Work Stations. Work includes demolition of the Men’s locker/restroom and construction of new Temporary Work Stations and a breakroom.

5. Phase 2b: Medical Teaming Area. This phase can only be initiated upon full completion of Phase

2a. Work includes demolition of walls to expand Room 167, and a new partition to create a secure medical supplies room in Room 144. Work also includes new access doors and windows.

6. Phase 3: Work includes demolition of existing darkroom equipment and casework. Removal and infill with lead‐back gypsum drywall of the existing doorway. Installation of a new doorway from

Pharmacy Room 123 into Room 112.

7. CLIN 0002 Option: Phase 4 Laboratory Area. Work includes demolition and relocation of the East wall of Room 116, demolition of the East wall of Room 115, and an addition to the South wall of

Room 115.

8. Phase 5: Lobby and Signage. Work includes installation of a new directory kiosk at the entry, installation of three (3) pre‐cast concrete site signs, and installation of primary departmental signage in the Lobby. Is this an option?

9. CLIN 0003 Option: Expanded front entry. Work includes demolition of curbing, concrete sidewalk, and asphalt. Installation of new concrete drive, concrete curbing, bollards, and repairs to CMU columns.

2.2 Location

The work is located at the David C. Wynecoop Memorial Clinic in Wellpinit, Washington.

2.3 WORK RESCHEDULING

Normal duty hours for work shall be from 7 a.m. to 4 p.m., Monday through Friday. Requests for additional work time periods shall require written approval from the Contracting Officer 7 days in advance of the proposed work time period.

3.1 SPECIAL SCHEDULING REQUIREMENTS

a. Phase 2a must be ready for operation as approved by Contracting Officer before work is started on Phase 2b, which would interfere with normal operation.

b. The Medical Wing will remain in operation during the entire construction period of Phases 2a and 2b. The Contractor must conduct their operations so as to cause the least possible interference with normal operations of the activity.

3.2.1.1 Subcontractors and Personnel Contacts

Provide a list of contact personnel of the contractor and subcontractors including addresses and telephone numbers for use in the event of an emergency. As changes occur and additional information becomes available, correct and change the information contained in previous lists.

3.2.1.2 No Smoking Policy

Smoking is prohibited within and outside of all buildings. This applies to existing buildings, buildings under construction and buildings under renovation. Discarding tobacco materials is considered littering and is subject to fines.

3.3.2 Exclusionary Period

No work must be performed during the period December 19 to December 30, inclusive, without prior written approval of the Contracting Officer. This period has not been considered in computing the time allowed for the performance of this contract.

3.4 Occupied and Existing Building[s]

The Contractor shall be working in an existing health clinic which is occupied and operational. Do not enter the building[s] without prior approval of the Contracting Officer.

The health clinic and their contents must be kept secure at all times. Provide temporary closures as required to maintain security, and as directed by the Contracting Officer.

Provide dust covers or protective enclosures to protect existing work that remains and Government material during the construction period.

Relocate movable furniture as required to perform the work, protect the furniture, and replace the furniture in its original location upon completion of the work. Leave attached equipment in place, and protect it against damage, or temporarily disconnect, relocate, protect, and reinstall it at the completion of the work.

The Contractor will remove and relocate Government property in the areas of the building[s] scheduled to receive work. The Contractor will not move IT equipment.

3.5 Utility Cutovers and Interruptions

a. Make utility cutovers and interruptions after normal working hours or on Saturdays, Sundays, and Government holidays.

5.1 Infection Control Requirements

The Infection Control Officer or his/her designee shall approve projects involving manipulation of ceiling tiles, performance of dust generating activities, manipulation of heating, ventilation, and air conditioning (HVAC) systems, plumbing, and/or other maintenance repairs or modifications prior to the initiation of the project. See Attachment 12, Infection Control Risk Assessment Policy IC.2014.I7.0.

9.1 Requests for Information (RFIs) (After Award)

Immediately on discovery of the need for additional information from the Contract Documents, Contractor shall prepare and submit an RFI in the form specified.

Content of the RFI:

1. Date

2. RFI number, numbered sequentially

3. RFI subject

4. Specification section number and title and related wording

5. Drawing number and detail references as appropriate

6. Field dimensions and conditions, as appropriate

7. Contractor’s suggested resolution. If Contractor’s solution impacts the Contract time or

Contract sum, Contractor shall state impact in the RFI

8. Contractor’s signature

9. Attachments: Information necessary to fully describe item

Allow seven (7) working days for COR’s response for each RFI.

9.2 Project Meetings

Attend regular weekly or bi‐weekly meetings and conferences at Project site.

1. Agenda: Prepare a summary of work completed since the last meeting, and a summary of projected work to be completed. Provide an updated project schedule, status of submittals, RFIs, change orders, field observations, pending claims, and documentation of information for payment requests.

9.3 Preconstruction conference

The CO will schedule and conduct a preconstruction conference before starting construction, at a time convenient to the Owner and COR, but no later than 15 days after execution of the Agreement.

Attendees: COR, CO, consultants; Contractor and superintendent; major subcontractors; other concerned parties. Participants shall be authorized to conclude matters relating to the work.

10.1 Submittals and Submittal procedures

Submit required submittals in PDF format. Submit a schedule of required submittals, arranged in chronological order by dates required for construction per the schedule. Include time required for review, ordering, manufacturing, fabrication, and delivery. Include additional time required for making corrections or revisions to submittals noted by COR.

Submittal Review time: Allow 15 days for initial review. Allow additional time if coordination is required.

Allow 15 days for review of each resubmittal.

Include the following information on all submittals:

1. Project name, date, name of COR, name of Contractor, applicable subcontractor, supplier, manufacturer.

2. Submittal number

a. Submittal number shall reference applicable Specifications section

3. Number and title of Specification section

4. Drawing number and detail references

5. Location where product is to be installed

6. Other necessary documentation

Include a transmittal for submittals:

1. Project name, date, name of COR, Contractor, Firm who prepared submittal, subcontractor,

2. Category and type of submittal

3. Submittal purpose and description

4. Specification section number and title

5. Drawing number and details

6. Indication of full or partial submittal

7. Transmittal number, sequential

8. Submittal and transmittal distribution record

9. Remarks

10. Signature of transmitter

COR action on submittals includes the following choices:

1. No Exceptions Taken

2. Approved w/Exception

3. Information/Record

4. Revise and Resubmit

5. Rejected

Construction schedule: Submit a comprehensive, fully developed, horizontal, Gantt‐chart type, Contractor’s construction schedule within seven (7) days prior to the date established for commencement of work.

Update schedule monthly. Submit to COR.

10.2 Daily Construction reports

Prepare a daily construction report recording the following:

1. List of subcontractors on site

2. Count of personnel on site

3. Equipment on site

4. Material deliveries

5. Weather conditions and high and low temperatures

6. Accidents

7. Meetings and decisions

8. Unusual events

9. Stoppages, delays, shortages, losses

10. Meter and other readings

11. Emergency procedures

12. Orders and requests

13. Change orders

14. Services connected/disconnected

15. Equipment of system tests and startups

Waste must be disposed in a permitted landfill.

Upon project completion, submit one set of complete Record Drawings, including all changes and deviations from the contract documents, in PDF format for approval by the COR.

19) Demonstration/Training

Conduct training on‐site using approved operation and maintenance manual submittals. Provide instruction at mutually agreed on times.
